Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. (ii) Solicitation VA258-13-Q-0906 is issued as a request for quotation. (iii) This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-36. (iv) This procurement is being issued using "Cascading Procedures." Reference the below listed Evaluation Addendum detailed in paragraph ix(b), for complete guidance on order of preference. Accordingly, all business concerns may submit offers for this requirement, but they will only be considered if and when the category of preference above your firm has been eliminated from consideration. The NAICS code is 811310 with a small business size standard of $7M. (v) CLIN 0001a thru CLIN 0005B. (vi) The Contractor shall furnish all management, supervision, trained labor with necessary certification for covered equipment, parts, travel, transportation, materials and all other required items necessary to perform all operations in connection with the project to provide preventative maintenance and on-call repair services, for "Elevator Maintenance and Support" services at the Phoenix Veterans Affairs Health Care System (PVAHCS). Work will consist of complete PMA and repair services for (20) elevators at the Carl T. Hayden Medical Center. Currently there are three (3) of the 20 elevators that are being built but will require maintenance and support upon completion (Oct/Nov 13). These services are required to provide continued elevator transportation for veterans. The contractor shall comply with all applicable current and future laws and regulations related to the repair and maintenance of all elevators. 2.0 Tasks: The contractor shall provide all personnel, materials, and equipment required for tasks outlined in this performance work statement (PWS). 2.1 Motion Controlled Traction kone elevators: The contractor shall provide maintenance and support for the following elevators: "S1, S2, S3, S4, S5 elevators are located in building 1 "S6, S7, and S8 elevators are located in building 16 "Provide 24/7 support and must respond to service calls within 2 hours. Note: There will be no overtime authorized for the maintenance and support of these elevators. 2.2Motion Controlled EECO Single Piston Elevators : The contractor shall provide maintenance and support for the following elevators: "S9 is located in building 16 "F13 Freight elevator is located in building 2 "S16 PKE located in building 34 Rehab building "S17 Future Elevator located in building 34 Rehab building "Provide 24/7 support and must respond to service calls within 2 hours. Note: There will be no overtime authorized for the maintenance and support of these elevators. 2.3Single Piston Hydraulic Ottis Elevators: The contractor shall provide maintenance and support for the following elevators: "S14 and S15 are located in Mental Health building "Provide 24/7 support and must respond to service calls within 2 hours. Note: There will be no overtime authorized for the maintenance and support of these elevators. 2.4 Hydraulic/Kone/Montgomery Elevators "S10, S11, and F12 are located in building 8 "Provide 24/7 support and must respond to service calls within 2 hours. Note: There will be no overtime authorized for the maintenance and support of the T1, T3, T6, and S1 and there will be no additional costs to the Government for these elevators. 2.4Otis Traction Food Service Elevators T1 and T3 are located in building 1 T6 is located in building 16 Ensure 24/7 support is available for elevators T1, T3, T6, and S1 and must have 2 hour response time. Note: There will be no overtime authorized for the maintenance and support of the T1, T3, T6, and S1 and there will be no additional costs to the Government for these elevators. 2.2 Elevator Maintenance Support: The contractor shall do the following: "Provide two (2) hour response time during regular working hours (7:30am - 3:30pm Monday through Friday. "Ensure any bank of elevators that does not have at least one elevator running will be covered 24/7 when called in for service "Ensure C&D Wing elevators (S1, S2, and S3) have 2 of 3 elevators running at all times and will be covered 24/7 if called in for service. Ensure 24/7 support is available for elevators T1, T3, T6, and S1 and must have 2 hour response time. Note: There will be no overtime authorized for the maintence and support of the T1, T3, T6, and S1 and there will be no additional costs to the Government for these elevators. 2.5Support for Damage or Abuse of Elevators "Unforseen damages to elevators will be covered accept in extreme circumstances where proven to be abuse the medical center will be given detailed quotes of parts and labor needed. Work must be authorized in writing prior to work being done with pricing IAW the quoted labor rate and parts mark-up percentage. "All repair and maintence support for abuse must be authorized by the Contracting Officer Representative (COR) and approved by Contracting Officer. Contractor shall perform these commercial services IAW with all federal, state, county, local, and VA requirements. (vii) Period of Performance: There shall be a base contract period with four additional option years available for continuation of services. (viii) The provision at 52.212-1, Instructions to Offerors -- Commercial, applies to this acquisition. (ix (a)) 52.212-2, Evaluation -- Commercial Items applies. Quotes will be evaluated based on prices offered from vendors certified to work on the equipment. (ix (b)) Addendum to 52.212-2 Evaluation-Commercial Items (JAN 99) - Method of Award - "Cascade Procedure" - As follows: 1. Any award(s) resulting from this solicitation will be made using the following cascading order of precedence: 1.1. In accordance with FAR Subpart 19.14 and PL 109-461, award under this solicitation will be made on a competitive basis first to an eligible Service Disabled Veteran Owned small business (SDVOSB) concern (see Section K, FAR 52.219-1, "Small Business Program Representations" for small business size definition) who submits a responsive offer, provided that there is adequate competition among such firms. 1.2. If there is inadequate competition for award to a SDVOSB concern, award will be made competitively to an eligible Veteran Owned Small Business (VOSB) concern. 1.3. If there is inadequate competition for award to a VOSB concern, award will be made competitively to a certified HubZone small business concern. 1.4. If there is inadequate competition for award to a HubZone concern, award will be made competitively to a Women-Owned small business concern. 1.5. If there is inadequate competition for award to a Women-owned concern, award will be made competitively to a small business concern IAW FAR 19.5. 1.6. If there is inadequate competition for award to a small business concern, award will be made on the basis of full and open competition considering all responsive offers submitted by responsible business concerns. 2. Adequate competition shall be deemed to exist if- 2.1 At least two competitive offers are received from qualified, responsible business concerns at the tier under consideration; and 2.2. Award will be made at fair market prices as determined in accordance with FAR 19.202-6. 3. When there is inadequate competition at any tier, an otherwise competitive offer from a responsible business concern at that tier will be considered with the eligible offers for the next lower tier. For example, only one competitive offer is received from a SDVOSB business. That offer will be included with the offer(s) received from all VOSB businesses. If there is still inadequate competition, the offer from the SDVOSB business will then be included with SB offer(s) received; finally, if there is inadequate competition at the SB tier, the SDVOSB offer will be included with all responsive offers. 4. Award will be made to the offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. 5. Pricing will be firm fixed price for each line item identified.
