CANCELLATION NOTICE:This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice.The solicitation number is VA262-13-R-1374 and is issued as an invitation for bids (IFB), unless otherwise indicated herein.The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular FAC 2005-69. The associated North American Industrial Classification System (NAICS) code for this procurement is 238330 with a small business size standard of $14.00M.This requirement is a [ Service-Disabled Veteran Owned ] set-aside and only qualified offerors may submit bids.The solicitation pricing on www.FedBid.com will start on the date this solicitation is posted and will end on 2013-08-29 14:00:00.0 Eastern Time or as otherwise displayed at www.FedBid.com.FOB Destination shall be in the Statement of Work. The VHA NCO 22 - 600 - Long Beach Healthcare System requires the following items, Exact Match Only, to the following: LI 001: Contractor shall provide all labor, parts, materials, tools and equipment required to repair the floor located in the dialysis area of building 213. PRESENT CONDITION: The floor had been saw cut and excavated recently for emergency repairs to the sewer piping. Upon completion of the sewer piping repairs the floor is approximately ?? low, and is approximately 428 square feet of area that is not even with the existing floor and is bare concrete. CONTRACTOR SHALL PERFORM THE FOLLOWING: Saw cut parallel with the existing cut floor in order to eliminate saw kerfs and chips to create solid masses of uninterrupted space. Chip away unwanted existing flooring in order to square off the area and prepare the surface for new epoxy flooring. Contractor shall provide and install full coverage rubber underlayment for crack control. Contractor shall work after hours and work shall progress continuously until completed in order for the work to be completed in a window of Friday 4:30p to Sunday 5:30p of the same week, for a total of 49 hours. Remove all broken base cove and replace with like material, which is a hand troweled epoxy seamless floor base cove. Contractor shall make every attempt to color match the existing floor color. Contractor shall provide and install dust control, negative air machines, infection control measures and comply with all VA construction regulations SITE VISIT: An organized site has been scheduled for: 11:00 am on Monday,August 26, 2013. Participants will meet in front of Building 213, VA Greater Los Angeles, 11301 Wilshire Boulevard, Los Angeles, CA 90073. Attendance is not mandatory, however all offerors are strongly encouraged to attend. 9. BID DUE DATE: No later than Thursday, August 29, 2013 at 11:00 am PST. Compliance to Labor Standards Provisions and Davis Bacon Wage Rate Determinations. REQUEST FOR INFORMATION: All Requests for Information (RFIs) regarding this RFP must be submitted in writing to the Contracting Officer and must be received no later than close of business 1600 on August 27, 2013. Responses to questions will be posted as an amendment to the solicitation and the government reserves the right to consolidate questions and responses., 1, job; Solicitation and Buy Attachments ***Question Submission: Interested offerors must submit any questions concerning the solicitation at the earliest time possible to enable the Buyer to respond. Questions not received within a reasonable time prior to close of the solicitation may not be considered.*** For this solicitation, VHA NCO 22 - 600 - Long Beach Healthcare System intends to conduct an online competitive reverse auction to be facilitated by the third-party reverse auction provider, FedBid, Inc. FedBid has developed an online, anonymous, browser based application to conduct the reverse auction. An Offeror may submit a series of pricing bids, which descend in price during the specified period of time for the aforementioned reverse auction. VHA NCO 22 - 600 - Long Beach Healthcare System is taking this action in an effort to improve both vendor access and awareness of requests and the agency's ability to gather multiple, competed, real-time bids.All responsible Offerors that respond to this solicitation MUST submit the pricing portion of their bid using the online exchange located at www.FedBid.com. There is no cost to register, review procurement data or make a bid on www.FedBid.com.Offerors that are not currently registered to use www.FedBid.com should proceed to www.FedBid.com to complete their free registration. In addition to any VHA buy terms entered in FedBid, the Seller (Bidder) has reviewed and acknowledges any attached solicitation documents, item details, delivery schedule, and/or instructions to Seller (collectively referred to as requirements), and agrees to all terms and conditions set forth in the solicitation documents. While additional support documentation (e.g. Seller Attachments) may be requested, pricing shall be entered as a 'bid' in the reverse auction - offline pricing is not an acceptable bid.
