Description
This is a SOURCES SOUGHT NOTICE. This notice is a market survey and is for information only, to be used for preliminary planning purposes. No proposals are being requested or accepted with this notice. THIS IS NOT A SOLICITATION FOR QUOTES AND NO CONTRACT SHALL BE AWARDED FROM THIS NOTICE. The VA NCO 15 Contracting Center has been tasked to solicit for and award General Surgery Services for the Robert J. Dole VA Medical Center, Wichita, KS. Potential sources are responsible for General surgical services include comprehensive surgical evaluation and treatment of patients, surgical operating room procedures, surgical inpatient and outpatient consultations, outpatient clinic coverage, perform dialysis access surgery, provide graduate medical resident supervision and providing on call coverage. The contractor will utilize the VA-CPRS computer system to document physicians' orders and progress notes. The contractor must be Board Certified in General Surgery by the American Board of Surgery (ABS) and maintain licensure with the Kansas Board of Healing Arts Certification throughout the contract period of performance. No solicitation is currently available at this time. The proposed project is anticipated to be a firm, fixed-price requirement to provide for Outpatient Clinics during the hours of 1300 - 1600, on Mondays, Endoscopy Clinics during the hours of 0800 - 1200 on Monday, Wednesday, Thursday and Fridays, and Inpatient Consultations on a as needed basis, on call coverage, two weekends a month and week nights as needed, shifts are as follows: weekend coverage 8:00 am Friday - 8:00 am Monday, 24 hour shifts, weekday Monday - Thursday 4:30 pm to 8:00 am the following day, holiday 8:00 am - 8:00 am the following day, one (1) FTE to Robert J. Dole VAMC. The type of solicitation to be issued will depend upon the responses to this notice. No reimbursement will be made for any costs associated with providing information in response to this notice or any follow up information requested. The purpose of this notice is to gain knowledge of interest, capabilities and qualifications of various members of the Business Community to include Small Business or Large Business, Veteran Owned Small Business, Service-Disabled Veteran-Owned Small Business (SDVOSB), HUBZONE, 8(a), etc., to complete and perform a Firm, Fixed-Price contract. After review of the responses to this sources sought announcement a synopsis and/or solicitation announcement may be published at a later time. The Government must ensure there is adequate competition among the potential pool of responsive contractors. The duration of the requirement is for a base period (start-TBD and end TBD) with four each one-year option periods. The North American Industrial Classification System (NAICS) Code applicable to this acquisition is 621111. The Small Business Size Standard for this acquisition is $10M. DRAFT: Performance Work Statement GENERAL SURGERY SERVICES FTE/HOURLY ONSITE PERFORMANCE WORK STATEMENT (PWS) 1.GENERAL 1.1.SERVICES REQUIRED: General surgical services include comprehensive surgical evaluation and treatment of patients, surgical operating room procedures, surgical inpatient and outpatient consultations, outpatient clinic coverage, perform dialysis access surgery, graduate medical resident supervision and providing on call coverage. The Department of Veterans Affairs (VA) Medical Center (VAMC) located in Wichita, KS requires a HCR to provide onsite support for general surgery services to eligible VA beneficiaries. 1.2.PLACE OF PERFORMANCE: Services shall be provided on site in Surgery Service, Robert J. Dole VAMC, 5500 East Kellogg Ave, Wichita, KS 67218. 1.3.AUTHORITY: Title 38 USC 8153, Health Care Resources (HCR) sharing Authority, FAR Part 12 and FAR Part 15. 2.QUALIFICATIONS: 2.1.1.LICENSE: All licenses held by the Contractor staff working on this contract shall be full and unrestricted licenses. Contractor staff must submit evidence of at least one full, active, current and unrestricted licensure, registration, certification, and/or other relevant credentials, for verification prior to appointment and throughout the employment process, as requested by the COR. Contractor is responsible for keeping the VA COR apprised of anything that would adversely affect or otherwise limit their clinical privileges. Note: Failure to keep VA fully informed on these matters may result in administrative or disciplinary action. 2.1.2.BOARD CERTIFICATION /ELIGIBILITY: Contractor's General Surgeon must be Board Certified in General Surgery by the American Board of Surgery (ABS) and maintain licensure with the Kansas Board of Healing Arts Certification throughout the contract POP. In the event that Contractor's general surgeon(s) is/are not directly employed by the treating facility, documentation must be provided to ensure adequate certification. All continuing education courses required for maintaining certification must be kept up to date at all times. Documentation verifying current certification must be provided by the Contractor's General Surgeons to the VA COR on an annual basis. 2.1.3.RESIDENT SUPERVISION/FACILITY APPOINTMENT: The Contractor Staff shall hold a facility appointment with the University of Kansas School of Medicine - Wichita. The Contractor Staff will participate in the clinical medical education of the surgical physician residents. 2.1.4.CREDENTIALING AND PRIVILEGING: Credentialing and privileging will be done in accordance with the provisions of VHA Handbook 1100.19. This VHA Handbook provides updated VHA procedures regarding credentialing and privileging, to include incorporating: VHA policy concerning VetPro; the Expedited Medical Staff Appointment Process; credentialing during activation of the facility Disaster Plan; requirements for querying the FSMB; credentialing and privileging requirements for Telemedicine and remote health care; clarifications for the Summary Suspension of Privileges process in order to ensure both patient safety and practitioner rights; and the credentialing requirements for other required providers. 2.1.5.TECHNICAL PROFICIENCY: Contractors shall be technically proficient in the skills necessary to fulfill the government's requirements, including the ability to speak, understand, read and write English fluently. Contractor will provide documents as needed to verify current and ongoing competency, skills, certification and/or licensure related to the provision of care, treatment and/or services performed. 