Description
Description: This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Solicitation Number VA250-13-R-0872 is being issued as Request for Proposal (RFP) and the intent is to award an indefinite quantity, FIRM FIXED PRICE contract for services to multiple contractors. The contract duration is for six (6) months, with no option periods to be exercised. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-69. This requirement is unrestricted-insufficient small business for commercial items under the North Atlantic Industrial Classification System (NAICS) 624221; size standard/$10 million per annum, temporary shelters. A.Price/Cost Schedule PURPOSE The purpose of this contract is to engage community providers (Contractors) who will provide emergency housing to Veterans along with services consistent with Health Care for Homeless Veterans (HCHV) program objectives. The goal of this contract is to remove homeless Veterans or Veterans with families from the street or places unfit for human habitation and place them in community-based, residential environments. These residences must have sufficient services to meet the needs of the Veterans or Veterans with families served. Multiple contracts may be awarded. Prospective Contractors will be considered based on proposals detailing how each prospective contractor is able to complete the Tasks, Deliverables and other requirements of the following Performance Work Statement for the Cuyahoga County catchment area in Northeast Ohio. 1)Five (5) units in Cuyahoga County which will house any combination of the following: a)Female Veterans with children / male Veterans with children / couples with or without children / single male Veterans / single female Veterans. 1)As children will be occupying units in this facility, sex offenders are prohibited from being admitted to this facility. 2)Regarding the couples with or without children population, one of the couple must be a Veteran. CONTRACT MINIMUM GUARNATEE AND MAXIMUM The total minimum order guarantee for services that will be acquired under this contract will be one (1) bed per day. The total maximum order for services that will be acquired under this contract shall not exceed five (5) beds per day. ORDERING OFFICIALS Ordering Officials are Louis Stokes Cleveland VAMC personnel (i.e. Contracting Officer's Representative (COR) authorized by the Network Contracting Office (NCO) 10 Contracting Officer. A list of authorized ordering personnel shall be made available upon award of contract. VA personnel may be added or deleted from the list during the term of the contract at the discretion of the Contracting Officer. The Contractor shall be notified of the changes as they occur. ORDERING PROCEDURES When a Veteran is approved for the HCHV program by the Ordering Official (COR), the Ordering Official will contact the Contractor to make arrangements to place the Veteran in a bed. A faxed information sheet shall follow each telephone authorization as soon as possible of agreed upon approval date. The Contractor shall admit the Veteran(s) or Veteran(s) with family to a residential bed(s) within 24 hours. SIX (6) MONTH CONTRACT PERIOD: CLINDESCRIPTIONESTIMATE QUANTITY UNITRATE PER BEDTOTAL 11 DAY OF SERVICE PER VETERAN5BED/DAY/188 Days ** VA estimates that it will require placement for approximately 5 Veterans every day during the 6 month contract period. ** NEED TO ADD IN PRICE COLUMN FOR FAMILY PRICING B.PERFORMANCE WORK STATEMENT (PWS) A.BACKGROUND. Through HCHV, VA provides case management and outreach services to Veterans and facilitates their access to a broad range of medical, mental health, and rehabilitative services. HCHV's goal is to engage homeless Veterans in treatment and place them in a safe and secure environment that supports their rehabilitation goals. The HCHV Program, an essential and critical part of VHA, is vital for providing a gateway to VA and community supportive services for eligible Veterans who are homeless. The HCHV Contract Residential Program for homeless Veterans is a vital program focused on addressing the needs of the most vulnerable and at-risk homeless Veterans. Ensuring that Veterans can be placed in community-based programs which provide quality housing and services that meet the needs of this special population is a particular focus of the HCHV program. B.TASKS AND ASSOCIATED DELIVERABLES. The Contractor shall be responsible for planning and coordinating all activities associated with providing safe and appropriate residential care. The Fixed Price rate established will include the services listed in the document and will also include all services normally provided to other patients by the facility without extra charge. The Contractor shall admit Veteran(s) or Veteran(s) with family to a residential bed(s) within 24 hours of agreed upon admission date between Contractor and VA COR. Specific tasks are as follows: Task 1: Housing Available for Rapid Placement Contractor shall be responsible for identifying sufficient residential capacity to place Veterans or Veterans with families in safe, community-based residences. Identification of suitable residential placement must require: a) Sufficient personnel to assure security. b) Staffing available 24 hours a day, 7 days per week. c) Provide to 3 healthy meals per day. d) Available laundry facilities. e) Room and Board to include a bed and other furnishings such as a dresser, storage, and linen for the bed. f) Adequate separation of genders is required to ensure privacy. g) Available rooms for families with children, and child care professionals readily available on the facility's premises, or child care professionals can be accessed as needed to serve children's needs. No person who has been convicted of, is convicted of, has pleaded guilty to, or pleads guilty to either a sexually oriented offense or a child-victim oriented offense can be admitted to Cuyahoga County contract facility. h) The facility shall be handicapped accessible in accordance with the 2010 ADA Standards for Acceptable Design. If the facility is more than one story, there shall handicapped accessible rooms on the first along, along with at least one handicapped accessible bathroom. i) The facility shall be in reasonable proximity to transport to VA service centers. Task 1 Deliverables: Housing will be available for rapid replacement within 24 hours of the agreed upon admission date with maximum availability of 6 beds per day specified in this contract at the per diem rate. Task 2: Delivery of Support Services Contractor is expected to offer supportive services to Veterans that will provide assistance to them during their contract residential stay, including case management and cooperation with VA resources. These services will continue throughout their authorized stay, generally not to exceed 90 days. The Contractor shall determine therapeutic, rehabilitative, and recovery services needed by the Veteran resident in a plan developed by the Contractor with consultation by the Veteran resident and appropriate VA staff. The supportive services are listed below: a) Ability to provide structured group activities, as appropriate - examples include