Statement of Work Drawing of Mitchell Hall Ceiling Plenum Drawing of Mitchell Hall Ceiling Plenum Praxis Plenum-Mezzanine Smith Dining and Mech Rooms Praxis Report II XQPZ 13-0816F Industrial Hygiene Services - Asbestos Response Actions Building 2350, Mitchell Hall, USAFA Owner: 10th Civil Engineer Squadron August 14, 2013 SCOPE OF WORK (SOW) I. Project: The ceiling plenum above the Mitchell Hall dining area has been identified as having asbestos containing dust. Review the attached Asbestos Surveys and conduct a thorough walk-through survey of the area 1. The Contractor shall determine the true extent of asbestos contamination by means of sampling using appropriate ASTM Methods. 2. Prepare a report of findings. The report must include the extent of contamination; identify if the contamination meets spill criteria identified in CCR 8 Part B; identify appropriate OSHA measures required to work in this space; fully document sampling by location and result, and include photos of the sampling locations; recommend appropriate actions the Building Owner should initiate based on these findings. Provide regulatory drivers. 3. Develop a Work Plan based on the recommended actions identified in the Report. 4. Provide daily oversight of the Work Plan operations in Mitchell Hall as Building Owner Representative (BOR). Provide final clearance of Mitchell Hall. 5. For bidding purposes, base your bid for item 4 above on a 30 day rate of oversight that includes all labor and material costs to achieve final clearance of cleanup performed by others. II. Work is to be performed by qualified, personnel trained and licensed by the State of Colorado to perform these tasks. III. Bid Walk: A bid walk has been scheduled for 5 September 2013 at 1300, at Mitchell Hall. All interested bidders must notify David Willers no later than 1200, MST, 4 September 2013 if they intend on attending the bid walk. Transportation for the bid walk will leave the Pass and ID at the South Gate at 1230. 1. It is the Contractor's duty and responsibility to verify all quantities referenced in this SOW. 2. It is the Contractor's duty and responsibility to field verify all measurements which may include (but not limited to) quantities of ACM and contaminated materials, lengths/dimensions, room dimensions, access to areas. 3. It is the Contractor's duty and responsibility to determine accessibility, limitations, and obtain all information necessary to provide a firm fixed price for the project. 4. Entrance to Mitchell Hall will be permitted during the walk. For personnel entering the Ceiling Plenum, Personal Protective Equipment (PPE) is recommended. IV. General Information: The Ceiling Plenum consists of 115,200 square feet of a grid structure above the dining areas. There are two access points to the plenum. Access is through the East and West Mechanical Rooms. V. Cost Proposals: Questions must be submitted in writing via email to David Willers. The deadline for questions will be by close of business on 9 September 2013. All bids are due on or before 12 September 2013 by 0830. Bids are to be electronically sent to david.willers@us.af.mil. VI. Contract: The Survey, Work Plan, and Oversight will be conducted under a single contract. Asbestos abatement, if required will be conducted under a separate contract. 1. Cooperate fully with separate contractors so that work under those contracts may be carried out smoothly, without interfering with or delaying work under that Contract. VII. Project Management: The Project Manager for this contract is Jonathan Burgoon, 10 CES/CEV, 333-6455. VIII. CONTRACTOR USE OF PREMISES 1. General: During the project period contractors shall have NO use of the premises for other than construction operations. The Contractors use of the premises is limited to the following areas; Mitchell Hall, and direct routes to and from areas necessary to complete the project. 2. Driveways and Entrances: Keep driveways and entrances serving the premises clear and available to the Owner, the Owners representative, the Owners employees and emergency vehicles at all times. 3. Smoking: Smoking is NOT permitted on the premises. Smoking is restricted to designated areas only.
