- Solicitation W912P4-13-R-0009 MMD Drain Cleaning 1. The Government is soliciting businesses to accomplish the work described in the scope of work for Mount Morris Dam, Drain and Pressure Cell Cleaning, Mount Morris, New York. 2. Estimated magnitude of this project is between $250,000 and $500,000. 3. This solicitation is being advertised under North American Industry Classification Systems (NAICS) Code is 562998 All Other Miscellaneous Waste Management Services. Please ensure this NAICS code is incorporated in your current SAM profile at www.sam.gov. Failure to have this NAICS code current in SAM may result in not being considered for the award. Please make sure that certifications and representations are up to date at www.sam.gov. 4. Wage determination is General Decision Number WD 05-2381. Visit http://www.wdol.gov, Service Contract Act, for the wage determination. 5. The Government anticipates to award a Firm-Fixed-Price service contract to the lowest price technically acceptable offer. In order to be deemed technically acceptable, the Offeror must demonstrate their capabilities in accordance with Section 8.1 of the Scope of Work. 6. Please see Section 8.2 of the Scope of Work for details pertaining to the Period of Performance. 7. A Pre-Bid site visit is scheduled for 22 August 2013 at 10:00am. Meet at U.S. Army Corps of Engineers, William Hoyt II Visitor Center, 6103 Visitor Center Road, Mount Morris, NY 14510. 8. Contractors are highly encouraged to attend the pre-bid site visit. 9. Please submit proposal via e-mail to: oksana.strekha@usace.army.mil with cc: jeffrey.g.ernest@usace.army.mil by 14:00 EST 30 Aug 2013. 10. Proposal Submittal shall include the documents listed below: - Offerors shall provide a company cover sheet with their respective information to include the name and title of person authorized to negotiate and sign on behalf of the Offeror; CAGE Code: DUNS Number; POC's Email Addres; - Completely filled out and signed SF-1449, including all required clauses and complete pricing based on the line items; - Completed Bid Schedule (Attached in the Scope of Work); - Supply the names, locations, and client contact information for two (2) previous foundation drain cleaning projects completed within the last five (5) years where drains of a minimum 30 foot embedment depth into bedrock and at minimum 2" in diameter where cleaned under the method outlined in the Statement of Work. Provide a bid submittal with a summary of the previous work including equipment and methods used, as well as references. Include a bid submittal for what equipment shall be used on the work in the Statement of Work. The narrative portion of the proposal shall not exceed fifteen (15) pages; 11. All interested contractors must be registered in System for Award Management (SAM) at https://www.sam.gov/portal/public/SAM to be eligible for award of Government contracts.
