- MSHA Academy Drawing Details DOL Wage Determination MSHA Paving Picture Statement of Work Mine Academy Paving This is a combined synopsis/solicitation for commercial construction services prepared in accordance with the format in Subpart 12.6, with additional information included in this notice under FAR Part 13, Simplified Acquisition Procedures, and FAR Part 36, Construction and Architect-Engineer Contracts. The solicitation number for this acquisition is: 483-13MR-7238(A) and is issued as a Request for Quote (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-69. The Mine Safety and Health Administration intends award a contract for asphalt paving as identified in the attached Statement of Work. All work will be performed under a single firm-fixed price construction contract, to a single contractor; wages shall be subject to the Davis-Bacon Act. This announcement constitutes the only solicitation; and proposals are being requested and a written solicitation will not be issued. PAPER COPIES OF THIS SOLICITATION WILL NOT BE ISSUED, AND TELEPHONE REQUESTS OR FAX REQUESTS WILL NOT BE ACCEPTED. IT IS THE SOLE RESPONSIBILITY OF THE OFFEROR TO CONTINUALLY VIEW THE www.fbo.gov WEBSITE FOR ANY AMENDMENTS ISSUED TO THE SOLICITATION. The NAICS codes for this procurement are 237990/237310 and the business size is $33.5 Million. This procurement is set-aside for Small Businesses. All offerors must establish and/or maintain an active registration and have up to date representations and certifications at http://www.sam.gov prior to closing date of this RFQ. Weather conditions will be a determining factor in deciding the period of performance and the start date, project completion goal is no later than September 30, 2013. In accordance with FAR 36.204 the magnitude of this contract is between $25,000.00 and $45,000. This solicitation is intended to be a "Best Value" procurement with past experience to be equal to price in the selection of the most responsive, responsible offeroror. Award may be made to a firm other than that offering the lowest price. Any interested firm shall be able to provide a list of previous jobs showing experience in completion of the work described above. This work may consist of road blading, excavating, dozer maintenance on roads, hauling, aggregate and borrow placement, clearing roadsides and/or trails, constructing trails, miscellaneous road and construction or repair, concrete and asphalt patching and or paving, road crack sealing, sign construction, guardrail construction, general excavation and other miscellaneous work. The contractor shall provide all management and project administration services, quality control, logistical support, vehicles, labor, equipment and materials to complete the work described in the solicitation. Award may be made to a firm other than that offering the lowest price. The Government intends to evaluate offers and award a contract without discussions with Contractors. Therefore, the Contractor's initial offer should contain the Contractor's best terms from a price and technical standpoint. The Government reserves the right to conduct discussions if later determined by the Contracting Officer to be necessary. The Government may reject any or all offers if such action is in the public interest; accept other than the lowest offer; and waive informalities and minor irregularities in offers received. The following factors shall be used to evaluate offers: 1) Technical Approach 2) Past Performance 3) Price Pricing should reflect fully burdened labour rates, and include material, delivery, and installation. Company's quotes MUST include: (1) The solicitation number; (2) The time specified in the solicitation for receipt of offers; (3) The name, address, and telephone number of the offeror; (4) A technical description of the items being offered in sufficient detail to evaluate compliance with the requirements in the solicitation. This may include product literature, or other documents, if necessary; (5) Terms of any express warranty; (6) Price, availability and any discount terms; (7) "Remit to" address, if different than mailing address; (8) A completed copy of the representations and certifications at FAR 52.212-3 (Dec 2012), (see FAR 52.212-3(j) for those representations and certifications that the offeror shall complete electronically); (9) Acknowledgment of Solicitation Amendments; (10) Past performance information, when included as an evaluation factor, to include recent and relevant contracts for the same or similar items and other references (including contract numbers, points of contact with telephone numbers and other relevant information); (11) If the offer is not submitted on the SF 1449, include a statement specifying the extent of agreement with all terms, conditions, and provisions included in the solicitation. Offers that fail to furnish required representations or information, or reject the terms and conditions of the solicitation may be excluded from consideration; (12) Company Tax Information Number and DUNS Number.
