- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR 12.6, as supplemented with additional information included in this notice. The solicitation number is RFQP06081300013 and is issued as a request for quotes (RFQ). The solicitation document incorporated provisions and clauses are those in effect through Federal Acquisition Circular FAC 2005-69. The associated North American Industrial Classification System (NAICS) code for this procurement is 424410 with a small business size standard of 100 employees. This requirement is 100% set aside for small business and only qualified offerors may submit quotes. The solicitation will begin September 3, 2013 and quotes are due in the contracting office at FCI Phoenix by September 13, 2013 at 2:00 p.m. mountain time. The Federal Bureau of Prisons, FCI Phoenix requires the following items: LI 0001 - Bread, Whole Wheat, Fresh, Enriched, Pan Baked, Sandwich, Regular Slice, (First Ingredient Must Be Whole Wheat Flour), 24 oz. Loaf, 58,300 Loafs. LI 0002 - Bread, Hamburger Buns, White, Fresh, Enriched, 12 per Package, 13,200 Packages. LI 0003 - Bread, Hot Dog Buns, White, Fresh, Enriched, 12 per Package, 5,760 Packages. LI 0004 - Bread, Hoagie, 6", White, Fresh, Enriched, 6 per Package, 19,740 Packages. Delivery must be made based on attached schedule. Delivery is FOB destination. Delivery will be made to Federal Correctional Institution Phoenix at 37900 N. 45th Avenue, Phoenix, AZ, 85086. The prison is located just off I17 and the Pioneer exit. This solicitation requires registration with the System for Award Management (SAM) prior to award, pursuant to applicable regulations and guidelines. Registration information can be found at www.sam.gov. Quote must be good for 30 calendar days after the close of this solicitation. The selected Offeror must comply with the following commercial item terms and conditions, which are incorporated herein by reference: FAR 52.212-1, Instructions to Offerors - Commercial Items, applies to this acquisition; FAR 52.212-3, Offeror Representations and Certifications - Commercial Items - the selected offeror must submit a completed copy of the listed representations and certifications; FAR 52.212-4, Contract Terms and Conditions - Commercial Items; FAR 52.212-5, Contract Terms and Conditions Required To Implement Statutes or Executive Orders-Commercial Items, paragraph (a) and the following clauses in paragraph (b): 52.222-21, 52.222-26, 52.222-35, 52.222-36, 52.222-37, 52.225-13, 52.232-34.
