- RFQ 13-239-SOL-00034 Wage Grade 52.212-3 Reps & Certs Provision 52.212-1 This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation. Quotes are being requested and a written solicitation will not be issued. The current solicitation number is 13-239-SOL-00034 and is issued as a Request for Quotation (RFQ). This acquisition is being conducted under Simplified Acquisitions Procedures in accordance with FAR part 13. The clauses and provisions referenced in this solicitation may be obtained in full text form at http://www.arnet.gov/far. The Department of Health and Human Services/Indian Health Service has a requirement for an Preventative Maintenance on Generator for the Bemidji Area Indian Health Service. Delivery/ship to location: the Red Lake Indian Hospital, located in Red Lake, MN. Please see attachment for additional information. POC: Christopher.Millard@ihs.gov@ihs.gov. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular FAC 2005-66. The associated North American Industrial Classification System (NAICS) code for this procurement is 811310 with a small business size standard of $7.0.This requirement is a 100% Total Small Business set-aside and only qualified offerors may submit bid. BASIS FOR AWARD Quotation shall contain the offeror's best price and shall be determined to be the overall best value and most advantageous to the Government.
