Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with FAR 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested and a written solicitation will not be issued. The solicitation number is N00259-13-T-0376. It is issued as a Request for Quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-69. This is small business set-aside action. The NAICS code is 325412. The size standard is 750 employees. It is the contractors' responsibility to be familiar with the applicable clauses and provisions. The clauses and provisions may be accessed in full text at http://acquisition.gov /far/ and www.acq.osd.mil. The contractor shall provide the items provided below in accordance with the Statement of Work: CLIN 0001 TOTAL PARENTAL NUTRITION (TPN), (TPN QUANTITY: 300/MO X 4M0) Quantity: 1200, Unit of Issue: EACH, Unit Price: _________; Extended Price: _____________ CLIN 0002 STARTER TPN, (TPN QUANTITY: 25/MO X 3M0) Quantity: 0100, Unit of Issue: EACH, Unit Price: _________; Extended Price: _____________ Performance period: 24 September 2013 - 31 December 2013 STATEMENT OF WORK GENERAL REQUIREMENTS: The effort required hereunder shall be performed in accordance with this Statement of Work and in accordance with all other terms and conditions set forth herein. The contractor shall provide all services, materials and equipment necessary for preparation of Total Parenteral Nutrition (TPN) solutions, as described in the Attachment A, and its delivery services to Pharmacy Department, Naval Medical Center San Diego. The scope of work performed under these specifications includes the furnishing of all labor, and parts to perform all services listed to ensure continued operation at their designed efficiency and capacity. In the event of a National Drug Shortage, NMC Pharmacy shall provide to CAPS any medication required for the manufacture of TPNS if Caps is unable to procure the medication and NMC Pharmacy has it available. Any medication provided to Caps will be for the sole use in preparation of TPN's for NMC Pharmacy. Once the shortage is resolved or at the end of the contract period, any unused medication will be returned to NMC Pharmacy. All medication provided by NMC Pharmacy will be returned to NMC Pharmacy. All medication provided by NMC Pharmacy will be purchased from FDA approved sources. CAPS will guarantee a monthly service level of 100% of all orders received prior to the cut off time as mutually agreed to in writing (1300 Hours Pacific Time). CAPS will strive to fulfill all change orders received after the cut off time but prior to physical shipment. Delivery company must have valid Rapid Gate Pass for entry to Naval Medical Center San Diego prior to start date of contract. Naval Medical Center, San Diego (NMCSD) will send the appropriate orders necessary for CAPS to accurately admix TPNs via direct computer interface. CAPS is responsible for ensuring CAPS proprietary software is updated and approved by the hospital's IT department. COMPLIANCE WITH APPLICABLE LAWS AND STANDARDS: CAPS shall comply with all federal, state and local laws, regulations and governmental orders, including applicable State Board of Pharmacy regulations and United States Pharmacopeia (USP) Chapter 797 and Good Compounding Practices (USP 1075), in providing items or services pursuant to this Agreement. CAPS shall comply with all applicable standards established by the Joint Commission. CAPS staff, affiliate personnel, and any others providing services shall be properly trained and credentialed and properly reviewed according to the requirements set forth by the Joint Commission. PARTS/SUPPLIES: Any equipment, component, or computer software provided to NMCSD for the purpose of transmitting prescription orders to CAPS (to include personal computer, monitor, printer, modem, and/or facsimile machine) shall be the sole property of CAPS. CAPS will be responsible for the maintenance of the aforementioned equipment, components, and computer software. CAPS is responsible for working with NMCSD's IT Medical Department to upgrade any software. LIABILITY: The contractor shall not be liable for any loss, damage, or delay due to any cause beyond his reasonable control including but not limited to, acts of government, strikes, lockouts, fire, explosion, theft, floods, riot, civil commotion, war, malicious mischief or acts of God. CONTRACTOR CHECK-IN/CHECK-OUT: The contractor, or his representative, is required to report to the Medical Repair Branch for Visitor Badges during the hours of 7:30am - 8:00pm, Monday through Friday, prior to and upon completion of any service performed. DELIVERY: Total Parenteral Nutrition(TPN) solutions and delivery services for Naval Medical Center San Diego to be provided in compliance with United States Pharmacopeia (USP) Chapter 797 and Good Compounding Practices (USP 1075). Local delivery logistics will be negotiated. Order cut-off time will be @ 1300 hours, and one (1) TPN delivery order will be provided per day at no charge to Naval Medical Center San Diego, Pharmacy Department.
