- SOW - Fire Alarm Vendor Questionnaire W-9 THIS IS A SUBCONTRACTING OPPORTUNITY WITH ADAMS & ASSOCIATES, INC., AND NOT WITH THE USDOL Scope of Work ID# LRJC13-012 FIRE ALARMS To complete the following Install manual fire alarm pull stations in the common areas in the natural egress path within 5' of the exit doors in Dormitory, Building 6922 and Building 6926. The project will be performed at the Little Rock Job Corps Center, 6900 Scott Hamilton Dr., Little Rock, AR 72209. Deficiency #91576 and Deficiency #91571. Table of Contents I. Proposal Requirements Project: Install manual fire alarm pull stations at the common areas of the dormitory, buildings 6922 and 6926. Owner: US Department of Labor (DOL) Employment and Training Administration Center Operator: Adams and Associates, Inc Mailing address Little Rock Job Corps Center 6900 Scott Hamilton Dr. Little Rock, AR.72209 Proposal Due Date: 5 PM CST on September 5, 2013 Evaluation Criteria Point Value PRICE: Responsible Contractor who can perform effective services at the lowest reasonable price. 60 QUALITY: Demonstrated experience and knowledge capabilities; Technical Excellence; Management Capabilities; Personnel qualification; Quality of the Tender packages; Technical excellence including where necessary, technical drawings, blue prints. 20 PAST PERFORMANCE: Past services to center or list of references for verification purposes. 10 SCHEDULE: Detailed explanation on how services will be carried out and the delivery time frame. 10 TOTAL POINTS 100 Payment: Payment Terms are Net 30 Days of completion and receipt of invoice. Site Visit Date: August 29, 2013 at 9:00 AM. Contractor must visit the site and familiarize himself with the condition under which the work will be performed. No allowance will be made for any error on the contractors part for failure to take into account any conditions or circumstance existing at the site which might affect the contemplate work. The fact that a bid is submitted with ID # LRJC13-012 will be taken as evidence that the contractor has full knowledge of the extent and character of the contemplate work and any problem to be encountered in performing same. II. Project Description Scope of Project: Manual fire alarm pull stations are not provided at the common areas of the dormitories. NFPA 101 requires these to be provided in the natural exit egress path near each required exit from an area. This project shall be performed by a qualified fire alarm contractor who is familiar with the components of the building fire alarm system. The building FA system is Gamewell with FA control panel Gamewell FCI 7100. Installed materials shall be 140' of ½" EMT and 150' of fire alarm cable. III Specific Requirements The proposal shall include materials and supplies, permits, inspections, and labor to install two manual fire alarm pull stations per building in the common areas of buildings 6922 and 6922, in the natural egress path within 5' of the exit doors. Installed materials shall be 140' of ½" EMT and 150' of fire alarm cable. •Patch and paint the wall where disturbed. •Paint the conduit (if required surface mounted installation, which is not welcome and only if necessary) to match the existing wall and ceiling colors. Warrantees: The contractor is to provide a written warranty for the workmanship and materials at the time of completion of the job. A minimum 5 years manufacturer warranty is required for vendor provided materials, and a minimum one year warranty is required for installation. IV Construction Requirements Center Regulations: Contractor and his employees shall abide by all of the Center's rules and regulations. There shall be no fraternization between contractor personnel and Job Corps Center students or personnel. The Contractor will be required to follow all Center safety and security requirements. Management of Jobsite: •The Contractor shall maintain a clean and safe workplace that is secured at the end of each working day. •The Contractor shall clear all deliveries with the Center's security personnel. Wage Rates: The bidders shall conform to the latest Davis-Bacon prevailing wage rate decisions for the area. (See the attached prevailing wage rate) ELEC0295-002 01/01/2013 ARKANSAS - Pulaski County ELECTRICIAN (Including HVAC, electrical and temperature controls; Excluding low voltage wiring for internet satellite and video/voice data) Electrical contracts over Rates Fringes $1,500,000........................$ 25.44 9.57 +2% Electrical contracts under $1,500,000.........................$ 23.62 10.43 Insurance: The Contractor shall obtain and maintain insurance as follows: Workman's Compensation/Employers Liability insurance, to meet Arkansas statutory requirements Comprehensive General Liability ($1,000,000 per occurrence) Comprehensive Automobile Liability ($1,000,000/combined single limit) Fire Legal Liability ($500,000 per occurrence) The Contractor must cover any deductible costs. Upon contract award, the Contractor must add Little Rock Job Corps Center and Adams and Associates, Inc., operator of the Little Rock Job Corps as additional named insured to the policies. Schedule: The Contractor shall submit a schedule for the work. The project shall be completed within 60 calendar days of the award of the contract. Written authorization is required to extend the 60 day completion requirements. Final Acceptance The Contractor shall notify the Center's facility manager when the project is substantially complete, and shall request an inspection. The completion of this inspection shall determine the date for the commencement of the warrantees, and the date for payment. V. Proposal Form The Contractor's proposal must be submitted on company letterhead, must reference the proposal ID# LRJC13-012 and address all items in section II and III, to be considered. Bids must be valid for 90 days. The Contractor shall submit the following information with the proposal: •A list of materials and product data sheets (MSDS) proposed for use. •References •Copies of Insurance Certificates and applicable licenses. •Completed vendor questionnaire and W9. All questions must be in writing and sent to the Purchasing Agent Smith.Carlas@jobcorps.org. No Phone Calls will be accepted.
