- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6 as supplemented with additional information included in this notice. Request for Quotations number is HSCG49-13-Q-AU062 and is issued as a request for quotations (RFQ). This announcement constitutes the only solicitation; quotations are being requested and a written solicitation will not be issued. This Request for Quotations incorporates provisions and clauses in effect through Federal Acquisition Circular 2005-65 dated January 29, 2013. The NAICS Code is 423430. The small business size standard is 1,000. This is an unrestricted procurement; all responsible sources may submit proposals that will be considered by the agency. The contract will be awarded on a firm-fixed-price basis using simplified acquisition procedures in accordance with FAR Part 13.5. Mobile Ambulance Simulator: A realistic ambulance patient care area configuration commonly found in the industry. It shall have the standard stretcher loading parameters in relation to height off the ground, stretcher securing apparatus and space to deliver patient care. The seats inside the ambulance shall be consistent with the standard bench and chair seat found in the industry. This simulator shall be capable of being towed. It shall be safe to tow behind a vehicle and be street legal. The trailer shall have break and light hook ups normally found on a trailer of the size and weight. It shall be weather proof as a normal ambulance would be. It shall have an instructor viewing window (48" x 24") on the right side of the simulator behind where the providers would be sitting. This view window will be big enough to allow multiple instructors to view at the same time. It shall have a sound system and hydraulic system to simulate road conditions and noise. It shall have back and side entry as normally found in an ambulance. It shall have all the lights a normal ambulance would have inside and out. It shall simulated oxygen and suction unit inside the ambulance as found in a typical ambulance. The simulator shall have all compartments, inside and out, that a normal ambulance would have. The unit shall provide adequate air conditioning. The unit shall have a generator that will be able to supply power to all systems. It shall have a power shore tie that will allow power to be provided on an 110V circuit and if needed converted to 12V with the proper safety breakers located in the unit being able to provide power to all systems. It shall have a switch to accommodate switching between shore and generator power. The trailer shall have the appropriate ball jack needed to load on and off a vehicle. The unit shall be white in color and have markings consistent with a normal ambulance on the exterior and interior. (7) Offerors are instructed to include a completed copy of Federal Acquisition Regulation (FAR) 52.212-3, "Offerors Representations and Certification--Commercial Items (DEC 2012)" and Alt 1 included with their quotation (the provision is attached), or be registered with Online Reps and Certs, https://www.SAM.gov. 3. FAR 52.212-2 Evaluation-Commercial Items (Jan 1999). - This is a commercial item acquisition. The evaluation and award procedures in FAR 13.106 apply. The Government will award a purchase order resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The Government will evaluate offers and make award to the lowest priced technically acceptable offer that is responsive to the terms of this solicitation. 4. FAR 52.212-3 Offeror Representations and Certifications-Commercial Items (NOV 2012) - An offeror shall complete only paragraphs (b) of this provision if the offeror has completed the annual representations and certificates electronically at http://orca.bpn.gov. If an offeror has not completed the annual representations and certifications electronically at the ORCA website, the offeror shall complete only paragraphs (c) through (o) of this provision which can be obtained electronically at http://www.arnet.gov/far. 5.
