- This change is to clarify the Performance Work Statement (PWS), clarify the contracting office address, and proposal turn in. This advertisement is for two (2) positions: Athletic Trainer and Strength and Conditioning Trainer. The Athletic Trainer position criteria begins at 5.1 of the PWS and the Strength and Conditioning Trainer's criteria begins at 5.3. Proposals can be emailed to the Point of contacts listed below, mailed, or submitted in person. The address for the latter two is: 176 MSG/MSC, 17441 Airlifter Drive, JBER, AK 99506. Please note that proposals must be received in our office by the submission date/time. When providing proposals, please be sure to provide past performance and technical ability information along with the dollar amounts in the format below. This is to help the Government determine the best value. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format prescribed in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This requirement is a 100% small business set-aside, NAICS 611620. DESCRIPTION OF REQUIREMENT: The primary objective is to provide contractor services and deliverables through performance as a subject matter expert in the field of Human Performance Optimization (HPO). 212 RQS requires professional services to support the 212 RQS operators in Alaska for HPO services. These specific performance enhancement roles will be a Strength and Conditioning Trainer and Athletic Trainer. The purpose of procuring these services is to maintain 212 RQS operators at peak performance and facilitate rapid recovery from various injuries sustained. These services have been show to greatly reduce the time an Operator is unable to perform their mission as well as increase the longevity of our most critical assets; humans. These roles will be part-time with the 212 RQS team and training//treatment sessions will be scheduled appropriately to meet the scheduling needs of the unit. PERFORMANCE WORK STATEMENT Contractor shall perform the requirements as described in the attached Performance Work Statement (PWS) DEPARTMENT OF LABOR WAGE DECISION applicable to this acquisition is: WD 05-2017 (Rev.-17) dated 06/25/2013 and it is located at http://www.wdol.gov/. PERIOD OF PERFORMANCE: 25 Sept 2013 - 24 Sept 2014 LOCATION OF PERFORMANCE: Anchorage, AK 99506 CONTRACT LINE ITEMS - submit proposal pricing in the following format: CLIN 0001, Strength and Conditioning Trainer 12 Months @ $__________ per month $________ total CLIN 0002, Athletic Trainer 12 Months @ $__________ per month $________ total Firms that believe that they can provide similar required items must provide supporting evidence to the Contracting Officer. This announcement constitutes the only written notice. Basis for award will be the best value to the Government. In accordance with FAR 52.212-2, the Government will award a contract/purchase order resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. An integrated assessment will be made of each offeror's proposal in the areas of Past Performance, Technical Capability and Price. The Government will make award to the responsible offeror whose offer conforms to the PWS and presents the best value to the Government as the lowest priced technically acceptable proposal. Offerors are advised that the Government may make award without discussions. Therefore, proposals shall be submitted initially on the most favorable terms from a price and technical standpoint. Do not assume that offerors will be contacted or afforded an opportunity to clarify, discuss or revise their proposals. For a proposal to result in an awardable contract, the proposal must at least meet all PWS requirements, conform to all required terms and conditions, and contracted employees must possess all required certifications. To be eligible for contract award, all contractors must be registered with the System for Award management (SAM) found at https://www.sam.gov/portal/public/SAM/.
