Description
Bid Schedule Statement of Work (SOW) Past Performance Questionnaire (PPQ) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subparts 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation. Quotes are being requested and a written solicitation will not be issued. The solicitation number is FA4497GREASETRAPS and this solicitation is being issued as a Request for Quote. The Offeror must quote on an all or none basis. The Government intends to award a Firm Fixed Price Requirements Contract of Base Period with Four Option Years. The Government will award a contract resulting from this solicitation to the responsible contractor that meets the minimum technical requirement at the lowest price. Since award will be made without discussions, it is highly encouraged to quote their most advantageous pricing in their initial response. The solicitation document, incorporated provisions, and clauses are those in effect through the Federal Acquisition Circular 2005-69 and Defense Acquisition Circular 20130808. This is a 100% small disabled veteran owned business set-aside. The North American Industry Classification System Code (NAICS) is 562991, size standard $7M. Notice to Offeror(s)/Supplier(s): Funds are not presently available for this effort. No award will be made under this solicitation until funds are available. The Government reserves the right to cancel this solicitation, either before or after the closing date. In the event the Government cancels this solicitation, the Government has no obligation to reimburse an offeror for any costs. Instructions to vendors: Note 1: To assure timely and equitable evaluation of the quote, the offeror must follow the instructions contained herein. The quote must be complete, self-sufficient, and respond directly to the requirements of this solicitation. The response shall consist of pricing for the items listed in the attached bid schedule and Statement of Work (SOW), as well as completed Past Performance Questionnaire(s). Note 2: The contractor agrees to hold the prices in its quote firm for at least 30 calendar days from the date specified for receipt of quote, unless another time period is specified. Note 3: The contractor shall include the provisions at 52.212-3, Representation and Certifications-Commercial Items. And shall be registered in System for Award Management (SAM) Registration: https://www.sam.gov/portal/public/SAM/ Note 4: Contractors are hereby notified that initial quotes which are found incomplete may be rejected without affording the contractor an opportunity to satisfy the requirement of this RFQ. Notwithstanding the information included in support of the offeror's technical capability, the contractor must meet all requirements of this Request for Quote. Note 5: Special Instructions Part I - PRICE PROPOSAL Insert proposed unit and extended prices in the Bid Schedule. The extended amount must equal the whole dollar unit price multiplied by the number of units. The proposal(s) must be submitted for a base year plus four (4) Option Years. Part II - PAST PERFORMANCE INFORMATION - Only references for the same or similar type contracts are desired. Past Performance Questionnaires (Attached): The government will evaluate the quality and extent of the offeror's performance deemed relevant to the requirements of this solicitation. The government will use information submitted by the offeror and other sources, such as other Federal government offices and commercial sources, to assess performance. Provide a list of no more than three (3) of the most relevant contracts performed for Federal agencies and commercial customers within the last three (3) years. The evaluation of past performance information will take into account past performance information regarding predecessor companies, key personnel who have relevant experience, or subcontractors that will perform major or critical aspects of the requirement when such information is relevant to the instant acquisition. Provision 52.212-2: Evaluation - Commercial Items (a)The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers; Price and Past Performance. Past Performance is approximately equal to price when being evaluated. The Contractor will send Attachment 3, Past Performance Questionnaire, to companies for which they have done similar work. The past performance shall be assessed based on the ratings mentioned in Attachment 3 (Exceptional, Good, Satisfactory, Marginal, Unsatisfactory, and Neutral). The questionnaire must come from the referenced companies, forms will not be considered if received from the offeror. (b) Options: The government will evaluate offers for award purposes by adding the total price for all options to the total price for the basic requirement. The government may determine that an offer is unacceptable if the option prices are significantly unbalanced. Evaluation of options shall not obligate the government to exercise the option(s) (c) A written notice of award or acceptance of an offer, mailed or otherwise furnished to the successful offeror within the time for acceptance specified in the offer, shall result in a binding contract without further action by either party. Before the offer's specified expiration time, the government may accept an offer (or part of an offer), whether or not there are negotiations after its receipt, unless a written notice of withdrawal is received before award. The Government intends to award without discussions. Therefore contractors are encouraged to offer their most advantageous/best quote. The contractor shall submit their proposal by 20 Sep 2013 at (1:00 PM) EST to the contracting office by email in order to be considered for award. Pricing shall include provision of all supervision, personnel, equipment, transportation, material, and other items and services necessary to test, maintain, and clean grease traps in accordance with the Statement of Work. The following provisions and clauses apply to this acquisition and can be viewed through internet access at the Air Force FAR Site, http://farsite.hill.af.mil. In accordance with 52.252-1 and 52.252-2. This contract incorporates one or more clauses by reference, with the same force and effect as if they were given in full text. Upon request the Contracting Officer will make their full text available, IAW FAR 52.107 or view in the sites mentioned in provision 52.252.-1 and Clause 52.252.2. Clause 52.217-8, Option to Extend Services The Government may require continued performance of any services within the limits and at the rates specified in the contract. These rates may be adjusted only as a result of revisions to prevailing labor rates provided by the Secretary of Labor. The option provision may be exercised more than once, but the total extension of performance hereunder shall not exceed 6 months. The Contracting Officer may exercise the option by written notice to the Contractor within 30 days The 6-month extension of services is IAW 52.217-8 "Option to Extend Services" and will be utilized if necessary. The 6-month extension of services is not to be considered part of the Option Year 4 period, and will be a separate option exercise if it is utilized. However, proposed pricing for the 6-month extension will be based on Option Year 4 pricing. The total evaluated price will be calculated as the sum of the offerors proposed prices for a basic period and all Option Year periods and the 6-month extension. Evaluation of Options/extensions shall not obligate the Government to exercise such Options or extensions. Clause 52.217-9, Option to Extend the Terms of the Contract (a) The Government may extend the term of this contract by written notice to the Contractor within 15 days; provided that the Government gives the Contractor a preliminary written notice of its intent to extend at least 60 days before the contract expires. The preliminary notice does not commit the Government to an extension. (b) If the Government exercises this option, the extended contract shall be considered to include this option clause. (c) The total duration of this contract, including the exercise of any options under this clause, shall not exceed 5 years and 6 months (if extended).
