- THIS IS A COMBINED SYNOPSIS/SOLICITATION FOR COMMERCIAL ITEMS PREPARED IN ACCORDANCE WITH THE FORMAT IN FAR SUBPART 12.6 AS SUPPLEMENTED WITH ADDITIONAL INFORMATION INCLUDED IN THIS NOTICE. THIS ANNOUNCEMENT CONSTITUTES THE ONLY SOLICITATION. PROPOSALS ARE BEING REQUESTED AND A WRITTEN SOLICITATION WILL NOT BE ISSUED. This solicitation is issued as a Request for Quotation (RFQ). ***************************************************************************** Purchase Request Number: F3W3303192AA02 - Request for Quotation - This document incorporates provisions and clauses in effect through the Federal Acquisition Circular 2005-69. ***************************************************************************** *PLEASE NOTE - THIS REQUIRES A MANDATORY SITE VISIT ON 22 AUGUST 2013 AT 10:30 A.M.* ‘**' = Amended Information Item No Item Description Qty Unit Amount Total Price CLIN 0001: Repair existing fall zone surfacing __1__ Package ________ _________ in substandard locations in current playground material IAW ASTM, NPPS or comparable U.S. and European safety standards. New surfacing must meet or exceed fall zone height requirements for height of existing structures and newly installed structures. Must include removal and disposal of existing surfacing material in Accordance with (IAW) all applicable laws and regulations including environmental laws. Must include all site and ground preparation to include proper subsurface leveling and installation of any material located below the new fall zone surface. **To include surfacing material 3 m sq. near kitchen and 3 m sq. near double slides. **-0001AA: Additional Option: Closure of the gap between surface material and curbing around the entire perimeter. CLIN 0002: Extend surfacing material __1__ Package ________ _________ approximately 5m x 22.5m on NE corner of playground, connecting the two existing barriers between surfacing material and base of 2 trees. **To include a 3 ft. diameter concrete curbing around the 2 trees. CLIN 0003: Installation of new concrete curbing __1__ Package ________ _________ along the entire perimeter of the new surfacing material. Must include removal and disposal of existing material IAW all applicable laws and regulations. CLIN 0004: Removal and disposal of existing __1__ Package ________ _________ rock wall/climbing wall. Disposal of material must be accomplished IAW all applicable laws and regulations. **To include ground leveling and removal of surrounding surfacing material. CLIN 0005: Repair and Maintenance of existing __1__ Package ________ _________ Spider structure to include all mounting and attachment hardware; replacing worn items; replacing any missing hardware; tightening loose hardware. All components and repairs must meet or exceed applicable U.S. and European safety standards. **To include replacement of all rusted hardware. CLIN 0006: Installation of 3 planter/garden __1__ Package ________ _________ boxes, located in the grassy area in the NW section between the playground structure and the courts. Garden boxes will measure 2m x 2m (quantity 2 of each) and 1m x 2m (quantity of 1 each) **hard plastice material with approximate height of 50-60cm. CLIN 0007: Purchase and Installation of new __1__ Package ________ _________ climbing structure in the area currently occupied by the rock wall. Item to be installed is similar to a stationary climbing rock. The new structure must meet all applicable U.S. and European Safety standards and installed meeting or exceeding the fall zone distances from other structures and borders of the surfacing material. **e.g. -Miracle Recreation Equipment Company: Mt. Kilimanjaro, Product model: 8873 OR EQUAL CLIN 0008: Additional Equipment and __6__ Each ________ _________ Amenities: Picnic tables, portable (6 ft., pinched steel with all-weather coating on table top and benches, QTY = 6) **color: royal blue. The following factors shall be used to evaluate offers (listed in relative order of importance): 1. Technical capability of the item to meet the government's requirement; 2. Price. (b) Options. The Government will evaluate offers for award purposes by adding the total price for all options to the total price for the basic requirement. The Government may determine that an offer is unacceptable if the option prices are significantly unbalanced. Evaluation of options shall not obligate the Government to exercise the option(s). (c) A written notice of award or acceptance of an offer, mailed or otherwise furnished to the successful offeror within the time for acceptance specified in the offer, shall result in a binding contract without further action by either party.
