- FA4621-13-T-007-0001 Amendment Amended Solicitation This is a combined synopsis/solicitation for commercial items prepared in accordance with FAR part 12, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation. Proposals are being requested and a written solicitation will not be issued. This solicitation is being issued as a Request for Quotation, which incorporates provisions and clauses for commercial items in effect through Federal Acquisition Circular 2005-69. This acquisition is set-aside for Small Businesses in accordance with FAR part 19.5. The associated NAICS code for this acquisition is 811212 with a size standard of $25.5 Million. The contractor shall provide all personnel, equipment, tools, supplies, materials, supervision and transportation necessary to provide an on call technician to operate, maintain, and repair the Uninterrupted Power Supply equipment in accordance with the attached Statement of Work, dated 22 July 2013. The following is a description of the items as detailed in the attached solicitation. Base Year CLIN 0001 - (12 Months) Uninterrupted Power Supply Maintenance CLIN 0002 - (2500 Lots) Uninterrupted Power Supply Parts Draw (Not to exceed $2,500.00) Period of Performance shall be 30 September 2013 - 29 September 2014 Option Year 1 CLIN 1001 - (12 Months) Uninterrupted Power Supply CLIN 1002 - (2500 Lots) Uninterrupted Power Supply Parts Draw (Not to exceed $2,500.00) Period of Performance shall be 30 September 2014 - 29 September 2015 Option Year 2 CLIN 2001 - (12 Months) Uninterrupted Power Supply CLIN 2002 - (2500 Lots) Uninterrupted Power Supply Parts Draw (Not to exceed $2,500.00) Period of Performance shall be 30 September 2015 - 29 September 2016 Option Year 3 CLIN 3001 - (12 Months) Uninterrupted Power Supply CLIN 3002 - (2500 Lots) Uninterrupted Power Supply Parts Draw (Not to exceed $2,500.00) Period of Performance shall be 30 September 2016 - 29 September 2017 Option Year 4 CLIN 4001 - (12 Months) Uninterrupted Power Supply CLIN 4002 - (2500 Lots) Uninterrupted Power Supply Parts Draw (Not to exceed $2,500.00) Period of Performance shall be 30 September 2017 - 29 September 2018 The following FAR Provisions/ Clauses are included in the attached solicitation: FAR 52.212-1, Instruction to Offerors. Far 52.212-2, Evaluation- Commercial Items FAR 52.212-3, Offeror Representations and Certifications-Commercial Items. FAR 52.212-4, Contract Terms and Conditions-Commercial Items FAR 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders-Commercial Items. Additional FAR Provisions and Clauses are included in the attached solicitation. Award will be made to the responsive offeror determined to be the most advantageous to the government in accordance with the criteria listed in the attached solicitation. Awardee will be required to to invoice through wide area work flow (WAWF). Award will only be made to contractors registered in System for Award Management( http://www.SAM.gov ) Please fill out the attached Request For Quote and send with your offer.
