- Statement of Work SYNOPSIS/SOLICITATION FOR COMMERCIAL SERVICES PREPARED IN ACCORDANCE WITH THE FORMAT IN FAR SUBPART 13.5 and SUBPART 12.6 - STREAMLINED PROCEDURES FOR EVALUATION AND SOLICITATION FOR COMMERCIAL ITEMS-AS SUPPLEMENTED WITH ADDITIONAL INFORMATION INCLUDED IN THIS NOTICE. SIMPLIFIED ACQUISITION PROCEDURES ARE UTILIZED IN THIS PROCUREMENT. THIS ANNOUNCEMENT CONSTITUTES THE ONLY SOLICITATION; QUOTATIONS ARE BEING REQUESTED, AND A WRITTEN SOLICITATION DOCUMENT WILL NOT BE ISSUED. This solicitation is a Request for Quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect though Federal Acquisition Circular (FAC) 2005-33. The associated North American Industrial Classification System (NAICS) code for this procurement is 541519 with a size standard of $25.5M. The purpose of this procurement is to acquire a Commercial-Off-the-Shelf (COTS) solution that will improve the accessibility of SharePoint at the Health Resources and Services Administration (HRSA). I. PROJECT TITLE Accessibility Solution for SharePoint 2010 II. BACKGROUND The Health Resources and Services Administration's (HRSA) Office of Information Technology, Division of Web Support and Collaboration Services support SharePoint services for the Operating Division. Since HRSA's upgrade to SharePoint 2010, there has been a tremendous increase in user adoption and platform utilization. SharePoint 2010 now is used for internal collaboration, the HRSA intranet, business solutions that utilize workflow and some document and content management functions. However, SharePoint isn't completely accessible to persons with disabilities who use assistive technology and disabled users at HRSA have voiced complaints. III. PURPOSE/GENERAL DESCRIPTION The purpose of this procurement is to acquire a Commercial-Off-the-Shelf (COTS) solution that will improve the accessibility of SharePoint at the Health Resources and Services Administration (HRSA). Currently, SharePoint 2010 is not fully accessible to all users. HRSA staff who depend on assistive technology (i.e. JAWS screen reader or Dragon Naturally Speaking voice recognition) are limited in how they can use SharePoint. It is not the intent of this procurement to solve every SharePoint accessibility issue but rather fix the structural problems most frequently encountered by the typical end user. These include: -Working with documents in a SharePoint library (adding, deleting, editing) -Working with list items (adding, deleting, editing) -Using blogs and discussion boards -Responding to surveys -Basic administrative tasks (managing permissions and site access) IV. PERIOD OF PERFORMANCE The period of performance is 12 months from the effective date of the contract plus four (4) option periods for maintenance and support. V. PLACE OF PERFORMANCE The work required shall be performed at HRSA, Parklawn Building, 5600 Fishers Lane, Rockville, MD 20857 VI. TASKS The contractor shall: 1. Improve the accessibility of SharePoint 2010 and allows persons with disabilities who use typical assistive technology access to common SharePoint functions. 2. Facilitate the installation of the software on the HRSA SharePoint instance. 3. Provide a training overview to the HRSA SharePoint Team in order for them to be able to support end users. 4. Provide product maintenance and support. VII. SCHEDULE OF DELIVERABLES 1 Accessibility solution for SharePoint Within 5 days of award 2 Installation of the software Within 10 days of award 3 Training overview to SharePoint Team Within 15 days of award 4 Maintenance and Support Throughout the period of performance VIII. PAYMENT SCHEDULE The contractor shall be paid in 12 equal payments upon receipt and approval of invoice by the COR. AWARD CRITERIA The Government intends to award a firm fixed purchase order from this solicitation to the responsible Offeror whose quote provides the best value to Government. Evaluation of quotes will be based on the following technical factors. In the even TECHNICAL FACTORS: The offeror must meet all of the following technical factors: 1. Ability Improve the accessibility of SharePoint 2010 and allows persons with disabilities who use typical assistive technology access to common SharePoint functions. 2. Facilitate the installation of the software on the HRSA SharePoint instance. 3.Provide a training overview to the HRSA SharePoint Team in order for them to be able to support end users. 4. Provide product maintenance and support. PRICE QUOTE INSTRUCTIONS The Offeror shall submit a price quote, fully supported by cost and pricing data, in sufficient detail to allow for cost analysis that will be utilized to establish the reasonableness of the proposed costs. The Offeror's price quote shall consist of the following sections: A. Standard Form 1449, Solicitation/Contract/Order for Commercial Items The contractor must complete the following Blocks: BLOCK 12 State the discount terms. BLOCK 17 State the name and address of the individual providing the price quote. BLOCK 23 State the unit price. BLOCK 24 State the amount. BLOCK 30A Signature of the person authorized to commit your organization to the contract.
 BLOCK 30B Print or type the name and title of the person authorized to commit your organization to the contract. B. Supporting Documentation The Contractor shall furnish a price breakdown in support of the statement of work. Offerors must complete annual representations and certifications on-line at http://www.sam.gov. The following clauses apply to this acquisition:

52.212-4 Contract Terms and Conditions Commercial Items; 
52.212-5 Contract Terms and Conditions Required to Implement Statutes or Executive Orders Commercial Items including subparagraphs: 
52.222-3, Convict Labor; 
52.222-19 Child Labor - Cooperation With Authorities And Remedies; 
52.222-21, Prohibition of Segregated Facilities; 
52-222-26, Equal Opportunity; 
52.222-36, Affirmative Action for Workers with Disabilities; 
52.232-33, Payment by Electronic Funds Transfer-Central Contractor Registration. Offerors shall submit their quote in accordance with FAR 52.212-1(b). EACH SUBMITTED QUOTE SHALL ADDRESS ITEMS (1) THROUGH (11) OF FAR 52.212-1(b) as follows:
As a minimum, offers must show- 
(1) The solicitation number; 
(2) The time specified in the solicitation for receipt of offers; 
(3) The name, address, and telephone number of the Offeror; 
(4) A technical description of the services being offered in sufficient detail to evaluate compliance with the requirements in the solicitation. This may include product literature, or other documents, if necessary; 
(5) Terms of any express warranty;
(6) Price and any discount terms;
(7) "Remit to" address, if different than mailing address; 
(8) A completed copy of the representations and certifications at FAR 52.212-3 (see FAR 52.212-3(b) for those representations and certifications that the offeror shall complete electronically); 
(9) Acknowledgment of Solicitation Amendments.
(10) Past performance information, when included as an evaluation factor, to include recent and relevant contracts for the same or similar items, Contract dollar amount, and other references (including contract numbers, points of contact with telephone numbers and other relevant information); and 
(11) If the offer is not submitted on the SF 1449, include a statement specifying the extent of agreement with all terms, conditions, and provisions included in the solicitation. Offers that fail to furnish required representations or information, or reject the terms and conditions of the solicitation may be excluded from consideration.
The terms and conditions of the resultant purchase order will include FAR clause 52.212-4 and FAR clause 52.212-5. All questions regarding this request for quote shall be submitted in writing via e-mail to the Contract Specialist, Kwasi Antwi, at kantwi@hrsa.gov no later than Tuesday, September 10, 2013 at 4:00 pm EST. NO QUESTIONS WILL BE ACCEPTED AFTER THE DUE DATE. The Government shall not be liable for any costs incurred in response to this request. The Government reserves the right to make a single award, multiple awards, or no award as a result of this RFQ.
