- Attachment 3, Wage Determination Attachment 2, Past Performance Questionnaire Attachment 1, Performance Work Statement Request for Proposal This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Solicitation F1C0CR3184AQ01 is being issued as a Request for Proposal (RFP). All solicitation documents and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-69. This procurement is a not a small business set-aside. The NAICS code is 811219 and the small business size standard is $19.0M. The following commercial items are requested: The contractor shall provide all labor, materials, transportation, and personnel to provide annual preventative maintenance and concurrency support to include installation of annual software and hardware updates to the 736th Security Forces Squadron (SFS), Andersen AFB, Guam for equipment identified below. This includes one routine visit and two potential repair visits. The contractor must perform all inspections, maintenance, and routine repair service calls.
