Description

This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation and proposals are being requested. A written solicitation will not be issued. The solicitation number is VA248-13-R-2398 and is issued as a Request for Proposals (RFP), unless otherwise indicated herein. The associated North American Industrial Classification System (NAICS) code for this procurement is 624221 with a small business size standard of $10M. This solicitation starts 08/22/2013 and will end 09/13/2013 at 16:00 pm EST. The Department of Veterans Affairs Bay Pines Healthcare system, hereafter referred to as BPHCS, 10,000 Bay Pines Blvd. St. Petersburg, FL has a need for ten (10) reserved beds located within a homeless shelter in Lee County Florida. This is a Firm Fixed Price, Base Year with four (4) one year options requirement. SCOPE OF WORK: The services will be furnished to beneficiaries for veterans whose care is specifically authorized by the Department of Veterans Affairs, Bay Pines VA Healthcare System. It is understood that the type of patients to be cared for under this contract will require care and services above the level of meals, room and board. Care Services will include: a supervised, alcohol and drug free environment, supportive case-management services and provision of transportation to and from medical care at the BP VA Healthcare System. It is further agreed that beneficiaries receiving care under this contract will be provided supplies and education to support basic health and personal hygiene maintenance. The per diem rate established will be a firm fixed price at an all-inclusive rate for 10 beds. Preference will be given to the facilities that can accommodate families and female Veterans. The selected Offeror shall comply with the following commercial item terms and conditions, which are incorporated herein by reference: FAR 52.212-1, Instructions to Offerors - Commercial Items, applies to this acquisition; FAR 52.212-3, Offeror Representations and Certifications - Commercial Items - the selected offeror shall submit a completed copy of the listed representations and certifications; FAR 52.212-4, Contract Terms and Conditions - Commercial Items; FAR 52.212-5, Contract Terms and Conditions Required To Implement Statutes or Executive Orders-Commercial Items. Offerors shall meet the Evaluation Criteria identified in the Statement of Work (SOW) at item #8. 52.212-2, Evaluations-Commercial Items, with addenda: paragraph (a) is filled in with: 1. Technical Capability and sub-factors (items# 2-4 of SOW) 2. List of Case Management Personnel with qualifications, certifications and/or licenses. (#5) 3. Past performance. 4. Price. Technical is significantly more important than price. Any sub-factors rated as unacceptable is reason for proposals to be rejected.
