- Vendor Questionnaire W-9 Scope of Work - Carbon Monoxide Alarms THIS IS A SUBCONTRACTING OPPORTUNITY WITH ADAMS & ASSOCIATES, INC., AND NOT WITH THE USDOL Scope of Work LRJC13-011 Carbon Monoxide Alarms To complete the following CARBON MONOXIDE ALARMS - ID # LRJC13-011 Install 120V single-station CO alarms with battery backup and integral detection in designated locations in buildings 6900, Admin/Medical Dental, building 6904, Hard Vocations, building 6908, Cafeteria, building 6914, Soft Vocations, building 6918, Maintenance, and building 6912, Recreation. The project will be performed at the Little Rock Job Corps Center, 6900 Scott Hamilton Dr., Little Rock, AR 72209. Deficiencies #92816, 92817, 92818, 92819, 92820, 91597 Table of Contents I. The installation shall be hard wired power from the nearest 120V un-switched lighting or receptacle circuit using 3 #12 THHN/THWN in ½" EMT or short runs of MC Cable where applicable. Contract award will be based on quality, experience, expertise and price. Payment: Payment Terms are Net 30 Days of completion and receipt of invoice. Site Visit Date: August 29, 2013 at 9:00 AM Contractor must visit the site and familiarize himself with the condition under which the work will be performed. No allowance will be made for any error on the contractors part for failure to take into account any conditions or circumstance existing at the site which might affect the contemplate work. The fact that a bid is submitted with ID # LRJC13-011 will be taken as evidence that the contractor has full knowledge of the extent and character of the contemplate work and any problem to be encountered in performing same. II. Project Description The identified buildings contain fuel using equipment and appliances with carbon monoxide (CO) as a by-product. A cracked heat exchanger or failed venting of the equipment or appliances could allow CO to enter the air supplied to occupied spaces. FPA 720 requires installation of CO detectors in sleeping areas required where a 75-decibel (75dB) signal volume can be heard at the pillow. CO detectors are also required on the ceilings in rooms where there is a fuel-burning appliance within the room or in rooms serviced by the first supply air register from a centrally located fuel-burning heating ventilation and air conditioning (HVAC) system. CO detectors are required in spaces that contain permanently installed fuel-burning appliances, including kitchens, laundry rooms, maintenance or mechanical rooms, and boiler rooms. (NFPA 720, 2012, 9; DOL Directive Information Notice 12-27). Note: A packet containing the exact location of all project deficiencies will be given out to all at the pre-bid walk-thru, which explains in detail the work to be completed. Scope of Project: To install 120 V single-station CO alarms with battery backup and integral detection in designated locations. Installation shall require hard wired power from the nearest 120V un-switched lighting or receptacle circuit using 3 #13 THHN/THWN in ½" EMT or short runs of MC Cable where applicable. Plug-in type units are acceptable. The alarms are to be installed significantly away from airborne particulates from such things as air supply registers. When the alarms are installed, the tag on the back must be marked according to the manufacturer's requirement for the required replacement period. III Specific Requirements The proposal shall include materials and supplies, permits, inspections, and labor to install 120V single-station CO alarms with battery backup and integral detection. Installation for each building is outlined below: •1. Deficiency# 92816 - Building 6900 - Admin/Medical/Dental Install (13) 120V single-station CO alarms with battery backup and integral detection. Provide one in each space containing the CO source equipment and appliances (one in each mechanical room, total of three). Install one in the room or occupied spacing having the closest supply register (Reception 100), Records (142), Copy Room (141), Conference Room (121), Records (125) Center Director (118), Women's Ward Room (164), Exam Room (166), and Corridor (163) to associated fuel using mechanical equipment measured along the duct from the unit. If there is more than one such register at equal distance, there should be one alarm for each such room or occupied space (storage room 103). •2. Deficiency# 92817 - Building 6904 - Hard Vocations Install (11) 120V single station CO alarms with battery backup and integral detection. Provide one in each space containing the CO source equipment and appliances (one in each of three mechanical rooms, and one in each of four shops, total of seven). Additionally install one in the room or occupied space having the closest supply register (one in each of four classrooms ) to associated fuel using mechanical equipment measured along the duct from the unit. •3. Deficiency# 92818 - Building 