This is a combined synopsis/solicitation for commercial items IP CAMERA AUDIO VISUAL TELECONFERENCING SOLUTION FOR THE VA LONG BEACH HEALTHCARE SYSTEM prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. The solicitation number is VA261-13-Q-1565 and is issued as an (RFQ) Request For Quotation, unless otherwise indicated herein. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular FAC 2005-69. The associated North American Industrial Classification System (NAICS) code for this procurement is 561621 with a small business size standard of 19 million. This requirement is solicited as TOTAL SMALL BUSINESS SET- ASIDE and only qualified vendors may submit quotes. ALL OFFERS ARE DUE BY 1:00PM PST on TUESDAY, 17 OCTOBER, 2013. Introduction The purpose of this Statement of Work (SOW) is to outline the tasks and responsibilities of the successful offeror in relation to the VA Long Beach Healthcare System (VALBHS) conference room project for the installation and integration of additional cameras and remote viewing of the Pharmacy Narcotics area, building 164. 1.1 Background The VALBHS at 5901 East 7th Street, Long Beach, CA 90822 is part of the Desert Pacific Healthcare Network (VISN 22), and is one of the most diversified tertiary health care providers in the VA. It is a teaching hospital, providing a full range of patient services, with state-of-the art technology as well as education and research. 2. Scope The breakdown of the conference rooms is as follows: Install 4 cameras and a monitor in the Pharmacy area 3. Tasks 3.1 General Requirements "The vendor shall submit a cost proposal with an associated project management plan addressing the tasks associated with the SOW, as described in the evaluation criteria section. "Subtasks not specified in the SOW will be identified and include associated costs by project task, milestones and deliverable dates. "All written deliverables must be phrased in terms and language that can be easily understood by non-technical personnel, i.e. a person without subject matter expertise. "All document deliverables, hard copy and electronic, must be in formats as specified-at a minimum, the formats must be in industry accepted standards. "A kickoff meeting will be held at a location and time selected by the VALBHS where the vendor and its staff will be introduced. "The vendor and/or its staff must have knowledge and expertise of the environment for which the work is to be performed. "The vendor shall provide total system integration for each of the conference rooms listed above. 3.2Mandatory Tasks "Install 4 cameras and monitor in Pharmacy area, Building 164. "Equipment shall be substituted for equipment within Attachment but shall still confirm to the requirements stated below. Building 164: Conference Room 328 Patient Education Suite "4 Linil Low light color cameras with needed ceiling mounting bracket (seismic reinforced) "One 42" color monitor Wall mounted "One 8 port PoE Cisco Switch "One 4 port IP Video Decoder/splitter "Four (4) OnSSI Ocularis CS camera licenses "All cabling, mounting and associated electronics for full integration of equipment requirements (projector, video conferencing and computer equipment) 3.2.1Implementation 3.2.2 Change Management Plan "The vendor shall provide a Change Management Plan to document procedures for additions, deletions, and modifications to the Scope of Work and the Mandatory Tasks as outline in section 3.2. 3.2.3 Hours of Operation "Normal tour-of-duty hours at the VALBHS are 7:30 am to 5:00 pm, Monday through Friday, excluding Federal holidays. There may be instances in which the vendor's workday may be outside of this period. The vendor is required to coordinate these times with the designated Program Manager. " In accordance with 5 U.S.C. 6103, Executive Order 11582 and Public Law 94-97 the following national holidays are observed and for the purpose of this contract are defined as "Legal Federal official holidays" New Year's DayJanuary 1st Martin Luther King's BirthdayThird Monday in January President's DayLast Monday in January Memorial DayLast Monday in January Independence DayJuly 4th Labor DayFirst Monday in September Columbus DaySecond Monday in October Veteran's DayNovember 11th Thanksgiving DayFourth Thursday in November Christmas DayDecember 25th When a holiday falls on a Sunday, the following Monday will be observed as a National Holiday. When a holiday falls on a Saturday, the preceding Friday is observed as a National Holiday by U.S. Government Agencies. 3.2.4Acceptance/Period of Performance "Upon completion of installation and training, the VALBHS shall monitor and verify the equipment and installation during operations as outlined by the manufacturer for a total of six (6), contiguous, consecutive, uninterrupted business weeks. This period of 6 weeks shall be the acceptance phase of the installation. The vendor is required to supply the VALBHS with an acceptance form to be signed off at the end of the acceptance phase to indicate possession of the installation and equipment and to inform the Contracting Office to proceed with payment(s) as outlined in the Purchase Order. "Any problems with the system must be isolated and repaired by the vendor during the acceptance period. The vendor will troubleshoot the hardware, software, and communications/ancillary