The Department of Veterans Affairs Veterans Integrated Service Network (VISN) 3 is seeking to determine availability of SDVOSB or VOSB or other sources with the capability to provide Medical Testing, Esoterically and Genetic testing for the recipients that use the VISN3 VA Healthcare Systems at five (5) specific locations. The VISN 3 Healthcare Network will be requiring service at the following locations: Bronx, NY (110), NJHCS, NJ (561), Northport, NY(632),New York (VA HARBOR 106), and Brooklyn (VA HARBOR, 105) and other possible Community Based Outpatient Clinics. The worksheet attached in will act as a statement of work and has all information concerning the test needed and locations they will be needed. The contractor shall submit a plan concerning the security of the patients' identity. This is a salient requirement, and is not flexible. Firms with interest and capability to respond to this requirement should develop a short narrative including relevant experience and other information Indicating and evidencing capability to successfully meet the aforementioned requirements. Company information must include business type e.g. Service Disabled Veteran Owned Business, Woman Owned Small Business, etc, we would also ask that you review the column and inform us if the test can be purchased on FSS/GSA schedule or open market. Information should be provided no later than September 20, 2013. The worksheet included space for 27 different tests and include all sub test under the main test and also whether it is all inclusive or must be order separately. Please answer all questions per line, per test. We need to know what tests are not provide on schedule so we might pursue a different path, thank you in advance for your time and co-operation.
