- Pre-solicitation Notice for Medical Hygiene Assessment dated August 28, 2013 for the Indiana National Guard. The purpose of this pre-solicitation notice is to provide potential contractors with the basic information required. This solicitation is for the procurement of Medical Hygiene Assessments for the Indiana National Guard Facility Maintenance Shops (FMS), Unit Training and Equipment Site (UTES) and, Limited Army Aviation Support Facility (LAASF). These facilities are located in various cities across Indiana. The subsequent award will be based on an all or none selection. The requirement is being advertised as unrestricted. The North American Industrial Classification System (NAICS) code for this requirement is 926150. Classification H3 will be utilized. All responsible sources may submit an offer, which shall be considered by the Agency. The solicitation reference number is W912L9-13-T-0049. The winning vendor will be required to be registered in the System for Award Management (SAM) located at https://www.sam.gov.
