Description
COMBINED SYNOPSIS/SOLICITATION PROVIDE CLINICAL PLATFORM BRAND NAME OR EQUAL TO EDWARDS LIFESCIENCES EV1000. (I) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This acquisition is Set-Aside for Service Disabled Veteran Owned Small Business Concerns. All Small Businesses submitting an offer must be registered on www.ccr.gov and https://www.sam.gov. Any amendments issued to this solicitation will ONLY be available on the FBO website (www.fbo.gov ). (II) This solicitation is issued as a Request For Proposal (RFQ). Submit written proposals on RFP Number VA246-13-Q-1828. (III) The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-66. (IV) This procurement is unrestricted. The associated NAICS code is 339112. The small business size standard is 500 employees. An ScvO2 value >73% can be targeted using fluid (including red blood cells) and inotropic agents. The cockpit screen combines large, easy-to-see numbers with specific color target ranges to clearly indicate patient status. You can choose parameters, alarms and targets to meet your precise patient monitoring needs. - The Goal Positioning Screen (GPS) plots two key hemodynamic parameters on the same X/Y plane. - The physio-relationship screen depicts the balance between oxygen delivery and consumption., - Has three options for connecting the platform to your Hospital Information System - IFM out through a serial connection, HL7 through an ethernet connection, as well as an HL7 integration engine. - PreSep oximetry catheter continuously monitors central venous oxygen saturation (ScvO2) for the treatment of sepsis. - Contains FloTrac sensor which connects toexisting arterial catheter and automatically calculates key flow parameters (CCO/CCI, SV/SVI, SVV, SVR/SVRI) every 20 seconds. - VolumeView set provides volumetric parameters (EVLW, GEDV, GEF, PVPI, ITBV) and continuous, calibrated hemodynamic parameters (CCO/CCI, SV/SVI, SVV, SVR/SVRI). - Graphical trend screen allows selection, place and track interventions over time providing key parameter trending data. The percent change indicator provides additional insight into the patient's condition. - Monitoring and optimizing stroke volume (SV) by volume loading during the surgical procedure or in the immediate postoperative period is a key strategy for reducing postoperative complications. - SVV measured by the FloTrac system also can be used to tailor fluid therapy. Cardiac output measured continuously by the FloTrac system can be used (in combination with SaO2 and hemoglobin) to monitor and optimize DO2 with fluid (including red blood cells) and inotropic agents. Training/Inservice (Applications): - Vendor shall provide operating manual, electronically and hardcopy Other: - Provide documentation of government facilities where this equipment is already in use. Identify references with points of contacts and telephone numbers. (VII) Required delivery 30 days ARO. Place of delivery is Salisbury VA Medical Center, 1601 Brenner Ave, Salisbury, NC 28144. Delivery shall be FOB Destination to Salisbury, NC. (VIII) FAR 52.212-1, Instructions to Offerors -- Commercial Items (Jun, 2008), applies to this acquisition. (IX) Evaluation - This acquisition will be conducted in accordance with Federal Acquisition Regulation (FAR) Subpart 15.101-2, and as supplemented by agency procedures using the Lowest Price Technically Acceptable source selection process. Technical ability will be evaluated as either Acceptable/Unacceptable, Pass/Fail, or Go/No-Go. Tradeoffs are not permitted. (X) The offeror must submit a completed copy of the provision at FAR 52.212-3, Offeror Representations and Certifications - Commercial Items (April 2011), with its quote. An offeror shall complete only paragraph (b) of this provision if the offeror has completed the annual representations and certifications electronically at https://www.sam.gov. If an offeror has not completed the annual representations and certifications electronically at the SAM website, the offeror shall complete only paragraphs (c) through (o) of this provision. (XI) The clause at FAR 52.212-4, Contract Terms and Conditions - Commercial Items (Jun, 2010), applies to this acquisition. (XII) The clause at FAR 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders - Commercial Items (Apr, 2011) applies to this acquisition.
