SECTION A A.1 SF 1449 SOLICITATION/CONTRACT/ORDER FOR COMMERCIAL ITEMS Combined Synopsis/Solicitation for Commercial Items Downloadable Computer and Phone Application (App) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. (ii) Solicitation VA251-13-Q-0741 is issued as a request for quote (RFQ). (iii) The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular through 2005-69. (iv) This is solicitation is unrestricted under NAICS code 541511. The small business size standard for NAICS 541511 is $25.5 million. (v) STATEMENT OF WORK: The Department of Veterans Affairs National Center for Patient Safety (NCPS) has a requirement for a game application (APP) to be deployed that's compatible with Apple iPhone and iPad using open source code following a virtual medical scenario based off a "time-out" simulation developed by NCPS and employed prior to an actual thoracentesis procedure. Desired outcome: The Contractor shall provide a "Time-Out" App that shall take a player through a virtual "time-out" prior to their thoracentesis procedure. During the thoracentesis preparation the player shall take the role of a Medical Resident; other characters in the simulation which they interact with shall include a nurse, nurse aid, and an Attending Physician. Each of the other characters shall respond or react based on actions taken by the player. Occasionally and at random intervals other character shall enter and create a situation which requires engagement, such as interrupting, failing to wash their hands, or similar inappropriate actions. The scenario and virtual environment shall resemble a hospital room including all the props (furniture, oxygen, suction, IV poles and pumps, telephone, and equipment etc.) consistent with the environment. Sounds (alarms, beeps, overhead pages) and lights (steady, blinking, etc.) shall be consistent with the setting. Diagnostic Information such as X-rays, lab reports, or other medical chart information shall be available upon request by the Resident. Specific examples include: touching a view box to illuminate a chest x-ray, and touching a chart to open up the chart for labs and other pertinent information. Since the scenario is to assess the residents "time-out" attentiveness, actual procedures shall not be the dominant part of the scenario though the simulation may end with performance of a thoracentesis. A phone permits contact with Attending Physician, Radiology, Laboratory, etc. The Resident can interact and communicate to nurse, nurse aid, and Attending etc. Built in metrics measure any safety violations (e.g. failure to perform a time-out, mark the site, confirm the consent etc.) and produce a report for educators. Additional Requirements: Application must be 508 compliant, All source code will be open source, (OSEHRA), VA will own 100% of the source code, VA will own 100% of the data and distribution rights, Offer or shall present completed work ready for deployment on an Apple Device. There will be no access to VA systems required for the use of this App; it shall be a standalone App. The App will not include any Protected Health Information (PHI) or Personally Identifiable Information (PII). Key Personnel: Contractor shall have an approved APPLE App development person participating on the project and a Project Manager. These can be the same person. Management and Travel: All cost related to the ultimate roll out of APPs shall be borne by the Contractor. Pricing and Invoicing: Pricing shall be linked to one of the following titles: "Time Out" Game App, The Idea, App Design, Building the App, App Testing, App Submission and "Acceptance ready for Apple Invoicing by Contractor shall be submitted after each line item is accepted by Contracting Officer's Representative, COR. Post Award Contract Management: Post award contract management will be based on the contractor's project management plan, including timelines and milestones. Post award orientation conference will emphasize on the project management plan. DESCRIPTION QUANTITY UNITUNIT PRICE TOTAL PRICE "Time-Out" Game App1 EA $_________$___________ The Idea App Design1 EA $_________$___________ Building the App App Testing1 EA $_________$___________ App Submission and Acceptance 1 EA $_________$___________ Total Aggregate: $__________________ (vi) Downloadable Computer and Phone Application (App) (vii) FOB Destination Deliverable Schedule (all days are calendar days): 1.Within FIVE days after award the VA will convene a Post Award Orientation Conference which the contractor and their key representatives shall participate. 2.Within FIFTEEN days after award, the contractor shall submit a final Project Management Plan with a timeline and milestones for the development and ultimate presentation of the App to VA. 3.Within SEVEN days after submission of the final Project Management Plan with a timeline and milestones, VA will review and meet with the Contractor to address any special considerations 4.Contractor shall present a Monthly Status Report to the COR on the project no later than FIVE days after the end of the month the work was completed. 5.General Format for reporting shall use the following titles, i.The Idea (Refining the Concept) ii.App Design (Converting Concept to Plan - Including logic) iii.Building the App (Creating App and associated linkage relationships) iv.App Testing (Proof testing of App by VA) v.App Submission and Acceptance ready by Apple. 6.Contractor shall schedule meetings with VA at a minimum SEVEN days after submission of the Monthly Status Report. More frequent meetings may be required in the beginning to refine The Idea and work on App Design logic. 7.Contractor shall schedule at least THREE face to face meetings with VA, the first during the refining stage of the Idea; the second during the App Design stage; and third during the App Submission and Acceptance. All costs related to any face to face meetings and teleconference or video conferences will be borne by the contractor. (viii) Provision 52.212-1, Instructions to Offerors -- Commercial, applies to this acquisition. Offeror shall complete section 17a, 30a., and 30b. of the SF 1449 with their offer. (ix) Provision 52.212-2, Evaluation -- Commercial Items, if used, and applies to this acquisition. The following factors shall be used to evaluate offers: Lowest price technically acceptable. (x) Offerors shall include a completed copy of provision 52.212-3, Offeror Representations and Certifications -- Commercial Items, with their offer. (xi) Clause 52.212-4, Contract Terms and Conditions -- Commercial Items, applies to this acquisition. (xii) Clause 52.212-5, Contract Terms and Conditions Required To Implement Statutes Or Executive Orders -- Commercial Items, applies to this. (xiii) RFQ Point of Contact - Diana J.
