- Attachment 2 - 'Questions and Answers' Attachment 1 - 'Past Perfomance Questionnaire' 13-233-SOL-00763_00002 (Amendment 00002 to the solicitation). *Amendment 00002 is issued as a result of the questions submitted during the solicitation. The questions and answers are incorporated into the solicitation via this amendment. Attachment 1 - "Past Performance Questionnaire" and Attachment 2 - "Questions and Answers" are attached to this amendment.* *Amendment 00001 corrects the stated deadline in Section 11.2 (page 48) of the base solicitation to submit questions regarding this solicitation. The (corrected) deadline for submitting questions is 3:00 p.m. (EDT) on September 11, 2013. The deadline for the receipt of proposals remains at 8:00 p.m. (EDT) on September 16, 2013, as stated in Section 11.2 (page 49) of the base solicitation.* Combined Synopsis/Solicitation "National Worksite Breastfeeding Support for Hourly-Wage Employers: Innovative Strategies for Success" This solicitation, number 13-233-SOL-00763, is issued as a request for proposals (RFP). This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued.
