- Solicitation Packet Dear Ladies and Gentlemen: Reference: 13-247-SOL-00137 - Preventive Maintenance and Certification for the Phoenix Indian Medical Center. You are invited to submit a Request for Quote (RFQ) to the referenced solicitation. The Phoenix Indian Medical Center (PIMC) has a requirement for Preventive Maintenance and Certification performed to two (2) 500KW Emergency Generators and one (1) 800KW Emergency Generator by a factory authorized representative on a semi-annual and annul basis at the Phoenix Indian Medical Center, located in Phoenix, Arizona. Standard Form (SF) 1449, Solicitation/Contract/Order for Commercial Items, requires completion of section nos. 17a, 30a, 30b and 30c. Offerors are required to enter price proposal quotes on Pages 3, under Price & Costs Schedule. Offerors are required to submit a copy of their Indemnity & Insurance, on Page 8. Offerors are required to submit all required requested information in accordance to Section B.3, Submittals, on Page 29. All sections must be completed otherwise the IHS will not consider your RFQ. Complete proposal are due by 12:00 P.M., Phoenix, Arizona Time on September 11, 2013.
