MODIFICATION NOTICE:Bids are being solicited under solicitation number VA262-13-Q-1252. This announcement constitutes the only solicitation; bids are being requested and a written solicitation will not be issued. The solicitation is issued as an invitation for bids (IFB), unless otherwise indicated herein.If your company can provide the product/services listed on the solicitation and comply with all of the solicitation instructions, please respond to this notice. GSA Schedule Bids Only: Sellers bidding on this opportunity MUST have the items requested on an existing GSA Schedule. The Schedule must either be in the Seller's name or the Seller must be able to document its ability to act as an agent of a partner's Schedule. Sellers must not bid more than their applicable contract ceiling price, excluding the FedBid Fee, for contract-specific items. If FedBid receives notice that, due to inclusion of the FedBid Fee, the Selected Bid's line item pricing is higher than the Selected Seller's applicable published government contract pricing, the FedBid Fee will be reduced to ensure the Selected Bid's line item pricing does not exceed the Selected Seller's applicable contract pricing. Sellers may offer Open Market items only in accordance with the approved Terms and Conditions of their respective GSA Schedule AND upon approval from the soliciting Contracting Officer. This requirement is a [ Vetreran Owned Small Business ] set-aside and only qualified sellers may submit bids.The solicitation pricing on www.FedBid.com will start on the date this solicitation is posted and will end on 2013-08-15 16:00:00.0 Eastern Time or as otherwise displayed at www.FedBid.com.FOB Destination shall be in the Statement of Work. The VHA NCO 22 - 600 - Long Beach Healthcare System requires the following items, Meet or Exceed, to the following: LI 001: Ice Machine Maintenance, Repair, Cleaning and Sanitizing. The VA hospital wishes to provide high quality health care. One element of this care is to ensure that the ice being used by patients, staff and visitors is clean, healthy and free of impurities. The goal of this maintenance contract is to ensure that routine periodic maintenance meets this goal. Contractor shall provide all parts labor and materials required to perform the following periodic inspection, maintenance and repairs. This is an all-inclusive maintenance and repair contract, the ice machine will be maintained to run at optimum efficiency and in good condition. Contractor shall provide parts and repair the machines following the completion of the monthly service. All parts are included with the exception of the refrigeration compressor, evaporator and condenser coil, and the auger motor. The contractor is responsible for the condition of the equipment from the inlet of the pre-filter to the discharge of the condensate drain. The contractor shall perform the following; "A" list tasks as normal monthly service and the "B" list tasks as the quarterly tasks."A" tasks are performed 8 times per year, "B" tasks are performed 4 times per year. "A" list tasks: Contractor shall wipe clean the exterior of the unit. Replace the pre-filter. Sign and date the maintenance tag on the pre-filter housing unit. Vacuum clean the condenser coil. Empty the ice bin and wash the bin using Follett approved "SafeCLEAN Cleaner", FOLLETT part # 00132001. Inspect the Ice Machine for proper operation. "B" List Tasks: Contractor shall wipe clean the exterior of the unit. Replace the pre-filter with a new filter. Replace the post filter with a new filter. Sign and date the maintenance tag on the pre-filter housing unit. Vacuum clean the condenser coil. Follow the detailed instructions for cleaning and sanitizing the Ice Machine at HTTP:// FOLLETTICE.com. Use only NSF tested and certified cleaning and sanitizing products according to the manufacturer's instructions, such as Follett safeCLEAN cleaner (Part # 00132001) and NU-CALGON IMS-II SANITIZER. Service shall be performed during the3rd full week of each month, plus or minus 3 days. QASP: 1. When there are not more than 2 instances when procedures were not followed properly in a 6 month period, contractor is fully successful. 2. When the period of service is within 3 days of schedule, considered successful. 3. Reports must be turned in within 5 days of completion of service., 12, MO; Solicitation and Buy Attachments ***Question Submission: Interested offerors must submit any questions concerning the solicitation at the earliest time possible to enable the Buyer to respond. Questions must be submitted by using the 'Submit a Question' feature at www.fedbid.com. Questions not received within a reasonable time prior to close of the solicitation may not be considered.*** For this solicitation, VHA NCO 22 - 600 - Long Beach Healthcare System intends to conduct an online competitive reverse auction to be facilitated by the third-party reverse auction provider, FedBid, Inc. FedBid has developed an online, anonymous, browser based application to conduct the reverse auction. A Seller may submit a series of pricing bids, which descend in price during the specified period of time for the aforementioned reverse auction. VHA NCO 22 - 600 - Long Beach Healthcare System is taking this action in an effort to improve both seller access and awareness of requests and the agency's ability to gather multiple, completed, real-time bids.All responsible Sellers that respond to this solicitation MUST submit the pricing portion of their bid using the online exchange located at www.FedBid.com. There is no cost to register, review procurement data or make a bid on www.FedBid.com.Sellers that are not currently registered to use www.FedBid.com should proceed to www.FedBid.com to complete their free registration. Sellers that require special considerations or assistance may contact the FedBid Helpdesk at 877-9FEDBID (877-933-3243) or via email at clientservices@fedbid.com. Sellers may not artificially manipulate the price of a transaction on www.FedBid.com by any means. In addition to any VHA buy terms entered in FedBid, the Seller (Bidder) has reviewed and acknowledges any attached solicitation documents, item details, delivery schedule, and/or instructions to Seller (collectively referred to as requirements), and agrees to all terms and conditions set forth in the solicitation documents. While additional support documentation (e.g. Seller Attachments) may be requested, pricing shall be entered as a 'bid' in the reverse auction - offline pricing is not an acceptable bid. At the conclusion of the reverse auction, if attached to the buy, the apparent winning bidder shall be required to complete the solicitation documents prior to award. All questions will be submitted via fedbid by COB Friday 08-09-2013 4:00 EST. There will be no extension granted for the RFQ deadline. Award will be based on lowest price technically acceptable to meet or exceed the requirement per the statement of work.