6908 - Cafeteria Install (5) 120V single station CO alarms with battery backup and integral detection. Provide one in each space containing the CO source equipment and appliances (one in the mechanical room, one in the main kitchen, one in the Culinary Arts kitchen, and one in the janitor/laundry room ( 519 ) a total of four ). Additionally install one in the room or occupied space having the closest supply register ( serving line - 530 ) to associated fuel using mechanical equipment measured along the duct from the unit. •4. Deficiency# 92819 - Building 6914 - Soft Vocations Install (19) 120V single station CO alarms with battery backup and integral detection. Provide one in each space containing the CO source equipment and appliances (one in each of four mechanical rooms, one in the electrical room (200) a total of five ). Additionally install one in the room or occupied space having the closest supply register ( adv 287 ), HS Online 283, corridor (293), CN1 (284), Admissions Office (280), Women's Restroom (201), Testing (233), Counselor's Office (234), Lounge (218), Copy Room (219), High School On-line (256) to associated fuel using mechanical equipment measured along the duct from the unit. If there is more than one such register at equal distance, there should be one alarm for each such room or occupied space (corridor 293), Counselor 236, and CPP 237. •5 Deficiency# 92820 - Building 6918 - Maintenance and Storage Install (5) 120V single station CO alarms with battery backup and integral detection. Provide one in each space containing the CO source equipment and appliances (one in the mechanical room, one in the general storage area (820), one in the O&M storage room (810), and one in maintenance shop ( 811 ). Additionally install one in the room or occupied space having the closest supply register ( Warehouse Clerk area ) to associated fuel using mechanical equipment measured along the duct from the unit. •6. Deficiency# 91597 - Building 6912 - Recreation Install (11) 120V single station CO alarms with battery backup and integral detection. Provide one in each space containing the CO source equipment and appliances (one in the mechanical room, a total of two ). Additionally install one in the room or occupied space having the closest supply register ( Aerobics (418), South part of Recreation (at the entrance - 411), Corridor (401), Weight Room (422), Platform (423), Hall (427) to associated fuel using mechanical equipment measured along the duct from the unit. If there is more than one such register at equal distance, there should be one alarm for each such room or occupied space (gymnasium, corridor (401), and recreation (411). Warrantees: The contractor is to provide a written warranty for the workmanship and materials at the time of completion of the job. A minimum 5 years manufacturer warranty is required for vendor provided materials, and a minimum one year warranty is required for installation. IV Construction Requirements Center Regulations: Contractor and his employees shall abide by all of the Center's rules and regulations. There shall be no fraternization between contractor personnel and Job Corps Center students or personnel. The Contractor will be required to follow all Center safety and security requirements. Management of Jobsite: •· The Contractor shall maintain a clean and safe workplace that is secured at the end of each working day. •· The Contractor shall clear all deliveries with the Center's security personnel. Wage Rates: The bidders shall conform to the latest Davis-Bacon prevailing wage rate decisions for the area. Upon contract award, the Contractor must add Little Rock Job Corps Center and Adams and Associates, Inc., operator of the Little Rock Job Corps as additional named insured to the policies. Schedule: The Contractor shall submit a schedule for the work. The project shall be completed within 60 calendar days of the award of the contract. Written authorization is required to extend the 60 day completion requirements. Final Acceptance The Contractor shall notify the Center's facility manager when the project is substantially complete, and shall request an inspection. The completion of this inspection shall determine the date for the commencement of the warrantees, and the date for payment. V. Proposal Form The Contractor's proposal must be submitted on company letterhead, must reference the proposal ID# LRJC13-011 and address all items in section II and III, to be considered. Bids must be valid for 90 days. The Contractor shall submit the following information with the proposal: •· A list of materials and product data sheets (MSDS) proposed for use. References Copies of Insurance Certificates and applicable licenses. Completed vendor questionnaire and W9. All questions must be in writing and sent to the Purchasing Agent Smith.Carlas@jobcorps.org. No Phone Calls will be accepted.
