Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This requirement is being solicited as an unrestricted procurement. This solicitation is issued in accordance with FAR Part 12 and evaluated in accordance with FAR Part 13. The Southern Regional Contracting Office (SRCO), Fort Hood, Texas, has a requirement to provide the Colblator II Surgery System Generator and the Reflex Ultra 45 & Reflex Ultra 70 collection wands at Carl R. Darnall Army Medical Center, Fort Hood, TX. This announcement constitutes the only solicitation; quotes are being requested. The solicitation number for these requirements is W81K00-13-T-0237. The solicitation is issued as a request for quotes (RFQ). This acquisition is subject to the availability of FY14 appropriations. The requirements consist of the following: A list of contract line items number(s) (CLINS) and items, quantities and units of measure; including option CLINs is available on the attachment entitled quote mark Pricing Schedule quote mark. This is a one (1) Year requirement beginning 01 Dec 13 to 30 Nov 14 to provide the Colblator II Surgery System Generator and the Reflex Ultra 45 & Reflex Ultra 70 series collection wands on an quote mark as needed quote mark basis to Carl R. Darnall Army Medical Center, Fort Hood, Texas. IAW the attached Pricing Schedule. Please see attached. SRCO anticipates a FFP contract and intends to make one award for the aggregate of the requirement resulting from this solicitation. The resulting contract will be for one (1) year with a period of performance beginning 01 Dec 13 through 30 Nov 14. The associated North American Industry Classification System (NAICS) code for this requirement is 339113 - quote mark Surgical Applience and Supplies Manufacturing quote mark. The Small Business size standard for this requirement is 500 Employees. The associated Product Service Code (PSC) for this requirement is 6515 - quote mark Surgical and Medical Instruments quote mark. The Standard Industry Classification (SIC) code is 3841 - quote mark Surgical and Medical Instruments quote mark. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-69. This combined synopsis/solicitation incorporates one or more clauses and provisions by reference with the same force and effect as if they were stated in full text. Full text provisions and clauses can be viewed at http://farsite.hill.af.mil. The provision at 52.212-1, Instructions to Offerors-Commercial Items, applies to this acquisition. The following instructions are provided to industry: 1. Technical Acceptability: IAW FAR 52.211-6; quote mark brand name or equal quote mark for the Colblator II Surgery System. In order for a quote to be deemed technically acceptable, the offeror must provide and/or substantiate the following requirements in their offer: 2. Pricing: Offeror shall submit pricing in accordance with the attached Pricing Schedule. Quotes must be submitted on time by fax to (254) 553-3906, Attn: Anthony Browner or email: anthony.browner.civ@mail.mil or anthony.browner@amedd.army.mil. Evaluation Process: All quotes will be evaluated based on lowest price technically acceptable (LPTA). The award decision will be based on the lowest technically acceptable quote. (End of provision) FAR Provision 52.212-2 applies to this acquisition. The Government anticipates the award of a FFP contract resulting from the solicitation to the responsible offeror whose offer, conforming to the solicitation, will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers: 1. Technical Acceptability: IAW FAR 52.211-6; quote mark brand name or equal quote mark for the Colblator II Surgery System. In order for a quote to be deemed technically acceptable, the offeror must provide and/or substantiate the following requirements in their offer: a. Capable of ablation, resection, and coagulation of soft tissue and hemostasis of blood vessels in otorhinolaryngology (ENT) b. Able to dissolves tissue at the molecular level. c. Highly-controlled manner with minimal thermal effect on surrounding tissue. d. Have a wide selection of Plasma attachments designed for specific ENT surgical applications. e. Controller, a flow control valve unit and cable, a foot control unit, and a hospital-grade power cord. f. Triple-active electrode configuration. g. Able to removes tissue for both tonsillectomy and adenoidectomy. h. Have integrated saline and suction port and Malleable shaft. i. Have access to the choanae during adenoidectomy. j. Have enhanced electrode material extends Wand life by approximately 30%. k. Able operate at a relatively low temperature to gently dissolve and/or shrink target tissue with minimal thermal damage to surrounding healthy tissue. Have a low temperature plasma excision (typically between 40 C to 70 C). l. Must be minimal thermal penetration to preserve healthy surrounding tissue. m. Should have controlled ablation quote mark, the name refers to the non-heat driven process of surgically dissociating soft tissue by using bipolar radiofrequency energy to excite the electrolytes in a conductive medium. such as saline solution, to create a precisely focused plasma field. - Acceptable Rating - Based on Southern Regional Contracting Office's market research, the proposed quote is sufficient to maintain the government's need. - Unacceptable Rating - Based on Southern Regional Contracting Office's market research, the proposed quote is not sufficient to maintain the government's need. As the quoted items does not meet all salient characteristics 2. Price. Technical Acceptability and Price, when combined, are equally as important. The Government intends to award a contract resulting from this solicitation to the responsible offeror, whose offer, conforming to this solicitation, is the Lowest Priced, Technically Acceptable (LPTA) Offer. The following FAR Clauses apply to this acquisition: 52.212-4; 52.209-6; 52.204-10; 52.209-6; 52.219-16; 52.222-3; 52.222-19; 52.222-21; 52.222-26; 52.222-36; 52.222-43; 52.222-50; 52.223-18; 52.225-13; 52.232-18; 52.232-33; 52.233-3; 52.233-4; 52.237-2; 52.247-29; and Local Wide Area Workflow (WAWF) clause. The following FAR Clauses are provided in full text: 52.211-6 BRAND NAME OR EQUAL (AUG 1999) (a) If an item in this solicitation is identified as quote mark brand name or equal, quote mark the purchase description reflects the characteristics and level of quality that will satisfy the Government's needs. The salient physical, functional, or performance characteristics that quote mark equal quote mark products must meet are specified in the solicitation. (b) To be considered for award, offers of quote mark equal quote mark products, including quote mark equal quote mark products of the brand name manufacturer, must- (1) Meet the salient physical, functional, or performance characteristic specified in this solicitation; (2) Clearly identify the item by-- (i) Brand name, if any; and (ii) Make or model number; (3) Include descriptive literature such as illustrations, drawings, or a clear reference to previously furnished descriptive data or information available to the Contracting Officer; and (4) Clearly describe any modifications the offeror plans to make in a product to make it conform to the solicitation requirements. Mark any descriptive material to clearly show the modifications. (c) The Contracting Officer will evaluate quote mark equal quote mark products on the basis of information furnished by the offeror or identified in the offer and reasonably available to the Contracting Officer. The Contracting Officer is not responsible for locating or obtaining any information not identified in the offer. (d) Unless the offeror clearly indicates in its offer that the product being offered is an quote mark equal quote mark product, the offeror shall provide the brand name product referenced in the solicitation. It is a Department of Defense requirement that Offerors must be registered in the Central Contractor Registry (CCR) located at the System for Award Management (SAM) at https://www.sam.gov to be considered for award. A determination by the government to not award based on the responses to this solicitation is solely within the discretion of the government.
