- Business Management Questionnaire - pdf Business Management Questionnaire - Word Solicitation Document This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; offers are being requested and a written solicitation will not be issued. The solicitation number is RC0011-13 and this solicitation is issued as a Request for Proposals (RFP). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-69. The North American Industry Classification System code is 562920. This requirement is unrestricted. UNICOR, Federal Prison Industries, Inc., intends to enter into a six month, firm-fixed-price, indefinite delivery/indefinite quantity contract for the following commercial item(s): GLASS AND COMPUTER MONITOR RECYCLING Contractor shall recycle whole cathode ray tubes (CRTs). Whole, bare CRT tubes are generated by UNICOR factories at the following locations: ATWATER, CALIFORNIA (ARRC) FORT DIX, NEW JERSEY (DXRC) LEAVENWORTH, KANSAS (LVRC) LEWISBURG, PENNSYLVANIA (LERC) MARIANNA, FLORIDA (MNRC) TEXARKANA, TEXAS (TERC) TUCSON, ARIZONA (TURC) Whole, bare CRT tubes shall be recycled in accordance with all federal, state and local regulations, as well as consistent with the requirements of the Responsible Recycler (R2) program. Contractor must be able to meet the down stream due diligence requirements of the R2 (Responsible Recycling) Program.