equipment to determine the best method of repair. The vendor shall provide brief descriptive resolution of all maintenance. Please note that any problems with the system during the acceptance phase will result in a reset of the acceptance phase, resulting in a new 3-week acceptance phase as outlined above. "The vendor and the VALBHS shall jointly cooperate in the development and implementation of a Quality Assurance Plan (QAP). The QAP shall document the process that will verify and validate quality assurance in compliance with the requirements and ensure these requirements meet the VALBHS's expectations. "The vendor shall develop the QAP based on accepted industry standards such as the CMMI SM, ISO 9000, IEEE 12207, or other accepted industry QA standards. The QAP shall detail the processes, procedures, and metrics for assuring quality. The QAP shall also include, at a minimum, a description of the following key activities: "Establishment of capable processes "Development and implementation of procedures to identify, prevent, and ensure non-recurrence of defective services "Establishment of mechanisms for feedback of performance "Monitoring and control of critical processes "Establishment of a designated point of contact (POC) to serve as the POC for quality control issues/complaints "Establishment of reliable methods of record keeping and documentation of all corrective actions and process improvements undertaken during the acceptance period 4.Vendor Responsibilities "The vendor shall be responsible for any and all liability actions or suits when the vendor is at fault or negligent in the performance of installation and testing activities. Pecuniary liability is also included for any and all damages to VA buildings and equipment caused by installation activities. The vendor shall comply with all local building and fire codes in addition to VA policy and procedures at the site. "The vendor shall operate under, but not limited, to the following national codes, telecommunications industry standards and practices: National Electric Code (NEC), Building Industry Consulting Service International (BICSI), all standards, Electronic Industries Association/Telecommunications Industry Association EIA/TIA, Americans with Disabilities Act (ADA), National Fire Protection Association (NFPA), Underwriters Laboratories (UL), ITU and IETF and all applicable audio, video, communication, data and control standards. "The vendor shall provide the VALBHS with two complete sets of detailed drawings/prints in both electronic and hard copy format, of the finished, installed AV system for each conference rooms. Drawings must be in industry accepted standards/format (e.g. MS Visio, Autodesk AutoCAD). "Prior to commencing work, vendor shall provide proof that an OSHA certified "competent person" (CP) (29 CFR 1926.20 (b) (2) will maintain a presence at the work site whenever the installers are present. 5.VALBHS Responsibilities "The VALBHS shall provide all high voltage wiring panels, breakers, relays, boxes, and receptacles and any related electrical work including, but not limited to 110VAC. "The VALBHS shall provide all the necessary core drillings as specified in the original design. Design changes and modifications presented as an addition by the vendor shall be the responsibility of the vendor. "Cutting, structural welding, or reinforcement of structural steel members required for support of assemblies, if required "Any necessary floor penetration, ceiling tile replacement, painting or patching as specified in the original design. "Any modifications or changes to the vendor's design to accommodate additional VA equipment. "THE VALBHS shall provide the vendor a list of key personnel associated with the project along with the name and contact information for the POC/Project Manager. "The Project Manager shall provide access to the appropriate personnel (management, technical, subject matter expertise, etc.) necessary to fulfill the vendor's requirements in a timely fashion. 6.Travel "All travel shall be responsibility of the vendor. "Whenever possible, conference calls and video conferencing should be used. 7.Security 7.1 Information Security "The vendor shall ensure adequate LAN/Internet, data, information, and system security in accordance with VA standard operating procedures and standard contract language, conditions laws, and regulations. The vendor's firewall and web server shall meet or exceed the government minimum requirements for security. All government data shall be protected behind an approved firewall. Any security violations or attempted violations shall be reported to the VA Project Manager and the VBA Headquarters Information Security Officer as soon as possible. The vendor shall follow all applicable VA policies and procedures governing information security, especially those that pertain to certification accreditation. "The contractor shall follow The Veterans Affair Acquisition Regulation (VAAR) security clause VAAR- 852.273-75 "SECURITY REQUIREMENTS FOR UNCLASSIFIED INFORMATION TECHNOLOGY RESOURCES" sited in section C. Contract Clauses (a) The contractor and their personnel shall be subject to the same Federal laws, regulations, standards and VA policies as VA personnel, regarding information and information system security. These include, but