U.S. Department of Veterans Affairs VA Insurance Center Philadelphia, PA STATEMENT OF WORK Pressure Sealer Maintenance - Philadelphia VA Insurance Center A. GENERAL INFORMATION 1.Overview of Project: The Philadelphia VA Insurance Center's (VAIC) current pressure sealer was installed in 2009. This pressure sealer glues and seals outgoing mail from VAIC. VAIC requires maintenance on its pressure sealer so that the pressure sealer will remain fully operation and suitable to manage all current mailing output from the VAIC mainframe computer. 2.Scope of Work: The Contractor shall provide all maintenance necessary to keep pressure sealer in a fully operation condition throughout duration of maintenance agreement. At a minimum, Contractor shall provide VAIC with the following: a. Maintenance Support as follows: i. "On call" maintenance 1.Response time within 2 hours from 6:00 a.m. to 6:00 p.m., Monday through Friday, excluding Federal holidays 2.Response time within 4 hours where maintenance is required outside the period of hours above 3.Response time within 4 hours where additional engineering assistance is required to maintain the continual operation of the system and/or associated equipment 4.Where maintenance personnel fail to arrive within required response time, Contractor shall grant VAIC credit in excess of specified response time and up to actual arrival time ii. Preventive Maintenance 1.Shall be performed during off-peak period of any given month 2.Contractor shall specify in writing frequency and duration of preventive maintenance required for all covered equipment 3.If mutually agreed upon schedule for preventive maintenance cannot be established, VAIC reserves right 9/23/2010 to specify schedule performance of preventive maintenance, subject to time constraints above iii. Remedial Maintenance 1. If equipment and/or operating software is inoperative: a. Contractor shall provide VAIC with designated point(s) of contact to enable maintenance representative to: i. Receive such notification; or ii. Provide answering service or other continuous telephone coverage to permit Contractor to make such contact b.Malfunction Reports as follows: i. Contractor shall furnish signed malfunction incident report to Contractor upon completion of each maintenance call ii. Report shall include the following: 1.Date and time notified 2.Date and time of arrival 3.Time spent for repair 4.Description of malfunction c.Maintenance Coverage as follows: i. Contractor shall honor orders for maintenance under any of the maintenance options set forth in this document for periods of one (1) year or less ii. These same maintenance provisions shall cover all equipment iii. Effective date of maintenance service shall not be prior to the expiration of the warranty period d.Parts i. Only new standard parts or parts equal in performance to new parts shall be used in effecting repairs e.Installation of Parts i. Only-Contractor trained and certified technicians shall conduct installation or maintenance on equipment 1. Technicians shall be fully trained and certified a. Technicians shall be fully trained in all components of equipment and be certified in full range of duties required to service equipment ii. Contractor shall provide resumes and security clearance documentation of individuals proposed for all aspects of field engineering services 3.Background: VAIC is responsible for administering six government life insurance programs for veterans and service members. This responsibility includes mailing all necessary correspondence to support policy services and claims activities of a large insurance company. 4.Performance Period: The period of performance is one (1) base year and four (4) option years. 9/23/2010 5.Type of Contract: Firm-Fixed-Price 6.Place of Performance: 5000 Wissahickon Ave. Philadelphia, PA 19144 - </S i 7 7.Performance Based Contract: N/A B.CONTRACT AWARD MEETING The contractor shall not commence performance on the tasks in this SOW until the CO has conducted a kick off meeting or has advised the contractor that a kick off meeting is waived. C.GENERAL REQUIREMENTS 1.For every task, the contractor shall identify in writing all necessary subtasks (if any), associated costs by task, together with associated sub-milestone dates. The contractor's subtask structure shall be reflected in the technical proposal and detailed project management plan (PMP). 2.All written deliverables will be phrased in layperson language. Statistical and other technical terminology will not be used without providing a glossary of terms. 3.Where a written milestone deliverable is required in draft form, the VBA will complete their review of the draft deliverable within 5 calendar days from date of receipt. The contractor shall have 5 calendar days to deliver the final deliverable from date of receipt of the government's comments. D.SPECIFIC MANDATORY TASKS Description of Tasks and Associated Deliverables: The Contractor shall provide the specific maintenance as described below within the performance period stated in Section A.4 of this SOW. a. Contractor shall provide resumes and security clearance documentation of individuals proposed for all aspects of field engineering services Task One: The Contractor shall provide maintenance for the pressure sealer per the SOW. G. CHANGES TO STATEMENT OF WORK Any changes to this SOW shall be authorized and approved only through written correspondence from the CO. A copy of each change will be kept in a project folder along with all other products of the project. Costs incurred by the contractor through the actions of parties other than the CO shall be borne by the contractor. J. GOVERNMENT RESPONSIBILITIES -N/A O. ELECTRONIC AND INFORMATION TECHNOLOGY STANDARDS SECTION 508 The contractor shall comply with Section 508 of the Rehabilitation Act (29 U.S.C. § 794d), as amended by the Workforce Investment Act of 1998 (P.L. 105-220), August 7, 1998. In December 2000, the Architectural and Transportation Barriers Compliance Board (Access Board), pursuant to Section 508(2)(A) of the Rehabilitation Act Amendments of 1998, established Information Technology accessibility standards for the Federal Government. Section 508(a)(1) requires that when Federal departments or agencies develop, procure, maintain, or use Electronic and Information Technology (EIT), they shall ensure that the EIT allows Federal employees with disabilities to have access to and use of information and data that is comparable to the access to and use of information and data by other Federal employees. The Section 508 requirement also applies to members of the public seeking information or services from a Federal department or agency. CONFIDENTIALITY AND NONDISCLOSURE It is agreed that: 1. The preliminary and final deliverables and all associated working papers, application source code, and other material deemed relevant by the VA which have been generated by the contractor in the performance of this task order are the exclusive property of the U.S. Government and shall be submitted to the CO at the conclusion of the task order. 2. The CO will be the sole authorized official to release verbally or in writing, any data, the draft deliverables, the final deliverables, or any other written or printed materials pertaining to this task order. No information shall be released by the contractor. Any request for information relating to this task order presented to the contractor shall be submitted to the CO for response. 3.Press releases, marketing material or any other printed or electronic documentation related to this project, shall not be publicized without the written approval of the CO. 9/23/2010 Attachment A SECURITY BACKGROUND INVESTIGATION INFORMATION (Submit after award and prior to contract performance) Complete this form after contract award if contractor employee does not possess a NACI clearance.
