- ORTHOPEDIC PHYSICIAN FOR FOUR DAYS A MONTH ON A BPA DESCRIPTION/SPECIFICATIONS/STATEMENT OF WORK Orthopedic Surgeon Services for the Pine Ridge Service Unit of the Aberdeen Area Indian Health Service. C-1 Purpose of the Contract The mission of the Pine Ridge Indian Health Service Hospital is to provide the best possible health care services to the Native American population on the Pine Ridge Sioux Indian Reservation in South Dakota. To meet the mission, the Pine Ridge Service Unit requires contractor support to assist the service unit in providing orthopedic surgeon services at Pine Ridge IHS Hospital. The purpose of the contract is for providing orthopedic surgery services to the Pine Ridge Indian Health Services Hospital. The care provided by the contractor shall be as comprehensive as the Indian Health Service supplies facilities, equipment, and support services permit. The Indian Health Service requires orthopedic services continue without interruption during the term of this contract. The contact resulting from this solicitation shall: 1. be a non-personal health care services contract under which the Contractor is an independent contractor: 2. allow the Government to evaluate the quality of professional and and administrative services provided, but retains no control over the medical, professional aspects of the services rendered, (e.g., professional judgements, diagnosis for specific medical treatment); 3. requires the Contractor to indemnify the government for any liability producing act or omission by the Contractor, its employees and agents occurring during contract performance; 4. require the Contractor to maintain medical liability insurance, in the coverage amount acceptable to the Contractor to maintain medical liability insurance, in the coverage amount acceptable to the contracting officer, which is not less than the amount normally prevailing within the local community for the medical specialty concerned; and 5. require the Contractor to ensure its subcontracts for provisions of health care service, contain the requirements of the clause at FAR 52.237-7, including the maintenance of medical liability insurance (see section 1) 6. The government reserves the right to terminate for convenience of the government in the event the Area Wide contract is awarded prior to completion of this contract period. C-2 GOVERNMENT FURNISHED PROPERTY/FACILITIES/SERVICES The service unit shall provide office space including a desk, chair, and office supplies. The contract physician will be authorized to use all areas of the medical treatment facility available to civil service and commissioned personnel in similar positions. The same restrictions apply to the contractor physician as any other Indian Health Service personnel to use these areas for official business activities only. The contract physician will be authorized to use all administrative and professional support available to Indian Health Service personnel. This will include but not limited to copy machines, fax machines, medical library, and telephone service. The same restrictions to use these items for official Indian Health Service business apply. The contract physician shall be provided with an orientation to the Indian Health Service and the Indian community being served. The initial orientation and continuing orientation shall be provided by the service unit personnel. The service unit shall provide all medical and non-medical equipment and supplies used in the Pine Ridge Indian Health Service facility for the care and management of patients. The service unit shall provide all required blank Government forms applicable to the Indian Health Service. An appropriate orientation to the necessity of such forms and the associated documentation will be provided by service unit personnel. The service unit shall provide the authority for the contract physician to purchase meals in the service unit dining room when available. When available, a pager shall be furnished by the service unit. The service unit shall provide an ongoing method of monitoring and evaluation of the quality and appropriateness of patient care performed before the level of performance becomes unacceptable. The service unit will provide patient transcription services. Legible handwritten documentation of medical records is acceptable in place of recorded dictation. C-3 CONTRACTOR FURNISHED EQUIPMENT/FACILITIES/SERVICES The Contractor shall provide the contract physician with an appropriate white physician coat and personal physician items such as stethoscopes, reflex hammers, penlights, etc. C-4 STATEMENT OF WORK The contractor will arrive one (1) business day prior to start of services at 8:00am for orientation purposes. The contractor will be required to utilize facility EHR (Electronic Health Record) for all patient encounter documentation. Experience is highly recommended. It is strongly recommended that the provider have the Flu and H1N1 vaccinations The Contractor shall furnish an Orthopedic Surgeon for primary care services at the Pine Ridge Indian Health Service Hospital. The Contract orthopedic surgeon shall perform physician services compatible with the Pine Ridge Indian Health Service medical facilities' operating capacity and equipment. The Contract Orthopedic services for Indian Health Service beneficiaries served by the Pine Ridge Service Unit. Contract Orthopedic physician services shall cover the range of orthopedic services typically provided in a rural civilian practice of similar size. Contract performance shall be according to the requirements contained in this Statement of work. The standards of medical practice and duties of the Contractor through its orthopedic physician shall be determined pursuant to the bylaws of the Pine Ridge IHS Hospital, the regulations of the