- Clauses applicable to this synopsis/solicitation Statement of Work RFQ # 2013-61831 This is a combined synopsis/solicitation for commercial services prepared in accordance with the format in FAR Subpart 12, Acquisition of Commercial Items, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation that will be issued. The solicitation number for this requirement is hereby issued as a Request for Quote (RFQ) using FAR Subpart 13.1, Simplified Acquisition Procedures. QUOTERS ARE ADVISED THAT AN AWARD MAY BE MADE WITHOUT DISCUSSION OR ANY CONTACT CONCERNING THE QUOTES RECEIVED. Quoters should not assume they will be contacted or afforded the opportunity to qualify, discuss, or revise their proposals. Therefore, the initial quote should contain the best terms from a technical and price standpoint. All quotes must include the full name, address, and DUNS number of the business to be named on the purchase order as well as the full name, phone number, and email address of the person who can accept the final purchase order. THIS IS A TOTAL SMALL BUSINESS SET-ASIDE. The North American Classification System (NAICS) code for this procurement is 541690 -Other Scientific and Technical Services- with a small business size standard of $14 Million. Quotes are due by Wednesday August 21, 2013, by 2:00 PM EST and should be sent to the attention of H. Dale Bish via email to hbish@cdc.gov. Hard copies can be delivered via Fed Ex or UPS to H. Dale Bish, Centers for Disease Control and Prevention, Procurement and Grants Office, 626 Cochrans Mill Road, Pittsburgh, PA 15236. The Offeror must cite RFQ Number 2013-61831 in the email subject line and include it on the shipping label if sending hard copies. Evaluation and award of the purchase will be performed in accordance with applicable sections of FAR Part 12, Acquisition of Commercial Items, and FAR Part FAR 13, Simplified Acquisition Procedures. The Offeror must have an active registration with the System for Account Management (SAM), which replaced the Central Contractor Registration (CCR), prior to award of the purchase order in accordance with FAR Clause 52.204-7. The government intends to issue a Firm Fixed Price purchase order in accordance with FAR Part 13. Proposals must be submitted in two parts: (1) Technical Proposal and (2) Pricing Proposal. After evaluation of the following technical factors, award will be made to the lowest priced technically acceptable offer. Each offer will be evaluated on a pass/fail basis, so offerors are cautioned that they should submit responses in sufficient detail to enable the government to evaluate their capabilities and experience. The Government anticipates multiple awards as a result of this solicitation. EVALUATION CRITERIA 1. The successful bidder(s) must possess a minimum of 10 years underground coal mining experience. 2. The successful bidder(s) must possess a minimum of 5 years of experience as a member of a mine rescue team. 3. The successful bidder(s) must have practical experience in mine rescue including: a. Exploration b. Mapping c. Gas detection d. Roof/rib control e. Ventilation f. Mine fire fighting 4. The successful bidder(s) must have knowledge of mine rescue rules and procedures. 5. The successful bidder(s) must have experience in development of mine rescue and emergency response training problems. 6. The successful bidder(s) must possess working knowledge of current federal mine safety and health regulations as prescribed in 30 CFR, Part 75. Questions must be submitted via email to H.
