- Description
- DM Local Policy 06, 010, 017. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format prescribed in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. Solicitation FA4877-13-Q-0378 is issued as a request for quotation (RFQ). This solicitation document incorporates provisions and clauses of those in effect through Federal Acquisition Circular 2005-69, Defense Circular Number 20130808, and Air Force Acquisition Circular 2013-0327. This procurement is being issued as a 100% small business set aside. The Associated North American Industrial Classification Standard (NAICS) code for this request is 314999 with a size standard 500 employees or under. The results of this requirement will be a firm fixed priced (FFP) contract. The right to make multiple or no award is reserved in the event it is advantageous to the Government to do so. The contractor shall provide the following Contract Line Item Number (CLIN): CLIN 0001- 15 EA: 6" x 6"x 5' Square post pads for shade structure poles CLIN 0002- 6 EA: 4" x 6" x 5' Square post pads for shade structure poles CLIN 0003- 10 EA: 3" x 3" x 5' Square post pads for shade structure poles CLIN 0004- 68 EA: 4" x 5' Round post pads for shade structure poles CLIN 0005- 1 EA: Installation of 68 post pads (CLIN 0004) at Finley Child Development Center (CDC). Installation includes removal and disposal of existing pads and installation of new pads. CLIN 0006- 1 EA: Installation of 31 post pads (CLINs 0001-0003) at Davis-Monthan CDC. Installation includes removal and disposal of existing pads and installation of new pads. Shipping will be FOB Destination to: 1. CLINs 0001- 0003 to be delivered to F1P32G - Davis-Monthan Child Development Center, 5335 E. Comanche St. Bldg. 2614, DMAFB, Tucson, AZ. 85707. This shipment should be marked for Michelle Crawford 2.