are not limited to Federal Information Security Management Act (FISMA), Appendix III of OMB Circular A-130, and guidance and standards, available from the Department of Commerce's National Institute of Standards and Technology (NIST). This also includes the use of common security configurations available from NIST's Web site at: http://checklists.nist.gov (b) To ensure that appropriate security controls are in place, Contractors must follow the procedures set forth in "VA Information and Information System Security/Privacy Requirements for IT Contracts" located at the following Web site: http://www.iprm.oit.va.gov 7.2 Contract Personnel Security All contractor employees who require access to the Department of Veterans Affairs' computer systems shall be the subject of a background investigation and must receive a favorable adjudication from the VA Security and Investigations Center (07C). The level of background security investigation will be in accordance with VA Directive 0710 dated September 10, 2004 and is available at: http://www.va.gov/pubs/asp/edsdirec.asp (VA Handbook 0710, Appendix A, and Tables 1 - 3). Appropriate Background Investigation (BI) forms will be provided upon contract (or task order) award, and are to be completed and returned to the VA Security and Investigations Center (07C) within 30 days for processing. Contractors will be notified by 07C when the BI has been completed and adjudicated. These requirements are applicable to all subcontractor personnel requiring the same access. If the security clearance investigation is not completed prior to the start date of the contract, the employee may work on the contract while the security clearance is being processed, but the contractor will be responsible for the actions of those individuals they provide to perform work for the VA. In the event that damage arises from work performed by contractor personnel, under the auspices of the contract, the contractor will be responsible for resources necessary to remedy the incident. 8. Completion Date The vendor shall have all services rendered and equipment received no later than 91 working days from the date of Contract Award and receipt of Contract Award. This solicitation requires registration with the System for Award Management (SAM) prior to award, pursuant to applicable regulations and guidelines. Registration information can be found at www.sam.gov. The selected Offeror must comply with the following commercial item terms and conditions, which are incorporated herein by reference: FAR 52.212-1, Instructions to Offerors - Commercial Items, applies to this acquisition; FAR 52.212-3, Offeror Representations and Certifications - Commercial Items - the selected offeror must submit a completed copy of the listed representations and certifications; FAR 52.212-4, Contract Terms and Conditions - Commercial Items; FAR 52.212-5, Contract Terms and Conditions Required To Implement Statutes or Executive Orders-Commercial Items, paragraph (a) and the following clauses in paragraph (b): 52.222-21, 52.222-26, 52.222-35, 52.222-36, 52.222-37, 52.225-13, 52.232-34. The following VAAR clauses are also applicable to this acquisition: VAAR 852.203-70 COMMERCIAL ADVERTISING (JAN 2008) The bidder or offeror agrees that if a contract is awarded to him/her, as a result of this solicitation, he/she will not advertise the award of the contract in his/her commercial advertising in such a manner as to state or imply that the Department of Veterans Affairs endorses a product, project or commercial line of endeavor. (End of Clause) VAAR 852.237-70 CONTRACTOR RESPONSIBILITIES (APR 1984) The contractor shall obtain all necessary licenses and/or permits required to perform this work. He/she shall take all reasonable precautions necessary to protect persons and property from injury or damage during the performance of this contract. He/she shall be responsible for any injury to himself/herself, his/her employees, as well as for any damage to personal or public property that occurs during the performance of this contract that is caused by his/her employees fault or negligence, and shall maintain personal liability and property damage insurance having coverage for a limit as required by the laws of the State of California. Further, it is agreed that any negligence of the Government, its officers, agents, servants and employees, shall not be the responsibility of the contractor hereunder with the regard to any claims, loss, damage, injury, and liability resulting there from. (End of Clause) VAAR 852.246-70 GUARANTEE (JAN 2008) The contractor guarantees the equipment against defective material, workmanship and performance for a period of, said guarantee to run from date of acceptance of the equipment by the Government. The contractor agrees to furnish, without cost to the Government, replacement of all parts and material that are found to be defective during the guarantee period. Replacement of material and parts will be furnished to the Government at the point of installation, if installation is within the continental United States or f.o.b. the continental U.S. port to be designated by the contracting officer if installation is outside of the continental United States. Cost of installation of replacement material and parts shall be borne by the contractor.
