- Hoist Repair and Certification Clauses Quotes are due electronically to katina.barham@dot.gov by 3:00pm local time on September 13, 2013. Vendors are cautioned that they must have an active Central Contractor Registration profile in order to be considered for award. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number is DTMA-91-Q-2013-0018 and is issued as a Request for Quotations. This RFQ is 100% set-aside for Small Businesses. The NAICS code is 333923 with a small business size standard of 500 employees. The provisions at 52.212-1, Instructions to Offerors-Commercial, 52.212-2, Evaluation -- Commercial Items, 52.212-3, Offeror Representations and Certifications--Commercial Items, 52.212-4, Contract Terms and Conditions--Commercial Items and 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders--Commercial Items apply to this acquisition and are attached Quote Submission Instructions Interested parties shall submit a fixed price quote for services described above for the period of 3 weeks after award. The response package shall also include the following: 1. For a company quote submission : At least two references (for the company) for the same or similar type and complexity of the services required in the RFQ for work performed during the previous two calendar years. The references should include the current and accurate Point of Contact information along with a description of the services provided. The resume of the proposed individual which includes work history for the previous two calendar years. A choice previously prepared writing product (two page limit) for the individual offered. 2. For quote submissions by an individual : A resume which includes work history for the previous three calendar years which represents work experience for the same or similar type and complexity of the services required in the RFQ. The resume should include two references for the relevant work performed during the previous two calendar years including current and accurate POC information. The submission must also include a choice previously prepared writing product (two page limit). Statement of Work • 1. DESCRIPTION: This work shall consist of but not limited to providing all Inspections, Repairs, labor, tools, materials and equipment for the Emergency services necessary to repair the (9) overhead hoists located throughout Fulton Gibbs buildings, each hoist shall be tested, repaired as needed, certified and all documentation shall be supplied to the COR upon completion of all testing. Once system repaired operational testing shall be performed in all stages of operation with the presence of the COR and Instructor personnel. All work shall take place at Fulton / Gibbs hall located at the U.S. MERCHANT MARINE ACADEMY, KINGS POINT, N.Y 2. PERFORMANCE TIME: Work is to be completed within 3 weeks once P.O. approved, any changes shall be approved by the Contracting Officer. 3. CONTRACTOR RESPONSIBILITIES: Verify exact locations of the areas to be worked in, remove all damaged and unusable equipment and replace as needed. The contractor shall once the equipment has been replaced or repaired perform testing on all equipment and ashore operation is correct. All certification shall be applied to each hoist and all documentation shall be reviewed and handed to the COR for filing. Any damage to existing facilities or appurtenances shall be repaired or replaced, in kind, by the contractor at no additional cost to the Government. No vehicles shall be driven on the sidewalks or grass. 4. PRINCIPLE FEATURES: The work to be accomplished consists of but is not limited to the following principal items. The Contractor is instructed to include all such items of work as part of his/her proposal of the work to be performed. GENERAL •a. The Contractor shall field existing conditions prior to any construction activities. •b. All work performed is to conform to the latest edition of all applicable codes and standards. •c. The Contractor is required to obtain all permits and licenses necessary to complete construction and testing if applicable. •d. The Contractor will coordinate any disruptions in services for construction or testing with the COTR at least three (3) days in advance. The Contractor is required to make all possible efforts to minimize the impact to the daily routines of the Academy and reduce the time service will be affected. Project Submittals: •a. The contractor shall submit all material submittals and specifications for COTR'S approval prior to installation. PROJECT REQUIREMENTS : •a. Contractor shall provide all selected materials, equipment and tools necessary to perform the work. •b. The COTR shall be notified immediately of any problems during this project. •A CLOSE OUT/TESTING REQUIREMENTS: •a.
