This is a COMBINED SYNOPSIS/SOLICITATION for commercial items prepared in accordance with the format in subpart 12.6, and is being conducted under FAR Subpart 13.5 Test Program; simplified acquisition procedures apply. VA will conduct the procurement according to a combination of FAR Par 12, Acquisition of Commercial Items and Part 13, Simplified Acquisition Procedures as supplemented with additional information included in this notice. This announcement constitutes the only solicitation and a written solicitation will not be issued. Under the auspices of FAR 13.501(a)(2) the Cleveland VA Medical Center (VAMC) anticipates entering into a sole source procurement with Berchtold Corporation, 1950 Hanahan Rd, Charleston, SC, 290406 for a three (3) Berchtold OR Surgery Suite Systems comprised of Lights, Monitor Arms and Booms. The government believes that Berchtold Corporation is the only manufacturer that will satisfy the agency requirements for the specified Brand Name equipment. (ii) The solicitation number is VA250-13-Q-0716 and is issued as a Request For Quotation (RFQ) (iii) The provisions and clauses incorporated into this solicitation document are those in effect at the time of publication. Provisions and clauses incorporated by reference have the same force and effect as if they were given in full text.
