REQUIRED REGISTRATIONS Federal Acquisition Regulations require all Contractors conducting business with the Federal Government to be registered on an online database(s): System for Award Management (SAM): After initial registration, the Contractor must review all data annually to ensure it remains current. Noncompliance with this requirement will preclude the exercising of any option periods that may be included in the contract and will be cause for termination of the contract at such time noncompliance is discovered. Refer to the cited Clause for more details. The Contractor can register online at the following website: www.sam.gov VetBiz: Veteran owned companies conducting business as Service Disabled and/or Veteran Owned Small Businesses are required to register in the VetBiz.gov online database to be considered for contract award. This database is used to promote and market Veteran Owned Small Businesses (VOSBs) and Service Disabled Veteran Owned Businesses (SDVOSBs). The Contractor can register online at the following website: www.vetbiz.gov For assistance with registration in VetBiz.gov, please contact the Center for Veterans Enterprise at 866-584-2344 or 202-303-3260. DATA UNIVERSAL NUMBERING SYSTEM (DUNS) NUMBER In accordance with FAR Provision 52.212-1 paragraph (j), the Contractor shall provide the Dun and Bradstreet number assigned to his/her firm in the space provided below: DUNS#: NOTICE TO OFFERORS The awarded Contractor is advised that only the Contracting Officer, acting within the scope of the contract, has the authority to make changes that affect the contract in terms of quality, quantity, price or delivery. In the event the Contractor initiates any such change at the direction of any person other than the Contracting Officer, the change shall be considered to have been made without authority and no adjustment shall be made in the contract price to cover any increase of costs incurred as the result of the change. GENERAL INSTRUCTIONS FOR SUBMISSION Contractor's shall submit a technical proposal for evaluation containing the following: The contractor must submit one (1) copy of a technical proposal for evaluation. The technical proposal must provide sufficient information to demonstrate the capabilities to satisfactorily perform the work detailed in the work statement. It must address evaluation criteria as listed in 52.212-2 - Evaluation - Commercial Items. No cost information shall be included in the technical proposal. Cost proposal shall be provided on the Schedule of Services and Prices/Cost. The cost proposal shall contain all information relative to cost and pricing. The contractor must complete FAR 52.212-3 Offeror Representations and Certifications Items with their proposal. Delivery of Proposal: Mail: All completed signed copies of proposals shall be mailed to address specified in Block 9 of Standard From 1449 (page 1) no later than date/time specified in Block 6 of Standard Form 1449. Offers may be submitted on a CD containing PDF copies of requirements within the solicitation, evaluation criteria. NOTE: Documents submitted in response to this solicitation as examples or evidence unless marked confidential shall be releasable to the public under the Freedom of Information Act (FOIA). Offerors must complete and return all information designated in FAR Provision 52.212-1, Instructions to Offerors - Commercial Items, paragraph "b", 52.212-2, Evaluation - Commercial Items. Failure to do so may preclude the offeror from further consideration.
