Description
Notice of Intent to Sole Source Assuring feasibility, reliability, validity and usability of two oral health electronic clinical quality measures Pursuant to the authority of FAR 6.302-1, the Department of Health and Human Services (DHHS), Program Support Center, Strategic Acquisition Service, Division of Acquisition Management, intends to negotiate a sole source, firm fixed-price contract with the American Dental Association, Dental Quality Alliance, 211 E. Chicago Avenue, Chicago, IL 60611, on behalf of the Office of the National Coordinator for Health IT (ONC). The period of performance for the award will be 12 months from the effective date of the contract. ONC provides counsel to the Secretary of HHS as well as federal department and agency leadership for the development and nationwide implementation of an interoperable health information technology infrastructure. To date, much of this work has focused on medical care not including dental and oral health. To enable availability of dental and oral health clinical quality measures (CQMs) and electronic clinical quality measures (eCQMs) suitable for use in the focus of quality measurement and improvement initiatives such as the Medicare and Medicaid Electronic Health Records (EHR) Incentive Programs, the Centers for Medicare and Medicaid Services (CMS) asked the American Dental Association (ADA) to lead the development of a broad coalition of parties that would participate in dental quality measurement. The ADA subsequently established the Dental Quality Alliance (DQA). The DQA now is a multi-stakeholder Alliance comprised of stakeholder organizations from across the oral health community; including federal agencies, payers, and professional associations. The DQA has a mission to advance the field of performance measurement to improve oral health, patient care, and safety through a consensus building process. DQA has achieved unparalleled breadth and depth of participation across key oral health professional and other stakeholders organizations as well as thought leaders and individual professionals with unique expertise in: dental health information technology; dental and oral health care delivery and best practices; measurement of oral health care quality; electronic clinical quality measurement; and dental and oral health care coding systems. Since its official inception in 2010, the DQA has identified a comprehensive set of pediatric oral health care quality measures aligned to national goals that will enable dentistry to systematically assess oral healthcare utilization, cost, and quality in both public and private sectors. In line with the mission of the DQA, these measures were developed through a voluntary consensus process and have undergone rigorous assessment but have not been fully tested for feasibility, reliability, and validity as electronic clinical quality measures implementable in EHRs and fully aligned with eCQM standards. Realizing the potential of EHRs to enable real-time quality measurement and improvement, the DQA has led much of the early efforts to explore the realm of electronic clinical quality measures (eCQM's) in dentistry. Additionally, the Medicare and Medicaid EHR Incentive Programs have sparked great interest in implementation of EHRs for dentists as well as physicians and hospitals. The Office of the National Coordinator's (ONC's) purposes for this contract are: 1. To develop and test electronic Clinical Quality Measures (eCQM) related to pediatric oral healthcare. 2. Coordinate a select team of experts, researchers, EHR vendors, and measure developers to develop and test the oral health eCQMs. 3. Promote consensus development and broad acceptance of oral health measures by healthcare professionals, assuring oral health eCQMs are available for consideration for inclusion in CMS' Meaningful Use incentive programs for eligible providers. 4. Promote the implementation of these measures in Electronic Health Records (EHRs) for overall better oral health care delivery. 5. To receive a final set of specified oral health electronic clinical quality measures. ONC's objectives are: • Coordinate and lead development of an efficient, sustainable process to assure presence of eCQMs in dentistry and oral health. • Prepare an appropriate measure testing plan, which shall address at a minimum: o Testing measure feasibility in electronic health records (EHRs). o Testing for measure reliability. o Testing for measure validity. • Using the testing plan, and conducting any further measure development that may be necessary based on the results of testing, establish feasibility, reliability, validity and usability of two Dental Quality Alliance (DQA) eCQMs for potential use in the EHR Incentive Program for Meaningful Use Stage 3. The identified dental eCQMs are: o Care Continuity: Children who receive a comprehensive or periodic oral evaluation in two consecutive years o Sealant Use: Children aged 6-9 years who receive sealants in the first permanent molar • Achieve standardization and alignment with other eCQMs (oral health and non-oral health). This acquisition requires specific breadth and depth of expertise and capability for stakeholder engagement in the area of quality measurement and improvement in the fields of dentistry and oral health care. To date, much of quality measurement and improvement efforts have focused on medical and clinical care provided by physicians and hospitals. However, dental and oral health care have unique characteristics and require expertise in aspects of clinical practice, clinical quality, quality measurement, and health information technology that are unique to dental and oral health. This acquisition requires knowledge and expertise in the specific field of electronic clinical quality measures in oral health care. The Government believes that DQA is currently the only entity able to provide all the knowledge, expertise, and stakeholder engagement to assure successful testing and acceptance of measures finalized through the eCQM testing activities required under this contract. The DQA is the only comprehensive multi-stakeholder collaborative quality measure developer in dentistry and is positioned to coordinate and lead measure development through its experience, expertise, and support in the oral health quality. By leading the oral health quality measurement enterprise through collaboration and consensus development the DQA would not only develop the measures but also engage the dental and oral health communities to begin to adopt measures as a path towards quality improvement. Thus, the DQA is uniquely positioned to test feasibility of measures and to assure acceptance of oral health CQMs by the oral health professionals to whose practice these measures apply. This is not a solicitation for competitive proposals. However, parties interested in responding to this notice shall submit technical data, including cost, sufficient to determine capability in providing the same services. A determination by the Government not to compete this requirement based upon responses to this Notice is solely within the discretion of the Government.
