Description
The proposed contract action is for commercial item, which the Naval Medical Center San Diego intends to solicit this requirement on a sole source basis to The Regents of the University of California under the authority of Far 13.106-1 (b). This is a combined solicitation/synopsis for commercial items, prepared in accordance with the format in FAR subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes only solicitation, quotations are being requested and a written solicitation will not be issued. The solicitation number, N00259-13-T-0333 is issued as a request for quotation (RFQ). The North American Industry Classification System (NAICS) Code for this acquisition is 621511. AVAILABILITY OF FISCAL YEAR 2014 FUNDS. THE GOVERNMENT OBLIGATION MADE HEREUNDER IS CONTINGENT UPON THE ENACTMENT OF THE FISCAL YEAR 2014 APPROPRIATION ACT. Naval Medical Center San Diego, California Laboratory Mail-Out Services: STATEMENT OF WORK 1.0 INTRODUCTION 1.1 Organization The Naval Medical Center (NMC) located in San Diego, California is responsible for providing the U.S. Navy, other military branches, and their military families with reliable quality health care. In addition to maintaining status as one of the nation's premier care providers, NMC is responsible for preparing for deployable support of operational forces, as well as shaping the future of military medicine through education, training and research. 1.2 Background and Objective In order to properly diagnose patients or make decisions on courses of treatment, NMC often requires expedited laboratory mail out testing in addition to the standard lab mail outs currently on contract. Due to the lack of current resources, in-house testing is not possible. The objective of this contract is to obtain services that provide quick results for some tests (<24 hours) which has been shown to decrease length of hospital stays, reduce unnecessary diagnostic testing and lessen/eliminate unnecessary treatment. In general because of the location, Reagents University of California is able to provide results within 24-72 hours and these results are immediately available to our providers. 2.0 REQUIREMENT 2.1 Task Description and Deliverables The vendor, UCSD Regents University of California, shall perform all required tests as specified in the attached spreadsheet to include labor, necessary materials and transportation related to the processing and testing of samples as well as the following: Requirements Test Turn Around Time (TAT) Reporting Requirements Critical Nature See attached test list. Provide accurate test results for STAT specimen requests within 24 hours For all other specimens, provide accurate test results within three working days turnaround time from time of specimen receipt. Exceptions include those specimens which require further comprehensive or confirmatory testing. Call back critical result values within 60 minutes of critical result verification. Test results are called and faxed directly to the phone numbers listed on the test requisition. Provide hard copies of all specimen test results within ten working days of result availability Delay in diagnosis will delay proper patient treatment. 2.2 Task Personnel Requirements 2.2.1 US Citizenship: US citizenship is required. 2.2.2 Technical and Personal Skills Laboratory professionals who perform testing must possess the technical skills associated with this testing methodology. The reference laboratory will provide a copy of its CAP accreditation certificate as required by CAP as well as their Clinical Laboratory Improvement Amendments (CLIA) certificate. The reference laboratory must act in accordance with commercial laboratory practices. All subcontracted referral laboratories used by the vendor must also meet the above requirements. Laboratory reports must comply with the Health Insurance Portability and Accountability Act (HIPAA) regulations with necessary HIPAA clauses included. Per HIPAA Part II CFR 164.501, 164.502, 164.506, laboratory services are considered indirect physician providers. Reports will include medical liability clauses for non-personal medical services. Information provided in the reports must comply with the College of American Pathologists (CAP) standards. 3.0 Classification: Unclassified Privacy Act Statement: "Pursuant to Title 5 United States Code 552a(m)(1), the contractor and all employees of the contractor working under this contract are required to comply with the requirements of 5 U.S.C. 552a ("The Privacy Act of 1974"). 4.0 Travel: The client will provide its own courier service to deliver the specimen to the vendor for testing. 5.0 Period of Performance: 1 October 2013 - 31 September 2014. NON PERSONAL SERVICES STATEMENT Contractor employees performing services under this order will be controlled, directed, and supervised at all times by management personnel of the contractor. Contractor management will ensure that employees properly comply with the performance work standards outlined in the statement of work. Contractor employees will perform their duties independent of, and without the supervision of, any Government official or other Defense Contractor. The tasks, duties, and responsibilities set forth in the task order may not be interpreted or implemented in any manner that results in any contractor employee creating or modifying Federal policy, obligating the appropriated funds of the United States Government, overseeing the work of Federal employees, providing direct personal services to any Federal employee, or otherwise violating the prohibitions set forth in Parts 7.5 and 37.1 of the Federal Acquisition Regulation (FAR). The Government will control access to the facility and will perform the inspection and acceptance of the completed work. 6.0 Payment: Invoices will be submitted for payment via Wide Area Workflow (WAWF) every 30 days by the vendor.