2.1.6.CONTINUING CME/CEU REQUIREMENTS: Contractor will provide the COR copies of current CMEs as required or requested by the VAMC. Contractors registered or certified by national/medical associations shall continue to meet the minimum standards for CME to remain current. CME hours shall be reported to the credentials office for tracking. These documents are required for both privileging and re privileging. Failure to provide will result in loss of privileges. 2.1.7.TRAINING (VA MANDATORY): Contractors shall meet all VA educational requirements. All mandatory courses must be completed by the Contractor staff as required by the VA. 3.VA HOURS OF OPERATION: 3.1.BUSINESS HOURS General Surgery services will be provided on Mondays for Outpatient Clinics during the hours of 1300 - 1600, on Monday, Wednesday, Thursday and Fridays for Endoscopy Clinics during the hours of 0800 - 1200 and Inpatient Consultations on as needed. On call coverage, two weekends a month and week nights as needed, shifts are as follows: weekend coverage 8:00 am Friday - 8:00 am Monday, 24 hour shifts, weekday Monday - Thursday 4:30 pm to 8:00 am the following day, holiday 8:00 am - 8:00 am the following day. 4.CONTRACTOR RESPONSIBILITIES 4.1.SERVICES REQUIRED: General surgical services include comprehensive surgical evaluation and treatment of patients, surgical operating room procedures, surgical inpatient and outpatient consultations, outpatient clinic coverage, perform dialysis access surgery, graduate medical resident supervision and providing on call coverage. Contracting for 1 FTE (2080) General Surgery physician. 4.2.STANDARDS OF PRACTICE: Contractors care shall cover the range of services as would be provided in a state-of-the-art civilian medical treatment facility and the standard of care must be equal or exceed VA Standards. 4.2.1.Performance shall be according to the requirements contained in this PWS and equal to that of the professional standards of the TJC. 4.2.2.Ensure that all operative reports are dictated and authenticated (signed) immediately after surgery. 4.3.RESIDENT SUPERVISION AND TEACHING: 95% of the time. 4.3.1.Resident Supervision/Teaching: According to the guidelines dictated by the Residency Review Committee of Accreditation Council for Graduate Medical Education (ACGME), the physician performing the services of the contract will be responsible for residents. Contract Provider shall be responsible for: 4.3.1.1.Academic environment: Provide for an academic environment conducive to the training and professional development for residents in the area of physics rotating through the General Surgery Service. 4.3.1.2.Resident patient care documentation: Contractors shall be responsible for complying with the Residency review documentation and insuring that all notes and encounters are completed and shall appropriately document medical records in accordance with VA standards, equivalent to TJC compliance guidelines, standard commercial practice and guidelines established by VAMC Robert J. Dole. The Contractors shall also perform any administrative duties relative to documentation of resident training, as required and directed by the VA COS or designated representative. 4.3.1.3.Technical Direction and Oversight: Contractors shall provide technical direction to and oversight of residents/fellows consistent with current accreditation guidelines, clinical research, protocol development, data management of protocols, quality assurance conferences and meetings, and VA staff meetings. Ensure on-site resident supervision in accordance with the national VHA Handbook 1400.01, Resident Supervision, dated December 19, 2012. http://www1.va.gov/vhapublications/ViewPublication.asp?pub_ID=2847 4.4.DIRECT PATIENT CARE: 95% of the time involved in direct patient care. 4.4.1.1.Contractor's General Surgeon shall provide comprehensive surgical evaluation and treatment of patients, surgical operating room procedures, surgical inpatient and outpatient consultations, outpatient clinic coverage and providing on call coverage for the VA medical center. The Robert J. Dole VA Medical and Regional Office Center will provide surgical supplies, adequate space for inpatient and outpatient treatment, and schedule patients for inpatient and outpatient care for patient episodes of care. 4.4.1.2.The Contractor's General Surgeon shall provide inpatient surgical rounds conducted according to patient workload and on call coverage obligations. 4.4.1.3.The Contractor's General Surgeon will participate in the clinical medical education of the surgical physician residents and medical students and will serve on the teaching panel and present conferences to the residents as well. Active participation in clinical oversight and teaching of the surgical residents is a major responsibility. 4.4.1.4.Contractor's General Surgeon shall also be responsible for ensuring continuity of medical general surgery operations including communication among key personnel. 4.4.1.5.In acute emergencies the Contractor's General Surgeon shall be available at the VAMC as soon as possible. Acute surgical emergencies will be determined by the contract physician via telephone consults. For routine cases, Contractor's General Surgeon must be present within twenty-four (24) hours or next business. Contractor's General Surgeon shall make all attempts to see patients within an average twenty (20) minutes of their scheduled appointments. The VAMC shall provide all patient files, tests, and X-rays on clinic day in the clinic for the Contractor's General Surgeon's review. 4.4.1.6.Contractor's General Surgeon shall, upon receiving a completed inpatient consult, meet with the patient and provide a written recommendation within twenty-four (24) hours, or the next business day for non-critical patients and within as soon as possible if the patient's condition is determined to be of a critical nature. 4.5.ADMINISTRATIVE: 5% of time not involved in direct patient care 4.5.1.Contractor's General Surgeon shall ensure professional coverage for inpatients, including making interdisciplinary rounds within the VA, and outpatient clinical services as required to deliver optimal patient care. 4.5.2.Contractor's General Surgeons shall ensure that all operative reports are dictated and authenticated (signed) immediately after surgery and ensure that all discharge summaries are dictated and signed within 24 hours and signed within 7 calendar days of patient discharge and that all workload capture processes (Encounter Forms) are completed at the conclusion of a patient clinical encounter.
