Description
THIS ANNOUNCEMENT CONSTITUTES THE ONLY SOLICITATION AND A SEPARATE SOLICITIATION WILL NOT BE ISSUED. This combined synopsis/solicitation for commercial services prepared in accordance with the format in FAR 13.5. The incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-69 (1 August 2013). This announcement constitutes the only solicitation. Proposals are being requested and a written solicitation will not be issued. The solicitation number for this acquisition is NIHCC-13011635 AND IS BEING ISSUED AS A Request for Quote (RFQ). The NAICS Code for this solicitation is 621111 and the Small Business Size Standard is $10.0 million. This action is being solicited as a Total Small Business Set-aside. The National Institutes of Health (NIH) is the nation's foremost federally funded biomedical research institution. It is comprised of twenty-seven Institutes, Centers and Divisions. The NIH Clinical Center is the on-site 242-bed hospital for the NIH campus comprised of clinical, laboratory and administrative staff who support the Institutes as they conduct clinical research working with inpatients and outpatients participating in research studies at the NIH. Whereas the Clinical Center has a clinical support team to address a broad variety of routine medical issues that may arise during a course of treatment, in some instances, a specialist is required for emergency consultations. The NIH does not have a full-time orthopedic surgeon on staff, necessitating a formal agreement with external contractors to provide emergency, urgent and routine orthopedics consultations. The contractor shall provide Routine, Urgent and Emergent Orthopedic care as determined by the attending physician. Services rendered will include 24/7 routine, urgent and emergent consultation on site. The contractor will discuss the case with the attending physician and will provide follow-up that meets patient care requirements. Such care may be provided on-site or, (in instances in which transport of the patient is appropriate), may be provided in a nearby clinic or office setting. The contractor shall perform consultative services onsite at the NIH or in their local office and work at the direction of the patient's NIH attending physician. The contractor shall discuss the case with the NIAMS physician and is expected to respond in person within two hours of notification. Proximity to the NIH is important to facilitate a two hour response. If surgery is required, the preferred location for surgery is within the designated operating suites and clinics in the NIH Clinical Center. However, if the surgery is beyond the capabilities of the NIH, surgery can be performed at a hospital that has the necessary orthopedics equipment and supplies to promote patient safety. If the patient requires urgent surgery, the attending physician will discuss the case with the contractor and the NIH anesthesiologist on call to see if the surgery can be performed at the Clinical Center. If the procedure cannot be performed safely at the NIH, the contractor will coordinate with the attending physician to arrange for the transfer of the patient to a nearby hospital where the contractor has privileges for surgery.  Surgical patients shall also be seen by the contractor for a medically appropriate number (minimum of one visit) of outpatient visits to assess post-surgical progress. Follow-up care for postoperative patients will meet the community standard for the provision of orthopedics care.  The contractor is required to document all care provided in the patient's medical record. The contractor must be a skilled orthopedics Board Certified Orthopedic Surgeon with at least five years post-residency experience. Must have familiarity with orthopedic oncologic procedures as well as experience performing procedures on pediatric patients who have genetically determined skeletal disease or bone dysplasia. The contractor shall be a licensed in a state, territory or commonwealth of the United States or District of Columbia. The contractor shall be board certified in Orthopedic Surgery. The contractor shall provide personal medical liability insurance at the standard $1 million per claim/$3M aggregate. The contractor shall indemnify the Government for any liability producing act or omission by the contractor during the contract performance. Contractors shall not be considered at any time employees of the Federal Government for any purpose. The contractor shall comply with all hospital policies and requirements established by the Joint Commission on the Accreditation of Healthcare Organizations. http://www.jointcommission.org/ The contractor must provide necessary credentialing documents and references for privileges in the Clinical Center. The contractor shall provide proof of a negative TB test, and documentation of Hepatitis B vaccination before reporting to work at the Clinical Center. The contractor shall complete appropriate Clinical Center competencies. The contractor shall be available to provide routine, urgent or emergent orthopedics consultations 24/7. The contractor may utilize other members of his/her practice to provide coverage when the primary contractor is out of town to assure proper management of CC patients and to maintain the required two hour response time. Neither the contractor nor approved subcontractor shall refer to the NIH or any of its affiliates, employees, agents, or medical or research staff in any press release or promotional statements without the prior written approval of the NIH. The NIH shall not refer to the contractor or subcontractor or any of its affiliates, employees, agents, or medical or research staff in any press release or promotional statements without the prior written approval of the contractor and/or subcontractor. Evaluation Criteria 30% Approach: Proposal meets requirements and indicates an exceptional approach and understanding of the requirements. Strengths far outweigh any weaknesses. Risk of unsuccessful performance is very low. 30% Staffing: Proposed staff meets the requirements (experience and credentialing) as stated in the Statement of Work. Adequacy to meet the 24/7 availability requirement. Corporate Experience 25% The proposal should focus on the contractor's prior experience in providing support. The information shall include sufficient information to demonstrate previous effectiveness of work performed. 15% Past Performance: The quotation will be evaluated on 3 recent references (within the last 5 years.) Price Best Value The government will evaluate the total price contained in the contractor's proposal. 250 hours are anticipated on an annual basis quotations are to cite an hourly on-call rate. Travel Travel expenses will not be reimbursed. Additional Fees Fees for surgeries and outpatient visits will be based on a fee schedule that is 1.5 times the prevailing Medicare rate for each CPT code. The contractor will bill directly to the Institutes requesting the care and services. The solicitation shall include pricing for the base year and options priced separately.
