Description
A. Description: 1. This is a Combined Synopsis and Solicitation for commercial items prepared in accordance with the format in the Federal Acquisition Regulation (FAR) Subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; therefore, quotes are being requested and a written solicitation will not be issued. 2. The solicitation number is CSS3031397 and the solicitation is issued as a request for quotation. 3. This acquisition is for a commercial items and is conducted under the authority of the FAR Part 13.5-Test Program for Certain Commercial Items; and the FAR Part 12-Acquisition of Commercial Items and is expected to exceed the simplified acquisition threshold. 4. The solicitation document and incorporated provisions and clauses are those in effect through the Federal Acquisition Circular (FAC) 2005-69 effective August 1, 2013. The resultant purchase order will include all applicable provisions and clauses in effect through the FAC 2005-69 effective August 1, 2013. 5. This solicitation is an Agilent Brand Name Solicitation. The provision at the FAR clause at 52.211-6, Brand Name or Equal applies to this acquisition. The intended procurement is classified under North American Industry Classification System Code 334515 with a Size Standard of 500 employees. 6.1. Scope: Ataxia Disease Research has a need for laboratory reagents, enzymes, buffers, tapes, and kits for whole exome sequencing and genome-wide genotyping neurologically normal controls and affected cases of Ataxia Disease, ALS, and Parkinson's Disease. The needed items will allow high thru-put processing of samples for whole exome sequencing. 6.2. Specification Requirements/Brand Name Justification: Brand Name Requirements: Agilent exome library reagents, enzymes, buffers, tapes, and kits. The NIH/NIA Laboratory purchased an Agilent Bravo Liquid Handler Robot in 2010 and maintain a $11,000.00 annual service contract on that equipment. The Agilent Bravo Liquid Handler Robot specificly requires Agilent reagents, enzymes, buffers, tapes, and kits reagents to properly operate which is used for the NIH/NIA Laboratory custom designed software program. Other manufacturer's non-Agilent items are not compatible with the Agilent Bravo Liquid Handler Robot. 7.1. Delivery Requirements: The general description of the salient physical, functional, or performance characteristics of the brand name items that the contractor shall provide are as follows: Item 1: SureSelectXT2 Custom 3-5.9Mb library (up to 55k oligos) re-order sufficient for pre-capture processing of 480 samples with Automation. Agilent DNA library with 55,000 oligonucleotides specifically selected for Parkinson's Disease association. QUANTITY: 7 Item 2: SureSelectXT2 Reagent kit for up to 480 samples, includes library prep and enrichment reagents for pre-capture processing on the HiSeq platform. Compatible with Automation and includes indexes 1-96. Agilent Buffers and enzymes for processing custom oligonucleotide DNA library. QUANTITY: 7 Item 3: Herculase II Fusion DNA Polymerase,400rxn Agilent Enzyme for elongation of DNA fragments. QUANTITY: 9 Item 4: High Sensitivity D1K ScreenTape. Box of 7 ScreenTape for highly sensitive DNA analysis up to 1 kb. Reagent necessary for DNA quantification and qualification on Agilent's TapeStation DNA analyzer. QUANTITY: 60 Item 5: High Sensitivity D1K Reagents. Include HS D1K ladder and sample buffer. Reagent necessary for DNA quantification and qualification on Agilent's TapeStation DNA analyzer. QUANTITY: 60 Item 6: D1K ScreenTape Box of 7 ScreenTape for std DNA analysis up to 1 kb. Reagent necessary for DNA quantification and qualification on Agilent's TapeStation DNA analyzer. QUANTITY: 30 Item 7: D1K Reagents. Include D1K ladder and sample buffer. Reagent necessary for DNA quantification and qualification on Agilent's TapeStation DNA analyzer. QTY: 30 Item 8: Shipping and Handling QUANTITY: 1 7.2. Applicable Documents: None 8. Delivery shall occur within 4 weeks. The place of delivery: The National Institutes of Health Campus; The National Institute on Aging; Building 35, Room 1A1015; Bethesda, MD 20892. 9. The provision at the FAR clause 52.212-1, Instructions to Offerors-Commercial Items, applies to this acquisition. 10. The provision at the FAR clause 52.212-2, Evaluation-Commercial Items, applies to this acquisition. The Government's evaluation procedure will be based on the ‘Technically Acceptable-Lowest Priced' method. Therefore, the quote which offers the technically acceptable and lowest priced solution to meet all the Government's requirements will be evaluated to be the ‘Best Value' to the Government. 11. The provision at the FAR clause 52.212-3, Offeror Representations and Certifications- Commercial Items, applies to this acquisition. An offeror must include a completed copy of the provision at the FAR clause 52.212-3, Offeror Representations and Certifications-Commercial Items, with its offer. 12. The provision at the FAR clause at 52.212-4, Contract Terms and Conditions-Commercial Items, applies to this acquisition. 13. The provision at FAR clause at 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders-Commercial Items, applies to this acquisition. 14. Additional contract requirement(s) or terms and conditions ( i.e. contract financing arrangements or warranty requirements): Not Applicable. 15. The Defense Priorities and Allocations System (DPAS): Not Applicable. B. Responses and Questions: 1. Respondents that believe that they are manufacturers or authorized resellers of the equipment specified in this announcement must provide, as part of their response: (a) product, catalog, model, and/or part number(s); (b) product description; (c) all relevant information and documentation that the item(s) offered meets the salient physical, functional, or performance characteristics as specified in the purchase description; quantity; estimated price or cost; shipping, handling, and/or installation charges; and delivery date after receipt of order. 2. Responses to this notice shall contain sufficient information to establish the interested party's bona-fide capabilities for fulfilling the requirement and include: unit price, list price, shipping and handling costs, the delivery period after contract award, the prompt payment discount terms, the F.O.B. Point (Destination or Origin), the Dun & Bradstreet Number (DUNS), the Taxpayer Identification Number (TIN), and the certification of business size.