group therapy, social skills training, Alcoholics Anonymous, Narcotics Anonymous, vocational counseling and physical activities as appropriate; b) Ability to collaborate with VA program staff, as appropriate. ; c) Contractor must ensure individual counseling is being provided (either through direct service or referral): d) Ability to provide assistance with health education and personal hygiene, as appropriate. e). Ability to monitor medications; f). Ability to provide supportive social services, in collaboration with the HCHV program; g) Ability to provide support for an alcohol and drug free lifestyle by maintaining a drug and alcohol free environment; h) Ability to provide assistance with linkages to transportation resources; i) The ability to assess and respond to a Veteran in crisis and make appropriate referrals for assistance; j) The ability to refer to alternate residential services for veterans with substance abuse disorders that are affecting the residential placement. k) The ability and willingness to provide services to Veterans with previous history of criminal offenses; l) The Contractor serving a Veteran's family understands they will only be reimbursed for direct services to the Veteran. However, the general costs of serving families (i.e.: larger rooms, day care, use of water) should be build in cost of serving veterans. Task 2 Deliverables: The Contractor shall provide a detailed service plan, mentioning the supportive services the veteran resident will partake, within one (1) week after admission into the Contractor's facility. Task 3: Status Reports Contractor shall be responsible for providing VA with documentation demonstrating that placement and support service objectives are being met throughout the contract period. VA intends to conduct both periodic fiscal and performance reviews of the selected Contractor. Task 3 Deliverables: 1. The Contractor shall make available to the VA, documentation deemed necessary by the VA to conduct inspection review audits (during initial inspection and at the beginning of each following year) audits to verify quality of patient care for Veterans, to assure confidentiality of care for Veterans, to assure confidentiality of patient record information, and to determine the completeness and accuracy of financial records. This may include but is not limited to case notes, sign in/sign out documents, billing records, program rules for patient rights and responsibilities, program policies or standards of practice. The Contractor shall always comply with the VA Patient's Bill of Rights as set forth in Section 17.33, Title 38, Code of Federal Regulations. 2. The Contractor shall maintain a daily census and provide any required admission and discharge forms within 12 hours. 3. Contractor is expected to provide HCHV staff with regular reports on changes in Veteran's status, including all changes and observations concerning the Veteran's needs. C.DELIVERABLE TIMETABLE. DeliverablesDue Dates Task 1 Deliverables (Housing Available for Rapid Placement)Effective 24 hrs after first Veteran agreed-upon admission date. Task 2 Deliverables (Delivery of Support Services)Required within one week after Veteran resident admission. Task 3 Deliverables (Status Reports)Provide reports quarterly from contract award to Louis Stokes Cleveland VAMC HCHV Staff. D.TRAVEL. VA estimates that Veterans' travel to health, vocational and housing services will be restricted to the local area, accessible by car or public transportation. The Contractor shall be expected to help the Veterans access public transportation, or, if not available and directed to do so by HCHV staff, provide transportation to necessary appointments. The site should be situated within close proximity to public transportation. E.INVOICES AND PAYMENTS. The Contractor shall invoice in monthly payments a month in arrears. Invoices will be paid at the fixed daily rate for days of residential placement already completed with services provided as described in the PWS. Invoices are to be submitted to the assigned Contracting Officer's Representative (COR) for approval. The Contractor submits invoices to the assigned Contracting Officer's Representative (COR) who, in turn, submits them to the Austin Automated Payment Center for approval. Invoices are reconciled and approved then certified by the COR (or designee) for payment by Austin Automated Payment Center. F.SECURITY a)The Contractor must store, transport or transmit VA sensitive information in an encrypted form. No VA sensitive information will be transmitted via computer unless VA approved public key infrastructure (PKI) has been deployed on contractor's network computers to securely and privately exchange data with the VAMC. b)Contractor shall complete and submit the required Business Associate Agreement (BAA) form. The Contractor and VAMC acknowledge that each party will be responsible for the medical records of Veterans when treated at their respective facilities. c)Adequate security measures, e.g., bars on windows and doors and area fenced, in order to provide optimum safety for residents. d)This requirement does not require any contracting owned devices connection to a VA internal (non-public) trusted network. IT requirements for IT security requirements for certification and accreditation (authorization) (C&A) do not apply and that a Security Accreditation Package is not required. e)Contractor and any subcontractors must adhere to the provisions of Public Law 104-191, Health Insurance Portability and Accountability Act (HIPAA) of 1996. This includes both Privacy and Security Rules published by the Department of Health and Human Services (HHS). As required by HIPAA, HHS has promulgated rules governing the use and disclosure of protected health information by covered entities. The covered entity component of VA is the Veterans Health Administration (VHA). In accordance with HIPAA, the Contractor shall be required to enter into a Business Associate Agreement. f)Employees involved in patient care shall have a current background investigation - The level of background investigation commensurate with the required level of access is Special Agreement Checks (SAC). g)To ensure that the individuals providing services under the contract have not engaged in fraud or abuse regarding Sections 1128 and 1128A of the Social Security Act regarding federal health care programs, the Contractor is required to check the Health and Human Services - Office of Inspector General, List of excluded individuals/entities on the OIG website (www.hhs.gov/oig) for each person providing services under the contract. Further the Contractor is required to certify in its proposal that all persons listed in the Contractor's proposal have been compared against the OIG list and are not listed. During the performance of this contract the Contractor is prohibited from using any individual or business listed on the List of Excluded Individual/Entities. h)Secure Fax Language: This language will be included on all fax cover sheets. G.KEY PERSONNEL The contractor shall employ sufficient personnel to carry out the policies, responsibilities, and the program for the facility.
