- THIS IS A COMBINED SYNOPSIS/SOLICIATION - NO FURTHER SOLICIATION WILL BE ISSUED This is a combined synopsis/solicitation for commercial services prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This solicitation document is issued by the Minneapolis Veterans Health Care System, Network Contracting Office 23, for services being requested at the Fargo (ND) Veterans Health System, as a request for quotation (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Regulations (FAR). This procurement is being set-aside for FSS/GSA vendors, NAICS code: 621511. The Vendor shall provide the following service as part of this solicitation: oProfessional activities related to the pathology work-ups of renal and muscle biopsy specimens. This includes, but is not limited to, the review and interpretation of special stains, molecular studies, and electron microscopy studies required in the work-up of complex surgical pathology cases. This shall be a "base with option years" contract - specifically, the base year: October 1, 2013 through September 30, 2014, first option year: October 1, 2014 through September 30, 2015, second option year: October 1, 2015 through September 30, 2016, third option year: October 1, 2016 through September 30, 2017, and the fourth option year: October 1, 2017 through September 30, 2018. The provision at 52.212-1, Instructions to Offerors -- Commercial, applies to this acquisition. 52.212-2, Evaluation -- Commercial Items applies. Offerors must include a completed copy of the provision at, 52.204-8, Annual Representations and Certifications and submit with its offer. Vendors are required to be fully registered and active in the System for Award Management (SAM) at www.sam.gov prior to submitting quote response - this includes being fully updated in the Representations and Certifications area of SAM. Vendors shall base their quote response on the following, which will be used as evaluation criteria: 1.Vendor shall be within a proximity to Fargo (ND) which assures specimen integrity can be maintained in transit. 2.Vendor shall have adequate and appropriately educated staff to provide the entire spectrum of review and interpretation generally required for the pathological work-up of renal and muscle biopsy specimens. 3.Vendor shall have a routine turn-around time for reporting findings which is within a generally acceptable timeframe. 4.Vendor's cost per service 5.Vendor's past performance VA payment of this awarded purchase shall be made via signed VA purchase order.
