Description
Adams and Associates, Inc., contracting for the U.S. Department of Labor for the operation of the Exeter Job Corps Ceter - a 150 student residential training facility located at 162 Main Street, Exeter, RI is curently soliciting written bids to repair a septic line. There will be a mandatory walk through held on Tuesday, September 3, 2013 at 10:00 am. Vendors must be present at walk through for a bid to be accepted. All bids must be received by Tuesday, September 17, 2013 by 5:00 pm. Quotes received after the deadline and/or without all requested documentation will not be considered. Vendor must include the Scope of Work Identification number on submitted bid. Scope of Work EX 2013-91989 To complete the following (Repair Septic Line) Table of Contents I. Proposal Information II. Project Description III. Specific Requirements IV Construction Requirements V. Proposal Form I. Proposal Requirements Project: Replace 4 inch septic line (approximately 80 ft.) from tank to building C wellness area just north of main entrance to building C. Camera analysis indicated that septic line does not have continuous slope to the septic tank but rather has a "belly" that acts to trap the sewage and block the flow. Mailing address Exeter Job Corps 162 Main Street Exeter, Rhode Island 02822 Proposal Due Date: Tuesday, September 17, 2013 at 5:00 PM Evaluation Criteria Point Value PRICE: Responsible Contractor who can perform effective services at the lowest reasonable price Point Value: 60 QUALITY: Demonstrated experience and knowledge capabilities; Technical Excellence; Management Capabilities; Personnel qualification; Quality of the Tender packages; Technical excellence including where necessary, technical drawings, blue prints, Point Value: 20 PAST PERFORMANCE: Past services to center or list of references for verification purposes Point Value: 10 SCHEDULE: Detailed explanation on how services will be carried out and the delivery timeframe. Point Value: 10 TOTAL POINTS 100 Payment: Payment Terms are Net 30 days of completion and receipt of Invoice. Site Visit Date: Tuesday, September 3, 2013, 10:00 AM a) Contractor must visit the site and familiarize himself with the condition under which the work will be performed. No allowance will be made for any error on the contractors part for failure to take into account any conditions or circumstance existing at the site which might affect the contemplate work. The fact that a bid is submitted with ID # 2013-91989 included will be taken as evidence that the contractor has full knowledge of the extent and character of the contemplate work and any problem to be encountered in the performing same. II. Project Description Scope of Project: Repair the septic line from Wellness Building C. Remove "belly" that is preventing the flow to the septic tank and install anew clean out as well as a septic T inside 7000 gallon tank. III Specific Requirements: Pump tank as needed to perform scope of work as outlined below Excavate and remove existing 4''septic line running from outside of building C Wellness area to 7000 gallon septic tank (excavation to be done in accordance to all dig safe rules and regulations) Replace with new 4'' septic line pitching towards tank as required by Rhode Island regulations. Install Clean out in line accessible from above ground with cover at grade. Install septic T inside tank as to prevent solids from blocking line. Regrade all disturbed areas and reseed or sod affected areas. All confined space entry needed to complete this scope to be included within bid Warrantees: a)The contractor is to provide written warranty for the above statements at time of completion of job IVConstruction Requirements Center Regulations: Contractor and his employees shall abide by all of the Center's rules and regulations. There shall be no fraternization between contractor personnel and Job Corps Center students or personnel. The Contractor will be required to follow all Center safety and security requirements. Management of Jobsite: The Contractor shall maintain a clean and safe workplace that is secured at the end of each working day. The Contractor shall clear all deliveries with the Center's security personnel. Wage Rates: The bidders shall conform to the latest Davis-Bacon prevailing wage rate decisions for the area. (See the attached prevailing wage rate). Employee time cards must be submitted for project. Insurance: The Contractor shall obtain and maintain insurance as follows: Workman's Compensation/Employers Liability insurance, to meet Rhode Islandstatutory requirements Comprehensive General Liability ($1,000,000 per occurrence) Comprehensive Automobile Liability ($1,000,000/combined single limit) Fire Legal Liability ($500,000 per occurrence) The Contractor must cover any deductible costs. Upon contract award, the Contractor must add Exeter Job Corps Center as additional named insured to the policies. Schedule: The Contractor shall submit a schedule for the work. The project shall be completed within 30 calendar days of the award of the contract. Written authorization is required to extend the 30 day completion requirements. Final Acceptance The Contractor shall notify the Center's facility manager when the project is substantially complete, and shall request an inspection. The completion of this inspection shall determine the date for the commencement of the warrantees, and the date for payment. V.Proposal Form The Contractor's proposal must be submitted on company letterhead, must reference the proposal ID # and address all items in sections II & III, to be considered. Bids must to be valid for 90 days. The Contractor shall submit the following information with the proposal: A list of materials and product data sheets (MSDS) proposed for use. References Copies of Insurance Certificates and applicable licenses. Completed vendor questionnaire and W9.
