Description
AMENDMENT W91ZLK-13-T-0294-0001 IS ISSUED TO INCLUDE 2 POSSIBLE OPTION YEARS. THE CLOSING DATE IS EXTENDED TO 09/13/2013 @ 10:00 AM EASTERN STANDARD TIME. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. The combined synopsis/ solicitation number is W91ZLK-13-T-0294. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 05-58. This requirement is Unrestricted. The Government contemplates award of a Firm-Fixed Price contract for routine laboratory consumable supplies. Description of Requirement: The USAPHC has a requirement for supply delivery services for Laboratory Non-Chemical consumables to support mission critical needs to include field monitoring, preparation of quality control samples and analyses of environmental and occupational health samples. The USAPHC laboratory accreditation requires the use of an approved vendor's list when procuring laboratory supplies. Using a vendor or company that is not on the approved vendor's list would result in the possible loss of laboratory accreditation and the USAPHC'S inability to perform accredited laboratory analysis. CLIN 0001 The contractor shall provide Environment Protection Agency (EPA) approved laboratory consumable supplies in accordance with the attached specifications on Attachment 1. Attachment 1 - List of non-chemical description, part number, unit, and approved vendor's name. These estimated quantities are intended for evaluation purposes only. The Government does not guarantee that the estimated quantities will be ordered during the order period. The offeror shall fill in all Line Items pricing information under Unit Price on Attachment 1. This shall be the pricing proposal. The offeror shall include the shipping cost into the overall cost of each consumable. Period of Performance: 12 months from date of award. SEE ATTACHED STATEMENT OF WORK : Offers shall note the lead time or approximate delivery date for items offered. The above requirement/items are for Brand Name for specific non-chemicals (See Attachment 1). The Associated North America Industry Classification Code (NAICS) 334516 and the Business Size Standard is 500. Acceptance shall be at destination. Shipping shall be FOB Destination to US Army Public Health Command (USAPHC) located at Aberdeen Proving Ground, Maryland 21010. The government will evaluate quotes and make a selection based on the Lowest Priced Technically Acceptable (LPTA) quotation. The following provisions and clauses will be incorporated by reference: 52.212-1 Instruction to Offerors Commercial Items 52.212-2, Evaluation Commercial Items The contract will be awarded to the Lowest Price Technically Acceptable (LPTA). The technical evaluation will be a determination based on information furnished by the vendor. The Government is not responsible for locating or securing any information which is not identified in the offer. The Government reserves the right to make an award without discussions. This solicitation is eligible for the US Army Contracting Command, Tenant Contracting Division, Aberdeen Proving Ground, MD, Executive-Level Agency Protest (ELAP) program, as an alternative to the usual provisions applicable for Agency protests under FAR 33.103.
