- Amendment 0003: Amendment 0003 has been issued for the following: 1) Rescind item 1 of amendment 0002. The unit of issue is clarified as standard cubic feet (scf). 2) Amend the table of section MIN/MAX ORDERING AMOUNTS as follows. To delete the wording quote mark Standard Cubic Feet is defined as the industry standard of 100 Cubic Feet. quote mark of note 1. 3) The solicitation has not been extended. The solicitation closing remains 10:00a Hawaii Standard Time (HST), 06 September 2013. Amendment 0002: Amendment 2 has been issued for the following: 1) Clarify that standard cubic feet equals the industry standard of 100 cubic feet. 2) Amend the table of section MIN/MAX ORDERING AMOUNTS. See Attachment W81K02-13-T-0101Att2. The change in the table is due to the maximum quantity under CLINs 0024, 1024, 2024, 3024, and 4024 has been amended from 10 to 9. 3) The solicitation has been hereby extended to 10:00a Hawaii Standard Time (HST), 06 September 2013. All other terms and conditions remain unchanged. Amendment 0001: Amendment 0001 has been issued to include the attachment W81K02-13-T-0101Att1. Attachment was left off the original posting in error. All other terms and conditions remain unchanged. Original: This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The Pacific Regional Contracting Office, Tripler Army Medical Center (TAMC), Honolulu, HI 96859, intends to solicit, negotiate, and award a contract for medical gases delivered in bulk and in delivered cylinders. This is a request for quotation (RFQ). This solicitation number is W81K02-13-T-0101. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-69. The Standard Industrial Classification Code is 2813, size standard is 1000 employees and the NAICS code of 325120. This is a 100% Total Small Business Set-aside. Provision at 52.212-1, Instructions to Offerors -- Commercial, and its addendum applies to this acquisition and a statement regarding any addenda to the provision. See W81K02-13-T-0101Att1. Provision at 52.212-2, Evaluation -- Commercial Items, and its addendum applies to this acquisition. See W81K02-13-T-0101Att1. Offerors shall include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications -- Commercial Items, with its offer. The clause at 52.212-4, Contract Terms and Conditions -- Commercial Items, and its addendum applies to this acquisition. See W81K02-13-T-0101Att1. The deviation to clause at 52.212-5, Contract Terms and Conditions Required To Implement Statutes Or Executive Orders -- Commercial Items, applies to this acquisition. See W81K02-13-T-0101Att1 for the FAR clauses that are applicable to this acquisition. The Defense Priorities and Allocations System (DPAS) and assigned rating is not applicable. The Government intends to award an Indefinite Delivery Indefinite Quantity contract with Firm, Fixed-Prices for a base period of 01 Oct 2013 through September 30, 2014 with four (4) one-year options to the successful offeror. Contract Requirements - The successful offeror shall be required to provide the following medical gases delivered in bulk and in delivered cylinders with the salient characteristics given in the attachment; W81K02-13-T-0101Att1. Offerors shall quote their unit pricing based on the estimated quantities identified with in the extended description of each Contract Line Item Number.