hospital, applicable provisions of law, other rules and regulations of any and all governmental authorities relating to licensure and regulations of Physicians Accreditation of Healthcare Organizations. The Contract orthopedic physician shall be required to participate in a quality improvement program and requirements for ongoing monitoring and evaluation of the quality and appropriateness of care. Medical and surgical services shall not be performed by a Contract orthopedic physician until professional staff membership and clinical privileges have been granted in writing by the Indian Health Service Governing Body. As a prerequisite to perform under the resultant contract, the contract physician performing the services must be privileged in accordance with Indian Health Service/Aberdeen area credentialing policy. Therefore, compliance with the Indian Health Service privileging requirements is essential in the performance of the contract. The Governing Body will not grant clinical privileges until it is determined that credentials information has been verified. Exceptions to the Indian Health Service credentialing review and clinical privileging process will not be considered. Any Contract physician who fails to maintain medical staff appointment and clinical privileges will not be permitted to perform services under the contract and shall be promptly replaced by the Contractor. DUTY HOURS: The contract orthopedic physician will work four days per month and will work collaboratively with the surgeon/providers within the Pine Ridge hospital to arrange for coverage of the patients as needed. The Contractor shall locate, credential, and have available sufficient staffing for the outpatient clinic setting. The orthopedist shall work four days per month within the Pine Ridge Service Unit. Sufficient staffing shall include a registered nurse, clinic coordinator/administrative staff that will be needed to provide orthopedic physician services within the outpatient clinic settings. Pine Ridge Hospital will provide sufficient staffing for surgical procedures that will be performed by the orthopedist. The expected duty hours shall be two days within the clinic and two surgical days - flexibility will be expected to meet the patient needs. The precise work hours shall be established at the time of contract award and may be adjusted when mutually agreed upon. The contractor should contact the Clinical Director or his designated representative concerning any matter relevant to the Contract physician's assumption and discharge of responsibilities under the terms of this contract. The Contract Orthopedic physician shall advise the clinical director or his designated representative of any problem encountered, or which may be encountered in connection with meeting the needs of patients treated by the Contract orthopedic physician. The briefing shall also include conditions of any inpatients for whom the contract physician may reasonably be expected to attend during the shift. Continuing problems of any nature shall be reported to the Indian Health Service project office by the Contractor. The Contract orthopedic physician shall generate and maintain proper Medical record information on beneficiaries to whom treatment is provided in accordance with Indian Health Service regulations. The contract orthopedic physician shall prepare all medical record documentation to meet or exceed the established standards of the IHS facility to include but not limited to: timeliness legibility, accuracy, content, and signature. All medical record information generated in performance of this contract must be completed by the time of departure and will remain the property of and subject to the exclusive control of the Indian Health Service. Failure to meet this requirement of completion may result in non-approval of the Contract physician's last timesheet prior to the end of his/her assignment and resultant nonpayment, until such time that incomplete or delinquent records are completed. Treatment of IHS patients shall be performed at the IHS facilities except in certain instances when patients are referred to private facilities for performance of special diagnostic procedures or definitive care not offered in the IHS facility. All referrals to private facilities or providers for specialized diagnostic studies or definitive care not offered at the Pine Ridge IHS facility shall follow Indian Health Service regulations and policies on patient referrals and transfers. The Contract orthopedic physician shall become familiar with and be guided by the Indian Health Service formulary. The Contract Orthopedic physician shall follow Indian Health Service procedures when prescribing drugs. The orthopedic physician authorized to prescribe pharmaceuticals shall do so according to the availability of drugs listed in the IHS formulary. IHS pharmacy services will provide instructions to the prescribing contract practitioner as to substitutions of generic drugs for prescribed drugs. Drugs not on the formulary must be approved by the service unit pharmacy and therapeutic committee. The Contract orthopedic physician shall assess orthopedic services to assure its quality. The Contract orthopedic physician shall participate in a continual process of self-evaluation and strive for excellence in providing orthopedic physician services. The Contract orthopedic physician shall comply with all IHS facilities infection control and safety procedures, practices, and standards. The Contract orthopedic physician shall meet all applicable Joint Commission on Accreditation of Healthcare Organizations standards during the performance of this contract. The Contract orthopedic physician shall respect and maintain basic rights of patients, to include but not limited to personal dignity.
