- Attachment 'C' Attachment 'B' QUESTIONS & ANSWERS QUESTION #1: Do you have a proposal form / format that are to be filled out? ANSWER #1: Provide total estimate for each year. Base Year: ________ Option Year: _______ etc... QUESTION #2: Carpet colors are noted as TBD. Do you have minimum order quantities for each color established for the carpet? ANSWER #2: 10 square feet QUESTION #3: Sheet vinyl colors are noted as TBD. Do you have minimum order quantities for each color established for the sheet vinyl? ANSWER #3: 10 square feet QUESTION #4: Vinyl wall covering colors are noted as TBD. Do you have minimum order quantities for each color established for the vinyl wall covering? ANSWER #4: 10 square feet QUESTION #5: Is the carpet to be removed the same type product (vinyl back/power bond) as the new specified carpet? ANSWER #5: Yes, it is the same type QUESTION #6: What is the scope of work required for the 7,000 SF of drywall repair? Is this patching holes and repairing minor damage or does this include removal and replacement of existing drywall? ANSWER #6: Both QUESTION #7: Are seams in the sheet vinyl installations to be heat welded? ANSWER #7: Yes QUESTION #8: Is flash cove required for the sheet vinyl installations? If so what is the linear feet quantity? ANSWER #8: Yes, rough estimate for quantity is 2000 LF QUESTION #9: Can the proposal due date be extended to Friday, 9/13/2013? ANSWER #9: The due date is extended to 9/13/2013 QUESTION #10: Is there an existing similar type contract in place to fulfill NIH's Painting & Flooring needs? ANSWER #10: New Requirement QUESTION #11: Could you provide us with an example of a past Handyman requirement specifically what work was requested? ANSWER #11: Examples included in the statement of work. QUESTION #12: Could you provide us with a few different types of Flooring & Wall covering products that NIH has used in the recent past? ANSWER #12: See Attachment A of the SOW for specifications. QUESTION #13: During you most recent past contracts which method did NIH personal use to notify the Incumbent contractor of the specific work order requests i.e. Email, Daily meetings, Phone calls or Faxes? ANSWER #13: No recent contract, but if specific work is needed the contractor will be notified via email with Task/Delivery Order QUESTION #14: Is there a stated response time for Non-Emergency Work Order requests i.e. follow up confirmation or action within 4, 12 or 24 hours? ANSWER #14: Initial response within 24 hours with schedule to follow. QUESTION #15: Over the life of the most recent past similar contract could you provide us with an approximate dollar value of the contract? ANSWER #15: There are no exiting contracts (New Requirement) QUESTION #16: If you could provide us with an estimated number Work Requests that are open any at time. ANSWER #16: Approximately 20. QUESTION #17: Pricing will this contract be based on Per Man hour pricing or are you requesting unit cost pricing i.e. per sq. yd. carpet installation cost & per sq. ft. ANSWER #17: See Attachment ‘B' for pricing format QUESTION #18: Can an equal product be supplied if identified? We normal use a Shaw commercial product called Biolife. We do however have access to both products specified in the statement of work. ANSWER #18: No, we are matching existing building specs. Note there is also Armstrong Medintech in the facility. QUESTION #19: The Statement of Work Sections 2 and 3 indicate that documentation for work must be provided monthly including hours and materials. This is typical for similar IDIQ Contracts, and invoicing is often provided based on established labor rates for various trades and the agreed fixed rate mark up on the cost of materials installed. There is no pricing schedule or schedule of Contractor mark ups included in the SOW. There is no Wage Determination provided. ANSWER #19: See Attachment ‘C' for Wage Determination Revision #13 QUESTION #20: Please provide the Wage determination for this project. ANSWER #20: See Attachment ‘C' for Wage Determination Revision #13 or www.wdol.gov QUESTION #21: Please provide a pricing schedule indicating hourly labor rates and materials mark up; or lump sum fixed price. ANSWER #21: See Attachment ‘B' for Price Quote Format QUESTION #22: Evaluation Sheet - D. Price: Provide a fix price quote in accordance with the Statement of Work identified. This anticipated contract type is IDIQ, Is this per unit? ANSWER #22: Yes QUESTION #23: Will the government supply all of the hardware for the installation of these products? ANSWER #23: No. QUESTION #24: Are any products mounted to the ceiling or above? ANSWER #24: Yes. QUESTION #25: What are the heights of the Clocks from floor (i.e. will a ladder be sufficient or a man lift). ANSWER #25: Ladder is sufficient. QUESTION #26: What is the maximum and average height of walls and ceilings we will work on? ANSWER #26: Average 9' QUESTION #27: What type of suspended ceiling is it, 2x2 or 2x4, and what is the make of the ceiling and what type of ceiling tile. Will you supply the ceiling tiles and grid work? ANSWER #27: There is no ceiling tile replacement in this project. QUESTION #28: On your Attachment A* 40,000 square feet of prepping walls for painting, what type of walls CMU, Concrete, Drywall or plaster? ANSWER #28: Primarily drywall, some plaster and CMU. QUESTION #29: On your Attachment A * 7,000 square feet of dry wall repair; is this just repairing holes in walls or replacing whole sheets? ANSWER #29: Both. QUESTION #30: On your Attachment A * 3,000 square yards of carpet removal and prepping floor for new carpet: What type of carpet currently on floor, is it direct glue down or is it carpet tile? ANSWER #30: Primarily broadloom, direct glue limited amount of carpet tile. QUESTION #31: Are we allowed to put a dumpster on site if needed? ANSWER #31: No - dumpster is government provided. QUESTION #32: On the sheet vinyl: will there be any in laid work and if so what is the percentage? ANSWER #32: Very small percentage if any for pattern work. QUESTION #33: Also on the sheet vinyl, are you using the flash coving method, if so, will it be 4" or 6". ANSWER #33: 4" flash cove. QUESTION #34: On the VCT: Are you doing any pattern work? ANSWER #34: Limited, 10-15%. QUESTION #35: Will you need a separate price for patching and skim coating the floors for the sheet vinyl? ANSWER #35: Should be included in the flooring price.
