Department of Veterans Affairs 7305 N. Military Trail West Palm Beach FL 33410 Cost Per Test(CPT) for PT/PTT and D-dimer Notice of Intent The Veterans Affairs Medical Center in West Palm Beach, FL intends to award an order off of a FSS/NAC contract with one Contract Line Item Number(CLIN) that is open market, per the FAR Part 8.402 (f)(1) requirement, this notice serves as publicizing per FAR Part 5. This award is for a CPT of PT/PTT and D-dimer, the latter being an open market item. The period of performance is for a base and four(4) option years with an estimated total value of $608,670. The awardee is to be INSTRUMENTATION LABORATORY'S, 620 WOODLOT TRAIL, ANNAPOLIS, MD 21401 Please contact the undersigned for any questions or concerns regarding the award.
