- Performance Work Statement Offutt AFB, NE is requesting information for a potential 100 percent set-aside for Small Business, HUBZone, Economically Disadvantaged Woman Owned Small Business (EDWOSB), or Service Disabled Veteran Owned Small Business (SDVOSB) concerns. IF ADEQUATE RESPONSES ARE NOT RECEIVED, THE ACQUISITION WILL BE SOLICITED ON AN UNRESTRICTED BASIS. This is a Request For Information for planning purposes only. The description of Utility Meter Reading services include: furnishing all plant labor, equipment and materials, and in performing all necessary travel and services required to read utility meters, provide complete billing service, perform meter repair and calibration, and provide utility billing for unmetered facilities based on engineering utility consumption estimates IAW the PWS. The base period will be from 1 October 2013 - 30 September 2014 and will include (4) four Option Years. NAICS Code: 561990 Size Standard: $10M. Prime contractors who are Small Business, HUBZone, EDWOSB or SDVOSB and who intend to submit a bid for this solicitation are highly encouraged to submit the following by 11:00 a.m. CDT on 27 August 2013: Responses should include the following: (1) a positive statement of your intention to submit a bid for this solicitation as a Prime Contractor; (2) date of HUBZone certification; and/or (3) documentation of SDVOSB status; and/or (4) EDWOSB must provide information IAW Federal Acquisition Regulation 19.1503; and (5) listing of capabilities and/or experience in work similar in type and scope to include contract numbers, project titles, dollar amounts, points of contact with their phone numbers, and an explanation of the relevancy of the project (limit to 5 single-sided pages); (6) documentation from surety or their agent of your capability to be bonded for bid, performance and payment bonds for this project; (7) If you are considering a joint venture that has not been formalized, you will need to specify the firm(s) you intend to partner with and how you plan to officially form the joint venture. Firms interested should provide the above documentation. Only e-mail responses will be accepted. Send all correspondence to john.ballesta@us.af.mil. All of the above must be submitted in sufficient detail for a decision to be made on the availability of interested Small Business, HUBZone, EDWOSB, or SDVOSB concerns. Responses shall be limited to 5 pages. Please include your DUNS Number or CAGE Code on your response. Failure to submit all information requested would result in a contractor being considered "non-responsive" in this Request For Information. A decision on whether this will be pursued as a set-aside or an unrestricted basis will be posted as a modification in Federal Business Opportunities. Be advised that all correspondence sent shall contain a subject line that reads, "F3H3G33179A002". When sending e-mail attachments ensure only.pdf,.doc, or.xls documents are sent.
