Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. II) Solicitation W912JB-13-T-4099 is issued as a Request for Quote (RFQ). (III) This solicitation incorporates provisions and clauses for commercial items in effect through Federal Acquisition Circular 2005-67. (IV) This Acquisition is set-aside for SDVOSB concerns. The associated NAICS code for this acquisition is 113310, small business size 500 employees. (V) SCHEDULE OF SUPPLIES/SERVICES: Performance Work Statement Project No. VGLZ 132019 Supplement for Remove Trees - 400 Area Selfridge Air National Guard Base, Michigan 1. DESCRIPTION OF SERVICES: The contractor shall provide all personnel, equipment, tools, supervision and all other items and services necessary to remove all tree growth vegetation (standing, leaning, or lying down) located within the designated areas shown on the project drawings. Removal of a pile of previously felled trees where shown. 1.01. PROJECT LOCATION: All work shall be performed at Selfridge Air National Guard Base (SANGB), Michigan (Macomb County). 1.02. TREE REMOVAL: All downed tree vegetation located within the designated areas shall be removed. Areas shall be identified at the site visit. Downed branches less than quote mark in diameter and less than two feet long may remain. Trees marked with yellow paint shall be cut and the stumps shall be ground two inches below the surrounding surface. Any existing stumps shall also be ground to that level. All material (trunks, branches, chips and other debris generated during the removal process, as well as any and all downed, deadfalls or leaning trees and branches) are to be removed from the designated cutting areas. All material shall be removed from SANGB and disposed of in a legal manner. 1.02.a Bid option 1, Trees marked with orange paint shall be cut and the stumps shall be ground two inches below the surrounding surface. All material shall be removed from SANGB and disposed of in a legal manner. 1.03. CHIPPING: Chipped material shall be removed from the site entirely. 1.04. BURNING: Burning will not be allowed. 1.05. WETLANDS: Wood chips shall not be stockpiled in wetland areas. 1.06. SCHEDULING: The contractor shall prepare and submit a practical schedule showing the order in which the contractor will perform the work and the dates on which the contractor contemplates starting and completing and inspecting each task. This schedule shall be submitted with the contractors' proposal. 1.07. WORK PLAN: The contractor shall describe how he/she plans to perform the contract. The plan shall be submitted with the contractors' proposal. 1.08. EQUIPMENT LIST: The contractor shall submit a complete list of equipment that will be used during the performance of this contract. This list shall be submitted with the contractors' proposal. 1.09. RESTORATION: All ground surfaces disturbed (non-wetland) by the processes necessary to perform this work, shall be restored. 1.10. CODE COMPLIANCE: All work shall be accomplished in accordance with the applicable codes and/or ordinances in force at the time of performance. It is the Contractor's responsibility to insure that where EPA, MDEQ or other such regulations control the removal, handling, installation or disposal of materials, they shall be strictly adhered to whether or not specifically referenced in the project documents. Contractor shall follow all Federal, State and Local regulations concerning wetland protection. 1.11. SAFETY: The contractor shall comply with all OSHA and MIOSHA regulations and with U.S. Army Corps of Engineers Safety and Health Requirements Manual EM385-1-1. 1.12. WORK HOURS: Hours of operations are Monday through Friday from 7:00 am through 4:30 pm eastern standard time, unless prior arrangements have been coordinated with the base-contracting officer. 1.13. MANPOWER REPORTING. The contractor shall report ALL contractor labor hours (including subcontractor labor hours) required for performance of services provided under this contract for Selfridge ANG Base, MI via a secure data collection site. The contractor is required to completely fill in all required data fields using the following web address: http://www.ecmra.mil/. 1.13.1. Reporting inputs will be for labor executed during the period of performance during each Government fiscal year (FY), which runs October 1 through September 30. While inputs may be reported any time during the FY, all data shall be reported no later than October 31 of each calendar year, beginning with 2013. Contractors may direct questions to the help desk at http://www.ecmra.mil. 2.SERVICE DELIVERY SUMMARY (SDS) #Performance ObjectivePerformance Threshold 1Remove tree vegetation from the designated areas. (Para 1.02)All tree vegetation in the designated areas has been removed. 2Height of remaining stumps. (Para 1.02)All remaining stumps shall be flush 3All chipped material shall be removed from the site. (Para 1.03)All chipped material has been removed. 3. GOVERNMENT FURNISHED PROPERTY AND SERVICES. For response to on-site emergencies, call 911. 4. GENERAL INFORMATION 4.1. All contractor employees who operate motorized vehicles on SANGB shall possess a valid state driver's license and shall comply with all base traffic regulations. 4.2. SECURITY REQUIREMENTS: The contractor shall submit and maintain a list of all contract employees to the CO. This list shall contain full names, the last four digits of their Social Security number, sex and date of birth. This information will be forwarded to Pass & ID (Bldg 906), where the employee will report to, for issuance of their ID card (to be accomplished within 5 days of employment). 4.3. QUALITY CONTROL: Quality Control is the sole responsibility of Contractor. 4.4. QUALITY ASSURANCE: The Government shall appoint a Contracting Officers Representative (COR) who will periodically evaluate the contractor's performance to ensure services are received. 4.5. HOLIDAYS: The following federal holidays are observed by this installation and are normally closed for business. The contractor may choose to perform work on recognized holidays or outside normal duty hours, which shall be performed at no additional cost to the government.
