Description
This is a combined synopsis/solicitation for commercial service prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation and quotes are being requested. A written solicitation will not be issued. The solicitation number is VA248-13-Q-2516 and is issued as a Request for Quote (RFQ), unless otherwise indicated herein. The associated North American Industrial Classification System (NAICS) code for this procurement is 621511 with a small business size standard of $30M. This solicitation starts 08/22/2013 and will end 09/13/2013 at 16:00 pm EST. This solicitation is a Brand Name or Equal and is unrestricted. The Department of Veterans Affairs Bay Pines Healthcare system, hereafter referred to as BPHCS, 10,000 Bay Pines Blvd. St. Petersburg, FL requires technical and professional services for performance and interpretation of the Oncotype DX test for breast cancer patients at Bay Pines VA Health Care System This is a Firm Fixed Price, Base Year with four (4) one year options requirement. Period of Performance Base Year: 20 Nov 2013 - 19 Nov 2014. SCOPE OF WORK: The VA Healthcare System (VAHCS) at Bay Pines Florida does not perform the specialized genomic test Oncotype DX which is used as a diagnostic test to determine treatment plan for breast cancer patients. These procedures require the services of an outside/reference laboratory to provide patient care analysis. These tests are critical for determination of the course of therapy for our breast cancer patients General: The VAHCS requires the reference laboratory to have the capability to perform and interpret Oncotype DX a diagnostic test performed on patient tissues and paraffin block samples. Quotes shall be quoted as total cost a year for an estimated yearly total of four (4) assays per under CPT code S3854. The Reference Laboratory shall: 1.Perform and interpret the Oncotype DX Breast Cancer Assay 2.Be a CAP/CLIA licensed/approved laboratory 3.Provide Bay Pines VAHCS a copy of licensure upon request 4.Provide preprinted order form/protocol for patient/specimen information. 5.Call all critical results to provider 6.Provide a descriptive, professional report to include comments that reflect clinical application. This report may be faxed and must be followed by a mailed report. Controls: 1.Provider reviews all test results and integrates information into patient treatment 2.Notation of results and location of test performed entered into patient records (provider notes) 3.Results sent to medical records to be scanned as a part of the permanent record Inspection and Acceptance: Pathologists evaluate and accept all work/ results. Any stop work or re-work request shall originate with the pathologists as determined by their professional evaluation of the results. Patient Information Security: A VHA HIPPA Business Associate Agreement must be signed by the vendor for appropriate security and privacy handling of PHI-patient health information and identity. Equal Certification: Due to the nature and critical diagnosis associated with the requested service. All laboratories presenting an "equal" service shall provide certified and medically approved documentation and test results to verify their service has been determined an "equal" diagnosis vehicle for genomic testing for Oncotype DX Breast Cancer Assay. Failure to provide the information in the above paragraph shall result in your quote being determined "non-responsive". The selected Offeror shall comply with the following commercial item terms and conditions, which are incorporated herein by reference: FAR 52.212-1, Instructions to Offerors - Commercial Items, applies to this acquisition; FAR 52.212-3, Offeror Representations and Certifications - Commercial Items - the selected offeror shall submit a completed copy of the listed representations and certifications; FAR 52.212-4, Contract Terms and Conditions - Commercial Items; FAR 52.212-5, Contract Terms and Conditions Required To Implement Statutes or Executive Orders-Commercial Items. Offerors shall meet the Evaluation Criteria identified in the Statement of Work (SOW). 52.212-2, Evaluations-Commercial Items, with addenda: paragraph (a) is filled in with: 1. Technical and sub-factors: -Certified and medically approved documentation -Test Results 2. Pass Performance 3. Price Technical and past performance is significantly more important than price. Any sub-factors rated as unacceptable shall be reason for proposals to be rejected. Offeror shall be registered in the System for Award Management (SAM) database before an award can be made to them.
